【摘要】：目的動態(tài)觀察手三里穴位在針刺條件下的超聲影像學(xué)解剖特征,并初步探討針灸療效機制的解剖學(xué)依據(jù)。方法選取健康志愿者15名,采用高頻超聲動態(tài)觀察針灸針刺入雙側(cè)手臂手三里穴位的過程,并記錄針刺雙側(cè)手三里穴后的針尖的解剖學(xué)位置,以及針刺過程中與得氣感相關(guān)聯(lián)的解剖學(xué)結(jié)構(gòu)。結(jié)果針刺15名受試者雙側(cè)手三里穴共30個穴位點均得氣,針尖位置:橈側(cè)腕伸肌13個,橈側(cè)腕伸肌與旋后肌間筋膜9個,旋后肌淺層2個,旋后肌內(nèi)筋膜4個,旋后肌深層1個,橈骨表面1個。28個(28/30,93.33%)針尖位于筋膜旁及筋膜集中處獲得較強得氣感、2個(2/30,6.67%)針尖位于較淺層即可獲得較強得氣感(P0.05);對針灸針行提插、捻轉(zhuǎn)等施針操作時,可觀察到周圍的結(jié)締組織纏繞針體,并可帶動鄰近的組織;高頻超聲可以清楚顯示針尖點附近筋膜內(nèi)的細(xì)小神經(jīng)。結(jié)論高頻超聲可以實時、動態(tài)、清楚地顯示針灸針尖附近筋膜及筋膜內(nèi)的細(xì)小神經(jīng)分布,表明針灸機制可能與筋膜結(jié)締組織及筋膜內(nèi)神經(jīng)相關(guān),為進一步闡明針灸機制的原理提供了解剖學(xué)依據(jù)。超聲可同時提供組織形態(tài)學(xué)和生物力學(xué)信息,并可對針灸位置進行定量測定,有望為闡明穴位的本質(zhì)以及針灸的標(biāo)準(zhǔn)化提供新的方法。
[Abstract]:Objective to observe the anatomic characteristics of ultrasonic imaging at the point of hand Sanli under acupuncture, and to explore the anatomical basis of the mechanism of acupuncture and moxibustion. Methods Fifteen healthy volunteers were selected to observe the process of acupuncture at Sanli acupoint of bilateral arms by using high frequency ultrasound, and to record the anatomic position of the tip of the needle after acupuncture at San-li point of bilateral hand. And the anatomic structure associated with the feeling of qi in the process of acupuncture. Results Acupuncture was performed at 30 acupoints of bilateral hand Sanli points in 15 subjects. The position of acupuncture tip was 13 extensor Carpi radialis muscle, 9 fascia between extensor Carpi radialis and reflex muscle, 2 superficial layer of supination muscle and 4 endofascial fascia of pronator muscle. The deep layer of circumflex muscle is 1, the surface of radius is 1. 28 (28 / 30 ~ 93.33%) tips are located near the fascia and the fascia concentration, and 2 (2 / 30 ~ 6.67%) tips are located in the superficial layer to get a stronger feeling of qi (P0.05). When the acupuncture needle is lifted and inserted, the needle is twirled. The surrounding connective tissue can be seen winding the needle body and the adjacent tissue can be driven, and the fine nerve in the fascia near the needle tip can be clearly displayed by high frequency ultrasound. Conclusion High-frequency ultrasound can display the distribution of fine nerves in the fascia and fascia near the needle tip in real time and dynamically, indicating that the mechanism of acupuncture and moxibustion may be related to the connective tissue of fascia and the nerve in fascia. It provides anatomic basis for further elucidation of the mechanism of acupuncture and moxibustion. Ultrasound can provide both histomorphology and biomechanical information and quantitatively determine the position of acupuncture and may provide a new method for elucidating the essence of acupoints and standardization of acupuncture and moxibustion.
