【摘要】：目的探討大株紅景天注射液對急性重度一氧化碳中毒患者心肌損傷的影響及可能作用機制。方法104例急性重度一氧化碳中毒患者隨機分成對照組和治療組各52例,對照組給予西醫(yī)常規(guī)治療,治療組在常規(guī)治療基礎上加用大株紅景天注射液10 ml靜脈滴注,每日1次,治療2周。兩組于治療前及治療6 h、14天檢測血清心型脂肪酸結(jié)合蛋白(H-FABP)、肌紅蛋白(Myo)變化,于治療前后檢測患者血一氧化氮(NO)、一氧化氮合酶(NOS)及誘導型一氧化氮合酶(iNOS)水平,記錄簡易精神狀態(tài)檢查量表(MMSE)評分和左心室射血分數(shù)(LVEF)水平變化,觀察主要心臟不良事件發(fā)生率。采用Pearson相關(guān)檢驗分析患者入院時血清H-FABP與LVEF的相關(guān)性。結(jié)果兩組患者治療6 h時血清H-FABP和Myo水平較治療前升高,但治療組低于對照組(P0.05),兩組治療14天時血清H-FABP和Myo水平均較本組治療前及治療6 h降低(P0.05)。治療后兩組血NO、NOS、iNOS水平較治療前降低(P0.05),LVEF水平和MMSE評分較治療前升高,治療后MMSE評分治療組優(yōu)于對照組(P0.05)。治療組主要心臟不良事件發(fā)生率明顯低于對照組(P0.05)。104例患者入院時血清H-FABP水平與其LVEF呈負相關(guān)(r=-0.235,P=0.017)。結(jié)論大株紅景天注射液能減少急性重度一氧化碳中毒心肌損傷及心臟不良事件的發(fā)生,改善心功能,可能與降低血H-FABP、Myo、NO、NOS、iNOS水平有關(guān)。
[Abstract]:Objective to investigate the effect and possible mechanism of large strain Rhodiola injection on myocardial injury in patients with acute severe carbon monoxide poisoning. Methods 104 patients with acute severe carbon monoxide poisoning were randomly divided into control group (n = 52) and treatment group (n = 52). The control group was treated with routine western medicine. Treatment 2 weeks. Serum levels of cardiac fatty acid binding protein (H-FABP), myoglobin (Myo), nitric oxide (no), nitric oxide synthase (NOS) and inducible nitric oxide synthase (iNOS) were measured before and after treatment in both groups. The changes of MMSE score and left ventricular ejection fraction (LVEF) were recorded to observe the incidence of major adverse cardiac events. The correlation between serum H-FABP and LVEF was analyzed by Pearson correlation test. Results the levels of serum H-FABP and Myo in the two groups increased at 6 h after treatment, but were lower in the treatment group than in the control group (P0.05), and the levels of H-FABP and Myo in the two groups decreased at 14 days after treatment (P0.05). After treatment, the levels of NOSN iNOS in the two groups were lower than those before treatment (P0.05) and the scores of LVEF and MMSE were higher in the treatment group than in the control group (P0.05). The incidence of major adverse cardiac events in the treatment group was significantly lower than that in the control group (P0.05). There was a negative correlation between the serum H-FABP level and LVEF in 104 patients (r = 0.235). Conclusion large strain Rhodiola injection can reduce myocardial injury and adverse cardiac events in acute severe carbon monoxide poisoning, and improve cardiac function, which may be related to the decrease of serum H-FABP- Myotropium novo NOSINOS level in patients with acute carbon monoxide poisoning.
