【摘要】：目的:系統(tǒng)分析中藥治療圍絕經(jīng)期功血的有效性、安全性,以期做出客觀且可信的評價,為圍絕經(jīng)期功血的臨床用藥治療提供有利依據(jù)。討論有關中藥治療圍絕經(jīng)期功血臨床研究中存在的問題,提出解決意見。方法:1全面檢索2005年1月1日至2016年6月30日國內(nèi)外各期刊已發(fā)表或未發(fā)表的有關中藥治療圍絕經(jīng)期功血的臨床研究文獻;2根據(jù)納入、排除標準篩選出有關中藥與西藥治療圍絕經(jīng)期功血的隨機對照試驗(RCT)或半隨機對照(qRCT)的臨床研究文獻;3對符合納入標準的文獻進行質(zhì)量評價及風險偏倚評估;4提取文獻資料,應用RevMan5.2軟件進行Meta分析合并效應量。結(jié)果:本次研究共檢索出文獻475篇,按照納入標準和排除標準,最終納入36篇中文文獻,涉及參與試驗者3563例,應用Jadad量表質(zhì)量評價,35篇低于3分,1篇4分。經(jīng)分析結(jié)果顯示:中藥治療圍絕經(jīng)期功血在臨床有效率、中醫(yī)癥候療效、中醫(yī)癥候積分、血常規(guī)、激素水平、不良反應發(fā)生率、復發(fā)率等方面相比西藥治療有優(yōu)勢,但兩者的止血時間療效和子宮內(nèi)膜厚度治療結(jié)果無明顯差異。結(jié)論:中藥治療圍絕經(jīng)期功血與西藥治療圍絕經(jīng)期功血的療效有顯著差異,中藥治療療效優(yōu)于西藥治療。但本次納入的文獻多數(shù)質(zhì)量偏低,研究內(nèi)容存在很大偏倚,所以當前研究結(jié)果尚不能完全適用于臨床。
[Abstract]:Objective: to analyze the efficacy and safety of traditional Chinese medicine in the treatment of peri-menopausal dysfunctional blood in order to make an objective and reliable evaluation and to provide a favorable basis for the clinical treatment of peri-menopausal dysfunctional blood. This paper discusses the problems existing in the clinical study on the treatment of peri-menopausal dysfunctional blood with Chinese medicine, and puts forward some suggestions. Methods from January 1, 2005 to June 30, 2016, the literature on clinical research on the treatment of peri-menopausal dysfunctional blood by traditional Chinese medicine was searched according to the inclusion of the literature published or not published in various domestic and foreign journals from January 1, 2005 to June 30, 2016. A randomized controlled trial (RCT) or a semi-randomized controlled (qRCT) (qRCT) clinical study on the treatment of menopausal dysfunctional blood with traditional Chinese medicine and western medicine was selected. The quality evaluation and risk bias assessment of the literature in accordance with the inclusion criteria were used to extract the literature data. The combined effect was analyzed by Meta with RevMan5.2 software. Results: a total of 475 articles were retrieved in this study. According to the inclusion criteria and exclusion criteria, 36 Chinese articles were included, involving 3563 participants. The quality evaluation of 35 articles with Jadad scale was less than 3 points and 1 article was 4 points. The results of analysis showed that the clinical effective rate, the curative effect of TCM symptom, the score of TCM symptom, the blood routine, the hormone level, the incidence rate of adverse reaction, the recurrence rate and so on of traditional Chinese medicine were superior to those of western medicine in the treatment of peri-menopausal dysfunctional blood. However, there was no significant difference in hemostatic time and endometrial thickness between the two groups. Conclusion: there is significant difference between Chinese medicine and western medicine in the treatment of peri-menopausal dysfunctional blood, and the curative effect of traditional Chinese medicine is better than that of western medicine. However, most of the literature included in this paper is of low quality and the research content is biased, so the current research results are not fully applicable to clinical practice.
