【摘要】：目的：本課題旨在通過(guò)觀察定痛湯在治療青春期寒凝血瘀型原發(fā)性痛經(jīng)的臨床療效,客觀地評(píng)價(jià)定痛湯治療本病的有效性及其對(duì)血清CA125的影響,以期對(duì)定痛湯療效有進(jìn)一步的認(rèn)識(shí)。方法：采用隨機(jī)分組方法將60例寒凝血瘀型原發(fā)性痛經(jīng)的青春期患者分為治療組和對(duì)照組2組,每組各30例。治療組口服定痛湯治療,對(duì)照組口服“月月舒”痛經(jīng)寶顆粒治療。通過(guò)觀察患者用藥前和用藥1個(gè)療程(3個(gè)月經(jīng)周期)后的臨床癥狀改善情況,采用積分法,比較患者的癥狀和血清CA125結(jié)果。結(jié)果：1.兩組患者共60例,治療前在年齡、病程、痛經(jīng)程度、中醫(yī)證候積分及血清CA125值均無(wú)明顯差異(P0.05)；治療一個(gè)療程后,兩組痛經(jīng)程度分值均下降,治療組下降更明顯,且組間比較差異有顯著性(P0.05);2.痛經(jīng)程度總療效比較：治療一個(gè)療程后,治療組痛經(jīng)程度改善總有效率為90%,對(duì)照組總有效率為86.67%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)；3.中醫(yī)證候積分比較：治療一個(gè)療程后,治療組與對(duì)照組中醫(yī)證候積分較治療前均有顯著差異(P0.05),組間比較有顯著差異(P0.05);4.中醫(yī)證候療效比較：治療一個(gè)療程后,治療組總有效率為90.0%,對(duì)照組總有效率為73.33%,組間比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)；5.血清CA125值：治療一個(gè)療程后,兩組血清CA125值均下降,治療組更明顯,但組間比較無(wú)明顯統(tǒng)計(jì)學(xué)差異(P0.05)。結(jié)論：經(jīng)過(guò)本課題研究發(fā)現(xiàn)定痛湯療效優(yōu)于“月月舒”痛經(jīng)寶顆粒,顯示了中醫(yī)辨證治療本病的優(yōu)越性,有一定的臨床應(yīng)用價(jià)值。
[Abstract]:Objective: to observe the clinical effect of Dingtong decoction on primary dysmenorrhea of puberty cold coagulation and blood stasis type, and to evaluate objectively the efficacy of Dingtong decoction in treating this disease and its effect on serum CA125. With a view to a further understanding of the efficacy of Dingtong decoction. Methods: 60 cases of primary dysmenorrhea with cold coagulation and blood stasis were randomly divided into treatment group and control group with 30 cases in each group. The treatment group was treated with Dingtong decoction and the control group was treated with Yueyuesu Tongjingbao granule. By observing the improvement of clinical symptoms before and after one course of treatment (3 menstrual cycles), the patients' symptoms and the results of serum CA125 were compared by integral method. The result is 1: 1. There were no significant differences in age, course of disease, degree of dysmenorrhea, TCM syndrome score and serum CA125 between the two groups before treatment (P0.05). After one course of treatment, the scores of dysmenorrhea degree decreased in both groups, especially in the treatment group. There was significant difference between groups (P0.05). Comparison of the total curative effect of dysmenorrhea: after a course of treatment, the total effective rate of improving the degree of dysmenorrhea in the treatment group was 90 and the total effective rate in the control group was 86.67. The difference between the two groups was statistically significant (P0.05). Comparison of TCM syndromes: after a course of treatment, there were significant differences in TCM syndrome scores between the treatment group and the control group (P0.05) and significant difference (P0.05) between the treatment group and the control group (P0.05). Comparison of TCM syndromes efficacy: after a course of treatment, the total effective rate of the treatment group was 90.0 and the total effective rate of the control group was 73.33. The difference between the two groups was statistically significant (P0.05). Serum CA125 value: after a course of treatment, the serum CA125 levels of the two groups decreased, especially in the treatment group, but there was no significant difference between the two groups (P0.05). Conclusion: Dingtong decoction is superior to "Yueyueshu" Tongjingbao granule through the research of this subject, which shows the superiority of TCM in treating this disease and has certain clinical application value.
