清腦益元湯對(duì)缺血性中風(fēng)患者急性期NSE、S100B表達(dá)的影響

【摘要】:目的:觀察清腦益元湯對(duì)缺血性中風(fēng)急性期血清NSE、S100B表達(dá)的影響,探討其對(duì)缺血性中風(fēng)患者的治療效果及作用機(jī)制。方法:采取隨機(jī)原則,選取廣西中醫(yī)藥大學(xué)第一附屬醫(yī)院腦病科從2016年04月至2016年12月的住院患者共60例,隨機(jī)分為治療組與對(duì)照組,每組各30例。對(duì)照組予以單純西醫(yī)常規(guī)治療,治療組在對(duì)照組基礎(chǔ)上加用清腦益元湯。采用酶聯(lián)免疫吸附法(ELISA)檢測(cè)兩組治療前及治療后第3天、第7天、第14天血清NSE、S100B表達(dá)水平,評(píng)估兩組患者神經(jīng)功能缺損程度評(píng)分、中醫(yī)證候積分、生活能力狀態(tài)及臨床療效。結(jié)果:(1)治療前,兩組神經(jīng)功能缺損評(píng)分、中醫(yī)臨床證候積分、生活能力狀態(tài)及血清NSE、S100B水平比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);(2)治療后,治療組與對(duì)照組神經(jīng)功能缺損評(píng)分、中醫(yī)臨床證候積分及血清NSE、S100B表達(dá)水平比較均較前下降,且治療組下降更明顯(P0.05或P0.01);(3)兩組治療前神經(jīng)功能積分與血清NSE、S100B表達(dá)水平呈正相關(guān)(相關(guān)系數(shù):NSE:r=0.925,r=0.958;S100B:r=0.891,r=0.870,P均0.01),血清NSE與S100B表達(dá)水呈正相關(guān)(兩組相關(guān)系數(shù):r=0.981,r=0.870,P均0.01)。(4)生活能力狀態(tài)較前得到明顯改善(P0.01),治療組改善更顯著(P0.05)。治療組臨床總有效率明顯高于對(duì)照組,兩組臨床總有效率分別為90%和76.7%,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:清腦益元湯能明顯減輕缺血性中風(fēng)患者急性期神經(jīng)功能缺損程度,其機(jī)制可能與下調(diào)血清NSE、S100B表達(dá)水平有關(guān)。清腦益元湯值得在臨床推廣應(yīng)用。
[Abstract]:Objective: to observe the effect of Qingnao Yiyuan decoction (QNYD) on the expression of serum NSE S100B in acute ischemic stroke, and to explore the therapeutic effect and mechanism of Qingnao Yiyuan decoction on ischemic apoplexy. Methods: a total of 60 inpatients from the Department of Encephalopathy in the first affiliated Hospital of Guangxi University of traditional Chinese Medicine from April 2016 to December 2016 were randomly divided into treatment group and control group with 30 cases in each group. The control group was treated with routine western medicine and the treatment group was treated with Qingnao Yiyuan decoction on the basis of the control group. Enzyme linked immunosorbent assay (Elisa) was used to detect the serum NSES100B expression before treatment and on the 3rd, 7th and 14th day after treatment, and to evaluate the neurological deficit score, TCM syndromes score, living ability status and clinical efficacy of the two groups. Results: (1) before treatment, there was no significant difference between the two groups in neurological deficit score, TCM clinical syndrome score, living ability status and serum NSE S100B level (P0.05). After treatment, the nerve function defect scores of the treatment group and the control group were not significantly different (P0.05), and no significant difference was found between the treatment group and the control group (P0.05). The scores of TCM clinical syndrome and the expression level of serum NSEP S100B were lower than those before. In the treatment group, there was a more significant decrease (P0.05 or P0.01); (3). There was a positive correlation between the scores of nerve function and the expression of serum NSE S100B in the two groups (the correlation coefficient was 0: 0. 925); (0. 925), the correlation coefficient was 0. 858 S0. 891B: R0. 870 (P0.01), and the serum NSE was positively correlated with the water expression of S100B (the correlation coefficient of the two groups was 0. 981rngr, 0. 981rr, P 0. 01). (4), and there was a positive correlation between the expression of serum NSE and the expression of S100B in the treatment group (P < 0. 01). The state of the treatment group was significantly improved (P0.01), and the improvement of the treatment group was more significant (P0.05). The total clinical effective rate of the treatment group was significantly higher than that of the control group, the total effective rate of the two groups was 90% and 76.7% respectively, the difference was statistically significant (P0.05). Conclusion: Qingnao Yiyuan decoction can obviously reduce the degree of nerve function defect in patients with ischemic stroke in acute stage, and its mechanism may be related to the down-regulation of serum NSE S100B expression level. Qingnao Yiyuan decoction is worth popularizing in clinic.
