【摘要】：中風(fēng)后尿潴留是臨床上常見(jiàn)的癥狀,表現(xiàn)為尿液潴留于膀胱卻不能排出。給患者造成痛苦的同時(shí),還嚴(yán)重影響了患者病情的恢復(fù)。該病屬于中醫(yī)“癃閉”范疇,其主要是由于病后體虛,由于飲食不節(jié),情志所傷,或瘀濁內(nèi)停,外邪侵襲而引發(fā)的膀胱氣化不利,小便閉塞不通。本文參考大量文獻(xiàn)資料,在文獻(xiàn)綜述方面論述中醫(yī)治療中風(fēng)后尿潴留的臨床研究進(jìn)展,包括針刺,灸法,中藥及其他療法對(duì)于本病的治療效果。正文從傳統(tǒng)醫(yī)學(xué)和現(xiàn)代醫(yī)學(xué)兩方面論述了對(duì)于中風(fēng)后尿潴留的認(rèn)識(shí)后,主要從病因病機(jī)、證治特點(diǎn)和選穴特色方面論述周鴻飛教授針刺治療本病的學(xué)術(shù)思想及其經(jīng)驗(yàn),并結(jié)合病房典型病例進(jìn)行探討。周鴻飛教授認(rèn)為中風(fēng)后尿潴留的病位主要在膀胱,但與肺、肝、脾、腎關(guān)系密切,其病機(jī)主要在于氣虛不化,陰虛生熱,瘀血阻內(nèi),最終導(dǎo)致膀胱氣化不利而引起的小便不利。周教授認(rèn)為本病雖有實(shí)象,卻因發(fā)于中風(fēng)之后而多為虛證或本虛標(biāo)實(shí)之證。故以“益氣開(kāi)竅”為法,通利同時(shí)補(bǔ)氣,祛邪不忘扶正,以恢復(fù)膀胱氣化功能,使得小便通暢。在“因人制宜”和“因證治宜”的治療基礎(chǔ)上,以強(qiáng)刺激至陰穴為治療特點(diǎn),激發(fā)膀胱經(jīng)氣,通利經(jīng)脈氣血,取得了令人滿意的療效,深受廣大患者的好評(píng)。
[Abstract]:Urinary retention after stroke is a common clinical symptom characterized by urinary retention in the bladder but not excreted. To the patient caused pain, but also seriously affected the patient's recovery. The disease belongs to the category of "dysphagia", which is mainly due to the deficiency of body after the disease, because the diet is not good, the emotion is injured, or the blood stasis is stopped inside, the bladder gasification caused by the invasion of external pathogens is unfavorable, and the urination is blocked and impassable. This article, referring to a large number of literature, discusses the clinical research progress of TCM in the treatment of urinary retention after stroke, including acupuncture, moxibustion, Chinese medicine and other treatments for the treatment of this disease. The text discusses the understanding of urinary retention after stroke from two aspects of traditional medicine and modern medicine, and mainly discusses the academic thought and experience of Professor Zhou Hongfei in the treatment of this disease from the aspects of etiology and pathogenesis, characteristics of syndrome and treatment and characteristics of acupoint selection. The typical cases in the ward were discussed. Professor Zhou Hongfei thinks that the disease position of urinary retention after stroke is mainly in the bladder, but it is closely related to lung, liver, spleen and kidney. Its pathogenesis mainly lies in the deficiency of qi, yin deficiency and heat, and blood stasis, which ultimately leads to the unfavorable urination caused by bladder gasification. Professor Zhou believes that although the disease is real, it occurs after a stroke and is usually due to deficiency or deficiency. Therefore, "Yiqi Kai orifices" as a method, Tongli at the same time tonifying qi, dispelling evil do not forget to correct, in order to restore the function of bladder gasification, make the urine smooth. On the basis of the treatment of "according to the conditions of people" and "treating due to syndrome", the strong stimulation of acupoint to yin was used to stimulate the bladder meridian qi and blood, and the curative effect was satisfactory, which was well received by the majority of patients.
