【摘要】：目的臨床穴位注射治療三叉神經(jīng)痛的療效暫無確切證據(jù),本次系統(tǒng)評價擬用循證醫(yī)學(xué)的方法對穴位注射治療三叉神經(jīng)痛的療效進行評估。方法參照Cochrane協(xié)作網(wǎng)規(guī)定的系統(tǒng)評價原則,制定檢索策略,全面檢索國內(nèi)外有關(guān)穴位注射治療三叉神經(jīng)痛的RCTs。計算機檢索清華同方CNKI數(shù)據(jù)庫、PubMed、CBM等數(shù)據(jù)庫。制定納入標準,并按照診斷標準進行文獻篩選。然后提取數(shù)據(jù),再按Cochrane協(xié)作網(wǎng)的“偏倚風(fēng)險評估標準”對篩選出的文獻進行質(zhì)量評級。用RevMan5.3軟件對數(shù)據(jù)進行Meta分析,并用GradePro3.6軟件對結(jié)局證據(jù)質(zhì)量等級進行評價。結(jié)果最后納入10個研究,共715例患者。有7個研究報告了(n=491)穴位注射配合針刺與針刺比較的有效率,合并效應(yīng)量RR=1.22,95%CI[1.13,1.30].7個研究中,3個研究是用中成藥進行穴位注射,RR=1.22,95%CI[1.09,1.36]; 3個研究是維生素+局麻藥進行穴位注射,RR=1.19,95%CI[1.08,1.31];1個研究是維生素進行穴位注射,Meta分析RR=1.33,95%CI[1.04,1.72].顯示穴位注射配合針刺療效高于單純針刺的療效。2篇研究是穴位注射與口服卡馬西平對照,RR=1.12,95%CI[0.94,1.33],合并效應(yīng)量95%CI跨越無效線,說明兩個研究合并結(jié)果無統(tǒng)計學(xué)意義,敏感性分析結(jié)果與Meta分析不一致,該結(jié)果可靠性較低,不能說明穴位注射療效是否優(yōu)于卡馬西平。1篇文章報告了穴位注射聯(lián)合電針與單純電針的對照,兩組的差別有統(tǒng)計學(xué)意義(P0.05)。提示穴位注射聯(lián)合電針的效果高于單純使用電針治療的療效。但其Meta分析RR的95%CI跨無效線,不能提示穴位注射配合電針的療效比單純電針高。GRADE證據(jù)質(zhì)量評價,穴位注射配合針刺VS單純針刺、穴位注射VS卡馬西平、穴位注射聯(lián)合電針VS電針質(zhì)量等級均為D級(very low)。10篇研究均未報告穴位注射療法的不良反應(yīng)。結(jié)論穴位注射治療三叉神經(jīng)痛可能具有一定療效,未報到不良反應(yīng)。但由于文獻質(zhì)量較低,納入的文獻數(shù)量較少,語種僅為中文,使得結(jié)論的可靠性受到影響,且GRADE證據(jù)質(zhì)量評級較低,結(jié)果需謹慎對待。因此有必要進行高質(zhì)量的臨床RCTs,為穴位注射治療三叉神經(jīng)痛的療效評價提供可信度更高的臨床證據(jù)。穴位注射治療TN的藥物選擇種類不一,不太規(guī)范。需要更多相關(guān)研究,以規(guī)范穴位注射治療TN的藥物選擇,提供更優(yōu)的藥物注射方案。
[Abstract]:Objective to evaluate the efficacy of acupoint injection in the treatment of trigeminal neuralgia. Methods according to the systematic evaluation principle stipulated by Cochrane Cooperative Network, the retrieval strategy was formulated, and the RCTs of acupoint injection in treating trigeminal neuralgia at home and abroad were comprehensively searched. To search the database of Tongfang CNKI in Tsinghua University by computer. The inclusion criteria were established and the literature was screened according to the diagnostic criteria. Then the data are extracted and the selected documents are rated according to the bias risk assessment criteria of Cochrane collaboration. RevMan5.3 software was used to analyze the data by Meta, and GradePro3.6 software was used to evaluate the quality of the outcome evidence. Results A total of 715 patients were enrolled in 10 studies. Seven studies have reported the effective rate of point injection combined with acupuncture in comparison with acupuncture. Among the 7 studies, 3 studies were done with Chinese patent medicine for acupoint injection (RRX 1.2295CI [1.09N1.36]; three studies were for vitamin local anesthetics for acupoint injection of RRN 1.1995CI [1.081.31]; one study was for point injection meta analysis of RRN 1.3395CI [1.04m1.72]. The results showed that the therapeutic effect of acupuncture combined with acupoint injection was higher than that of acupuncture alone. The results of the two studies showed that the combination of acupoint injection and oral carbamazepine had no statistical significance. The combined effect amount of 95%CI crossed the invalid line, and there was no significant difference between the two studies. The result of sensitivity analysis was inconsistent with that of Meta analysis, and the reliability of the result was low, which could not explain whether the curative effect of acupoint injection was better than that of carbamazepine. 1. The comparison between acupoint injection combined with electroacupuncture and simple electroacupuncture was reported. The difference between the two groups was statistically significant (P0.05). The results suggest that the effect of acupoint injection combined with electroacupuncture is higher than that of electroacupuncture alone. However, the Meta analysis of the 95%CI line of RR did not suggest that the efficacy of acupoint injection combined with electroacupuncture was higher than that of electroacupuncture alone. The quality of evidence was higher than that of electroacupuncture alone. Point injection combined with acupuncture with vs was simple acupuncture, and acupoint injection of vs carbamazepine was more effective than that of electroacupuncture alone. The quality grade of acupoint injection combined with electroacupuncture vs electroacupuncture was D grade (very low). 10 studies did not report the adverse reactions of acupoint injection therapy. Conclusion Acupoint injection may be effective in the treatment of trigeminal neuralgia. However, the reliability of the conclusions is affected because of the low quality of the literature, the small number of documents included and the Chinese language, and the low quality rating of GRADE evidence, so the results need to be treated with caution. Therefore, it is necessary to carry out high quality clinical RCTs to provide more reliable clinical evidence for the evaluation of the efficacy of acupoint injection in the treatment of trigeminal neuralgia. The drug choice of point injection for TN is different and not very standard. More research is needed to standardize the drug selection of point injection therapy for TN and to provide a better drug injection protocol.
