本文選題：萎縮性胃炎 + 益氣養(yǎng)陰化瘀　； 參考：《安徽中醫(yī)藥大學(xué)》2016年碩士論文

【摘要】：1目的觀察益氣養(yǎng)陰化瘀顆粒對(duì)慢性萎縮性胃炎患者臨床療效及胃蛋白酶原及胃泌素的影響。2方法選擇符合標(biāo)準(zhǔn)的CAG患者64例,隨機(jī)分成二組:治療組32例與對(duì)照組32例,治療組給予益氣養(yǎng)陰化瘀顆粒口服,對(duì)照組予以胃復(fù)春口服。療程12周,隨訪12周,觀察兩組患者治療前后對(duì)臨床癥狀、胃鏡下表現(xiàn)、病理、胃蛋白酶原及胃泌素的變化。3結(jié)果3.1臨床療效:治療后,兩組患者胃脘痛、痞滿、嘈雜、納差癥狀均有改善;治療組痞滿、嘈雜癥狀的改善起效較對(duì)照組快;痞滿、嘈雜、納差癥狀的改善,治療組療效優(yōu)于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.01或P0.05);治療組證候總有效率高于對(duì)照組(P0.05)。3.2胃鏡下表現(xiàn):兩組治療后胃黏膜情況均有改善,治療組有效率高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。3.3組織病理:兩組治療后胃黏膜萎縮、腸化均有改善。對(duì)胃黏膜萎縮的改善,治療組有效率為87.5%,對(duì)照組為62.5%;對(duì)腸化的改善,治療組有效率為91.7%,對(duì)照組為60.9%,治療組胃黏膜萎縮及腸化的改善均優(yōu)于對(duì)照組(P0.05)。3.4胃蛋白酶原水平:治療后兩組PGⅠ的水平均有提高,且治療組療效優(yōu)于對(duì)照組(P0.05);兩組PGII水平治療前后比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。3.5胃泌素水平:治療后兩組G17的水平均有提高,治療組療效優(yōu)于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。3.6遠(yuǎn)期療效觀察:隨訪12周,治療組癥候療效總有效率為62.5%,對(duì)照組為50.0%,差異無(wú)統(tǒng)計(jì)學(xué)意義。(P0.05)4結(jié)論4.1益氣養(yǎng)陰化瘀顆粒能較好的改善陰虛血瘀型患者的臨床癥狀。4.2益氣養(yǎng)陰化瘀顆粒能很好的改善陰虛血瘀型患者的病理組織,在胃黏膜萎縮及胃黏膜腸化的治療上明顯優(yōu)于為胃復(fù)春組。4.3益氣養(yǎng)陰化瘀顆粒能很好的改善陰虛血瘀型患者的胃鏡下胃黏膜情況,且優(yōu)于胃復(fù)春組。4.4益氣養(yǎng)陰化瘀顆粒能促進(jìn)胃蛋白酶原Ⅰ及胃泌素的分泌。4.5益氣養(yǎng)陰化瘀顆粒與胃復(fù)春遠(yuǎn)期療效相當(dāng)。
[Abstract]:1 Objective To observe the effect of Yiqi Yangyin Huayu Granule on the clinical efficacy of chronic atrophic gastritis and the effect of pepsinogen and gastrin in the.2 method, 64 patients with standard CAG were selected and divided into two groups randomly: 32 cases in the treatment group and 32 cases in the control group. The treatment group was given Yiqi Yangyin Huayu granule orally, and the control group was given Wei Fuchun oral administration. The course of treatment was 12. Week, follow up 12 weeks, observe the clinical symptoms, gastroscope, pepsinogen and gastrin changes.3 results in two groups of patients before and after treatment 3.1 clinical results: after treatment, the two groups of patients with epigastric pain, fullness, noise, and dysphasia were improved; the treatment group was full, noisy symptoms improved faster than the control group; ruffian, noisy, Na differential disease. The curative effect of the treatment group was better than the control group (P0.01 or P0.05); the total effective rate of the syndrome in the treatment group was higher than that of the control group (P0.05).3.2 Gastroscope: the two groups were improved after the treatment. The effective rate of the treatment group was higher than that of the control group. The difference was statistically significant (P0.05).3.3 histopathology: the gastric mucus after treatment. The efficiency of the treatment group was 87.5%, the control group was 62.5%, the efficiency of the treatment group was 91.7% and the control group was 60.9%. The improvement of gastric mucosa atrophy and intestinal metaplasia in the treatment group was better than that of the control group (P0.05).3.4 pepsinogen level: the level of the two groups of PG I after treatment were raised. The curative effect of the treatment group was better than the control group (P0.05), and the difference between the two groups before and after the treatment of PGII was not statistically significant (P0.05).3.5 gastrin level: after the treatment, the level of G17 in the two groups was improved, the curative effect of the treatment group was better than the control group, the difference was statistically significant (P0.05).3.6 long-term effect observation: the follow-up was 12 weeks, the treatment group's symptoms were effective and effective. The rate was 62.5% and the control group was 50%. (P0.05) 4 Conclusion 4.1 Yiqi Yangyin Huayu granule could better improve the clinical symptoms of yin deficiency and blood stasis syndrome,.4.2 Yiqi Yangyin Huayu granule could improve the pathological tissue of the patients with Yin deficiency and blood stasis, and it was obviously better than the stomach in the treatment of gastric mucosa atrophy and gastric mucosa intestinal metaplasia. The.4.3 Yiqi nourishing yin and removing stasis granule can improve the gastric mucosal condition of the patients with Yin deficiency and blood stasis, and it is superior to the.4.4 Yiqi Yangyin Huayu granule in the Wei Fuchun group, which can promote the secretion of pepsinogen I and the secreting of gastrin,.4.5, Yiqi Yangyin Huayu granule and the long-term effect of Wei Fuchun.
