【摘要】：目的:觀察南少林改良扶肩旋腰法治療中老年腰椎間盤突出癥的臨床療效,為優(yōu)化中老年腰椎間盤突出癥的治療方案提供臨床依據(jù)。方法:按診斷、納入、排除標(biāo)準(zhǔn),收集2015年7月至2016年9月就診于骨科確診為腰椎間盤突出癥的患者100例。隨機(jī)按1:1的比例分為治療組(予南少林改良扶肩旋腰法)和對(duì)照組(予傳統(tǒng)腰椎斜扳法),每組50例。兩組患者均每周接受6次治療,1周為一療程,治療兩個(gè)療程。治療結(jié)束后對(duì)兩組的治療效果進(jìn)行以下方面的比較:兩組患者治療前后的視覺(jué)模擬評(píng)分法(visual analogue scale,VAS)、兩組患者治療前后JOA下腰痛評(píng)分、兩組患者治療前后腰椎屈伸活動(dòng)度、兩組患者治療前后臨床療效,并對(duì)所得數(shù)據(jù)使用SPSS20.0軟件進(jìn)行統(tǒng)計(jì)學(xué)處理。結(jié)果:1.基線比較:兩組患者在性別、年齡、病程方面經(jīng)統(tǒng)計(jì)學(xué)分析,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),具有可比性。兩組患者在CT表現(xiàn)中的突出節(jié)段分布、突出物位置分布、突出物矢狀徑大小經(jīng)統(tǒng)計(jì)學(xué)分析,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);2.臨床療效比較:兩組患者治療2個(gè)療程后與治療前相比臨床療效有統(tǒng)計(jì)學(xué)差異(P0.05);3.視覺(jué)模擬評(píng)分法:兩組患者治療2個(gè)療程后與治療前疼痛綜合評(píng)定比較均有統(tǒng)計(jì)學(xué)差異(P0.05);4.JOA下腰痛評(píng)分:兩組患者治療2個(gè)療程后與治療前JOA下腰痛評(píng)分比較均有統(tǒng)計(jì)學(xué)差異(P0.05);5.腰椎屈伸活動(dòng)度:兩組患者治療2個(gè)療程后與治療前腰椎屈伸活動(dòng)度比較均有統(tǒng)計(jì)學(xué)差異(P0.05)。結(jié)論:·臨床上對(duì)中老年腰椎間盤突出癥患者,南少林改良扶肩旋腰法總體療效更佳,在減輕患者主觀的疼痛及客觀的增加腰椎屈伸活動(dòng)度中,南少林改良扶肩旋腰法比傳統(tǒng)腰椎斜扳法更有優(yōu)勢(shì),值得臨床推廣與運(yùn)用。
[Abstract]:Objective: to observe the clinical effect of South Shaolin improved shoulder rotation method in the treatment of middle and old patients with lumbar disc herniation, and to provide the clinical basis for optimizing the treatment scheme of middle and old age lumbar disc herniation. Methods: according to the criteria of diagnosis, inclusion and exclusion, 100 patients with lumbar disc herniation diagnosed in orthopedic department from July 2015 to September 2016 were collected. The patients were randomly divided into treatment group (treated with South Shaolin improved shoulder rotation method) and control group (traditional lumbar oblique manipulation) with 50 cases in each group. The patients in both groups were treated 6 times a week for one course of treatment and two courses of treatment. After the treatment, the two groups were compared in the following aspects: visual analogue score before and after treatment (visual analogue scale), low back pain score before and after treatment, and lumbar flexion and extension activity before and after treatment. Two groups of patients before and after treatment of clinical efficacy, and the data were analyzed by SPSS 20.0 software. The result is 1: 1. Baseline comparison: the two groups of patients in gender, age, course of statistical analysis, the differences were not statistically significant (P0.05), comparable. The distribution of herniation segment, protruding position and sagittal diameter of herniation in CT findings of the two groups were not statistically significant (P0.05). Comparison of clinical efficacy: two groups of patients after two courses of treatment compared with the treatment before the clinical efficacy of statistical difference (P0.05). Visual analogue score: the two groups of patients after two courses of treatment and before treatment pain comprehensive evaluation were statistically different (P0.05) 4. JOA low back pain score: the two groups of patients after two courses of treatment and before JOA low back pain scores were statistically different (P0.05). Lumbar flexion and extension activity: two groups of patients after two courses of treatment and before treatment of lumbar flexion and extension activity were significantly different (P0.05). Conclusion: in the middle and aged patients with lumbar disc herniation, South Shaolin improved shoulder rotation method is better in reducing subjective pain and increasing lumbar flexion and extension activity. South Shaolin improved shoulder rotation method has more advantages than traditional lumbar oblique manipulation method, which is worth popularizing and applying in clinic.
