【摘要】：目的探討還腦益聰方(HYD)改善腦卒中后認(rèn)知功能障礙的療效、安全性及其可能作用機(jī)制。方法 100例腦卒中后認(rèn)知功能障礙(PCI)的患者隨機(jī)分為兩組:對照組(50例)給予常規(guī)治療,吡拉西坦片0.8/次,3次/d,口服;鹽酸多奈哌齊膠囊,5 mg/次,1次/d,4 w后增至10 mg/次,1次/d。觀察組(50例)在對照組治療基礎(chǔ)上加用HYD中藥免煎顆�？诜�。兩組均連續(xù)治療12 w。治療前后檢測兩組患者認(rèn)知功能[蒙特利爾認(rèn)知評估量表(MoCA)、簡易智能檢查量表(MMSE)、日常生活活動能力(ADL)評分]及生化指標(biāo)[總膽固醇(TC)、甘油三酯(TG)、同型半胱氨酸(Hcy)、白細(xì)胞介素(IL)-6、超氧化物歧化酶(SOD)、丙二醛(MDA)和乙酰膽堿酯酶(AchE)]。結(jié)果治療12 w后,兩組患者的MoCA、MMSE及ADL評分、TC、TG、Hcy、IL-6、SOD、MDA和AchE水平均較治療前明顯改善(P0.05或P0.01),且觀察組患者改善更顯著(P0.05或P0.01)�？诜﨟YD中藥免煎顆粒未出現(xiàn)明顯的不良反應(yīng)。結(jié)論 HYD能顯著改善腦卒中后認(rèn)知功能障礙,提高患者日常生活自理能力,安全性好,作用機(jī)制可能與其調(diào)脂、抗炎、抗動脈粥樣硬化及提高乙酰膽堿活性有關(guān)。
[Abstract]:Objective to investigate the efficacy, safety and possible mechanism of Huanao Yicong recipe (HYD) in improving cognitive dysfunction after stroke. Methods one hundred patients with cognitive dysfunction (PCI) after stroke were randomly divided into two groups: the control group (50 cases) received routine treatment, Piracetam was given 0.8d / d, and Donepezil capsule was increased to 10 mg/ times per day after 4 weeks. Observation group (50 cases) in the control group on the basis of treatment plus HYD Chinese medicine decoction oral. Both groups were treated continuously for 12 weeks. Cognitive function was measured before and after treatment [Montreal Cognitive Assessment scale (MoCA), mini-intelligence scale (MMSE), activity of daily living (ADL) score] and biochemical indexes [total cholesterol (TC), triglyceride (TG), homocysteine (Hcy),] and biochemistry index [total cholesterol (TC), triglyceride (TG), homocysteine (Hcy),] Interleukin (IL)-6, superoxide dismutase (SOD), malondialdehyde (MDA) and acetylcholinesterase (AchE)]. Results after 12 weeks of treatment, the levels of MMSE and ADL in both groups were significantly improved compared with those before treatment (P0.05 or P0.01), and the improvement was more significant in the observation group (P0.05 or P0.01). There was no obvious adverse reaction in oral HYD granules. Conclusion HYD can significantly improve cognitive dysfunction and self-care ability in daily life of patients with stroke. The mechanism of HYD may be related to its lipid regulation, anti-inflammation, anti-atherosclerosis and increased acetylcholine activity.
