【摘要】：目的歸納總結(jié)鮑嚴鐘老師治療慢性非細菌性前列腺炎的學(xué)術(shù)思想及臨床經(jīng)驗,對其處方進行現(xiàn)代信息化研究,明確其辨證思路和用藥規(guī)律,從而提高該病的臨床療效,尋找治療新思路,解除男性患者身心痛苦,獲得良好的社會效益,對名醫(yī)經(jīng)驗的繼承和發(fā)揚有重要意義。方法收集和篩選2014年06月--2016年06月在杭州市紅十字會醫(yī)院生殖科鮑嚴鐘門診治療的慢性非細菌性前列腺炎患者的處方并且建立數(shù)據(jù)庫,利用Excel及SPASS統(tǒng)計入組病例中各證型方藥中出現(xiàn)的中藥種類及用藥頻次,從藥味、功效及歸經(jīng)等角度研究鮑嚴鐘老師的用藥規(guī)律。結(jié)果1.總結(jié)7點鮑老治療本病的學(xué)術(shù)思想。2.得到335張?zhí)幏剿婕?46味中藥的用藥頻數(shù)和分類。3.得到60味核心藥物及其藥味、藥性和歸經(jīng)。4.得到常用對藥6組。結(jié)論1.鮑老治療本病的學(xué)術(shù)思想主要包括以人為本,執(zhí)藥平治不妄投;明確前列腺的中醫(yī)定位,理解其病理生理特點;在前列腺觸診下,創(chuàng)造性的把本病分成七個類型:在對本病辨證的基礎(chǔ)上,尤重虛瘀兩端論治;重視并發(fā)癥治療,包括性功能障礙和不育癥;重視中醫(yī)外治法;提出“慎起居、暢情志、禁煙忌酒”自身調(diào)護要求。2.鮑老治療本病從用藥頻次看常用的有60味核心藥物:甘草、炒枳殼、川牛膝、穿山甲、當歸、雞內(nèi)金、路路通、太子參、敗醬草、黃芪、虎杖、茯苓、王不留行、臺烏藥、生地黃、萆參、覆盆子、鹿角、人參、肉蓯蓉、丹參、菟絲子、淫羊藿、黃柏、制首烏、夜交藤、仙茅、葫蘆巴、枸杞子、女貞子、炒白芍、黨參、澤瀉、蒼術(shù)、補骨脂、知母、神曲、佩蘭、懷山藥、蛇床子、陳皮、澤蘭、巴戟天、山茱萸、川樸、丹皮、川芎、仙鶴草、酸棗仁、白術(shù)、煅龍骨、半枝蓮、黃芩、藿香、車前子、續(xù)斷、紅花、香附、炒米仁、蒲公英。3.從藥物功效統(tǒng)計結(jié)果看常用的有補虛藥、活血祛瘀藥、清熱藥、利水滲濕藥等。4.從性味歸經(jīng)上統(tǒng)計來看,常用入肝、脾、腎經(jīng)的藥,藥性上寒溫并用,且在藥味上使用較多的為苦味藥、甘味藥和辛味藥等。5.常用的藥對包括黃芪-人參,何首烏-當歸,敗醬草-虎杖,柴胡-郁金,仙茅-淫羊藿,牛膝-枳殼。
[Abstract]:Objective to summarize the academic thought and clinical experience of teacher Bao Yanzhong in the treatment of chronic non-bacterial prostatitis, to study its prescription with modern information technology, and to clarify its dialectical thinking and drug use law, so as to improve the clinical efficacy of the disease. It is of great significance for the famous doctors to inherit and carry forward their experiences by seeking a new way of treatment, relieving the mental and physical pain of male patients and obtaining good social benefits. Methods from June 2014 to June 2016, prescriptions of patients with chronic non-bacterial prostatitis treated in Department of Reproductive Medicine of Hangzhou Red Cross Hospital were collected and selected, and a database was established. Excel and SPASS were used to calculate the types and frequency of traditional Chinese medicine in each syndromes. From the aspects of medicinal taste, efficacy and return to the meridian, teacher Bao Yanzhong's drug use was studied. Result 1. Summing up 7 points Baolao treatment of this disease academic thinking. 2. The drug frequency and classification of 146 kinds of traditional Chinese medicine were obtained from 335 prescriptions. Get 60 kinds of core drugs and their flavor, medicinal properties and Gui Jing. 4. 6 groups of common antidrugs were obtained. Conclusion 1. Bao Lao's academic thoughts on the treatment of this disease mainly include taking the people as the basis, holding drugs to treat the patients without making a mistake; defining the position of the prostate in TCM and understanding its pathophysiological characteristics; and under the condition of palpation of the prostate, Creatively divide the disease into seven types: on the basis of differentiation of symptoms and signs, especially on the treatment of deficiency and blood stasis; attach importance to the treatment of complications, including sexual dysfunction and infertility; attach importance to the external treatment of traditional Chinese medicine; Ban smoking and avoid alcohol "self-regulation and protection requirements. 2." There are 60 kinds of core drugs commonly used in the treatment of this disease: liquorice, fried Fructus aurantii, Achyranthes bidentata, pangolin, Angelica sinensis, Neijin, Luotong, Radix Pseudostellariae, Astragalus chinensis, Astragalus membranaceus, Polygonum cuspidatum, Poria cocos, Wang Buliu, Taiwu, Radix Rehmanniae, Radix Diopsis, Raspberry, antler, Ginseng, Cistanche, Salvia miltiorrhiza, Cuscuta, Herba Epimedii, Cortex Phellodendri, Radix Polygoni, Rhizoma Lycii, Cucurbita, Fructus Lycii, Fructus Ligustrum, Stir-fried Radix Paeoniae Alba, Codonopsis, Radix Atractylodes, Psoralen, Anemarrhena, Shenqu, Peran, Huai yam, Cnidium, Chen Pei, Zelan, Morinda officinalis, Fructus Corni, Chuanpu, Danpi, Chuanxiong, Xianjao, Zizuren, Atractylodes macrocephala, Forge keel, Scutellaria, Pogostemon, Plantago, Radix Scutellariae, Pogostemon, Plantago, Dipsacus, Carthami, Fragrant, fried rice, dandelion. 3. From the statistical results of drug efficacy, the commonly used drugs, such as tonifying deficiency, activating blood circulation and removing blood stasis, clearing heat, promoting water and infiltrating dampness, and so on. 4. According to statistics, commonly used drugs into the liver, spleen, kidney meridian, the combination of cold and warm medicine, and in the taste of the use of more bitter medicine, sweet medicine and xingwei medicine, and so on. 5. Common drug pairs include Astragalus-Ginseng, Polygonum multiflorum-Angelica sinensis, Patrinia chinensis-Polygonum cuspidatum, Bupleurum-Tulip, Festuca-Epimedium, Achyranthes bidentata-Fructus Aurantii.
