【摘要】：目的 觀察復(fù)元舒筋湯結(jié)合頜枕牽引治療寰樞關(guān)節(jié)半脫位患者的臨床療效,分析復(fù)元舒筋湯治療寰樞關(guān)節(jié)半脫位的臨床功效。方法 收集2014年9月至2016年6月于蘇州市中醫(yī)醫(yī)院診治的45例寰樞關(guān)節(jié)半脫位患者,隨機(jī)分為治療組及對照組。治療組予復(fù)元舒筋湯+領(lǐng)枕牽引治療,對照組予單純領(lǐng)枕牽引治療,分別從疼痛、功能、活動度、影像學(xué)指標(biāo)評估患者病情,并通過觀察記錄患者治療前、治療14日后NDI指數(shù)、VAS評分、頸椎屈伸旋轉(zhuǎn)活動度、影像學(xué)(VBLADS、ADI)四項指標(biāo),對復(fù)元舒筋湯的臨床療效進(jìn)行分析評估與研究。結(jié)果 (1)治療組與對照組治療前兩組比較,NDI指數(shù)、VAS評分、頸椎屈伸旋轉(zhuǎn)活動度、影像學(xué)指標(biāo)(VBLADS、ADI)無統(tǒng)計學(xué)差異(P0.05)。(2)治療組與對照組治療后比較,NDI指數(shù)、VAS評分有統(tǒng)計學(xué)差異(P0.05),頸椎屈伸旋轉(zhuǎn)活動度、影像學(xué)指標(biāo)(VBLADS、ADI)無統(tǒng)計學(xué)差異(P0.05)。(3)治療組與對照組各自治療前后自身比較,NDI指數(shù)、VAS評分、頸椎屈伸旋轉(zhuǎn)活動度、VBLADS均有統(tǒng)計學(xué)差異(P0.05),ADI無統(tǒng)計學(xué)差異(P0.05)。結(jié)論 (1)復(fù)元舒筋湯結(jié)合領(lǐng)枕牽引治療寰樞關(guān)節(jié)半脫位,在緩解頸部疼痛、改善頸椎功能方面優(yōu)于單純頜枕牽引的治療。(2)復(fù)元舒筋湯結(jié)合頜牽引在改變寰樞關(guān)節(jié)半脫位患者的頸椎屈曲旋轉(zhuǎn)活動度、影像學(xué)指標(biāo)(VBLADS、ADI)方面,并沒有發(fā)現(xiàn)有較單純領(lǐng)枕牽引更有臨床意義的改變。(3)復(fù)元舒筋湯有助于緩解寰樞關(guān)節(jié)半脫位的疼痛癥狀。
[Abstract]:Objective to observe the clinical effect of Fuyuan Shujin decoction combined with maxillary pillow traction in the treatment of atlantoaxial subluxation and to analyze the clinical efficacy of Fuyuan Shujin decoction in the treatment of atlantoaxial subluxation. Methods from September 2014 to June 2016, 45 patients with atlantoaxial subluxation were randomly divided into treatment group and control group. The treatment group was treated with Fuyuan Shujin decoction and the control group was treated with simple neck traction. The patients' condition was evaluated from pain, function, activity and imaging index, and the patients were observed and recorded before treatment. After 14 days of treatment, the clinical efficacy of Fuyuan Shujin decoction was evaluated and evaluated by NDI index and VAS score, cervical flexion and extension rotation, and imaging (VBLADS). Results (1) compared with the control group before treatment, the NDI index and VAS score were compared between the treatment group and the control group. There was no significant difference in imaging index (VBLADS) (P0.05) between the treatment group and the control group (P0.05). There was significant difference in VAS score between the treatment group and the control group (P0.05). There was no significant difference in imaging indexes (P0.05) between the treatment group and the control group before and after treatment. There was no significant difference between the treatment group and the control group before and after treatment (P0.05). There was no significant difference between the treatment group and the control group (P0.05). Conclusion (1) Fuyuan Shujin decoction combined with neck pillow traction in the treatment of atlantoaxial subluxation can relieve neck pain. (2) Fuyuan Shujin decoction combined with maxillary traction can change the cervical flexion and rotation and imaging index (VBLADS ADI) in patients with atlantoaxial subluxation. There were no clinical significance changes compared with simple neck traction. (3) Fuyuan Shujin decoction was helpful to relieve the pain symptoms of atlantoaxial joint subluxation.
