【摘要】：目的:觀察導(dǎo)師利用黃芪建中湯加味治療脾胃虛寒型慢性蕁麻疹的臨床療效,觀察治療前后患者外周血中CD4+/CD8+水平的變化,并就其探討可能出現(xiàn)的作用機(jī)理。方法:將60例脾胃虛寒型慢性蕁麻疹患者,隨機(jī)分成治療組和對照組。治療組:黃芪建中湯加味聯(lián)合鹽酸左西替利嗪分散片。對照組:祛風(fēng)止癢口服液聯(lián)合鹽酸左西替利嗪分散片。觀察兩組患者的臨床療效及治療前后兩組患者外周血中T淋巴細(xì)胞CD4+/CD8+的變化。結(jié)果:治療結(jié)束后治療組痊愈率60%,有效率93.33%;對照組治愈率為26.67%,總有效率為66.67%。兩組痊愈率、總有效率比較有顯著差異,具有統(tǒng)計(jì)學(xué)意義(P0.05),治療后外周血中CD4/CD8比值比治療前有差異,無統(tǒng)計(jì)學(xué)意義(P0.05);治療后隨訪3個月,治療組復(fù)發(fā)率22%,對照組復(fù)發(fā)率62.5%,兩組比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05),治療組優(yōu)于對照組。結(jié)論:黃芪建中湯加味治療脾胃虛寒型慢性蕁麻疹較對照組有效率高、治愈率高,復(fù)發(fā)率低,其療效好、安全性高。并對患者CD4+/CD8+水平有影響,其作用機(jī)理可能與調(diào)節(jié)患者的自身免疫有關(guān)。
[Abstract]:Objective: to observe the therapeutic effect of Huangqi Jianzhong decoction on chronic urticaria with deficiency of spleen and stomach, observe the changes of CD4 / CD8 level in peripheral blood before and after treatment, and discuss the possible mechanism. Methods: 60 cases of chronic urticaria with deficiency of spleen and stomach were randomly divided into treatment group and control group. Treatment group: Huangqi Jianzhong decoction combined with levocetirizine hydrochloride dispersible tablets. Control group: Qufeng Zhiyang oral liquid combined with levcetirizine hydrochloride dispersible tablets. To observe the clinical effect of the two groups and the changes of T lymphocyte CD4 / CD8 in peripheral blood before and after treatment. Results: the cure rate of the treatment group was 60 and the effective rate was 93.333.The cure rate of the control group was 26.67 and the total effective rate was 66.67. The recovery rate and total effective rate of the two groups were significantly different (P0.05). The ratio of CD4 / CD8 in peripheral blood after treatment was significantly higher than that before treatment (P0.05). The recurrence rate of the treatment group 22 and the control group 62.5, the two groups, the difference was statistically significant (P0.05), the treatment group is better than the control group. Conclusion: Huangqi Jianzhong decoction has higher effective rate, higher cure rate and lower recurrence rate in treating chronic urticaria with deficiency of spleen and stomach. The effect of CD _ 4 / CD _ 8 may be related to the regulation of autoimmunity.
