【摘要】：目的:探討膈下逐瘀湯對子宮不規(guī)律出血患者血清炎癥因子及HMBG1水平影響。方法:收集醫(yī)院婦科收治的子宮不規(guī)律出血患者64例,隨機分為試驗組和對照組,各32例。對照組予以達(dá)那唑膠囊400~800mg,每日1次/d;試驗組在對照組的基礎(chǔ)上予以膈下逐瘀湯1劑/d,兩組患者均連續(xù)治療3個月。治療結(jié)束后,比較患者臨床療效、性激素水平、血清炎癥因子、血清基質(zhì)金屬蛋白酶-9、金屬蛋白酶組織抑制因子-2、高遷移率族蛋白1以及不良反應(yīng)發(fā)生情況。結(jié)果:治療后與對照組總有效率68.75%相比,試驗組臨床總有效率為90.63%較高(P0.05)。治療后兩兩組卵泡刺激素、促黃體生成素、雌二醇、血清孕酮以及泌乳素水平降低(P0.05);與對照組相比,試驗組卵泡刺激素、促黃體生成素、雌二醇、血清孕酮以及泌乳素水平較低(P0.05)。兩組血清基質(zhì)金屬蛋白酶-9、金屬蛋白酶組織抑制因子-2以及高遷移率族蛋白1水平降低(P0.05);與對照組相比,試驗組血清基質(zhì)金屬蛋白酶-9、金屬蛋白酶組織抑制因子-2以及高遷移率族蛋白1較低(P0.05)。兩組白介素-6以及腫瘤壞死因子α水平降低(P0.05);與對照組相比,試驗組白介素-6以及腫瘤壞死因子α較低(P0.05),兩組不良反應(yīng)比較,差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論:膈下逐瘀湯對子宮不規(guī)律出血的療效顯著,能夠抑制炎癥反應(yīng),下調(diào)HMBG1水平,安全性較高,適宜臨床應(yīng)用推廣。
[Abstract]:Objective: to investigate the effect of Gexiazhuyu decoction on serum inflammatory factors and HMBG1 levels in patients with irregular uterine bleeding. Methods: 64 patients with irregular uterine bleeding were randomly divided into experimental group and control group, 32 cases in each group. The control group was given Danazol capsule 400 mg / d once a day, and the experimental group was given one dose of Gexia Zhuyu decoction on the basis of the control group. The patients in both groups were treated continuously for 3 months. After treatment, the clinical efficacy, sex hormone level, serum inflammatory factor, serum matrix metalloproteinase-9, tissue inhibitor of metalloproteinase-2, high mobility group protein-1 and adverse reactions were compared. Results: compared with the control group, the total effective rate of the experimental group was 90.63% (P 0.05). The levels of follicle stimulating hormone, luteinizing hormone, estradiol, serum progesterone and prolactin were decreased in two groups after treatment (P0.05). Serum progesterone and prolactin levels were lower (P0.05). The levels of matrix metalloproteinase-9, tissue inhibitor of metalloproteinase-2 and high mobility group protein-1 were decreased in both groups (P0.05). The serum levels of matrix metalloproteinase-9, tissue inhibitor of metalloproteinase -2 and high mobility group protein 1 in the experimental group were lower than those in the control group (P0.05). The levels of interleukin -6 and tumor necrosis factor 偽 decreased in the two groups (P0.05); compared with the control group, the levels of interleukin -6 and tumor necrosis factor 偽 were lower in the experimental group (P0.05), and there was no significant difference in adverse reactions between the two groups (P0.05). Conclusion: Gexiazhuyu decoction has a significant effect on irregular uterine bleeding and can inhibit inflammation and down-regulate HMBG1 level. It is safe and suitable for clinical application.
