【摘要】：目的通過低溫等離子刀成形術(shù)聯(lián)合中藥熏洗治療陳舊性踝關(guān)節(jié)扭傷的臨床療效觀察,根據(jù)治療前后患者的踝關(guān)節(jié)癥狀、體征等進(jìn)行科學(xué)的分析,為臨床規(guī)范化治療提供治療依據(jù)。方法將征集的黑龍江中醫(yī)藥大學(xué)附屬一院骨傷一科2015年03月01日至2016年03月31日期間門診及住院患者,且符合診斷標(biāo)準(zhǔn)的單側(cè)陳舊性踝關(guān)節(jié)扭傷患者68例,隨機(jī)分為低溫等離子刀成形術(shù)組(治療組)34例,小針刀組(對照組)34例,低溫等離子刀成形術(shù)組和小針刀組均予以中藥物熏洗患踝關(guān)節(jié)。15天一個(gè)療程,2個(gè)治療療程后觀察其踝關(guān)節(jié)指標(biāo)。觀察治療過程中有無并發(fā)癥及不良反應(yīng),并記錄下治療組和對照組治療前與治療后踝關(guān)節(jié)的癥狀體征的變化及改善情況。結(jié)果整個(gè)治療過程中無并發(fā)癥及不良反應(yīng),兩組治療后其VAS疼痛評分較治療前明顯減低,疼痛明顯緩解。治療組中34例,痊愈8例,痊愈率為23.53%,總有效率為97.06%。對照組中34例,痊愈6例,痊愈率為17.65%,總有效率為94.12%。美國足踝聯(lián)合會(huì)AOFAS踝與足功能評分分值及Baird-Jackson踝關(guān)節(jié)評分分值明顯得到提升,減輕了病患的痛苦,改善了病患的癥狀。治療結(jié)束后無論從VAS視覺模擬評分分值、治愈顯效有效情況、美國足踝聯(lián)合會(huì)AOFAS踝與足功能評分分值、Baird-Jackson踝關(guān)節(jié)評分分值、患者踝關(guān)節(jié)疼痛度、踝關(guān)節(jié)功能、運(yùn)動(dòng)工作能力等比治療前均得到了顯著的療效。且低溫等離子刀成形術(shù)組療效比小針刀組更佳。結(jié)論低溫等離子刀成形術(shù)能夠有效緩解患者陳舊性踝關(guān)節(jié)扭傷癥狀。在臨床上安全性能好、創(chuàng)傷小、副作用少,能減輕病患的痛苦,改善病患的癥狀。值得臨床推廣。
[Abstract]:Objective to observe the clinical effect of low temperature plasma knife angioplasty combined with fumigation and washing of traditional Chinese medicine for the treatment of old ankle sprain, and to make a scientific analysis according to the symptoms and signs of ankle joint of the patients before and after treatment. To provide treatment basis for clinical standardized treatment. Methods Sixty-eight patients with unilateral old ankle sprain were recruited from the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine from March 01, 2015 to March 31, 2016, and 68 old ankle sprain patients met the diagnostic criteria. They were randomly divided into low temperature plasma knife group (34 cases), small needle knife group (34 cases), and small needle knife group (34 cases). Hypothermia plasmaplasty group and small needle knife group were treated with middle drug fumigation and washing ankle joint for 15 days. The ankle index was observed after 2 courses of treatment. The changes and improvement of ankle symptoms and signs before and after treatment in the treatment group and control group were recorded. Results there were no complications and adverse reactions in the whole course of treatment. After treatment, the VAS pain score of the two groups was significantly lower than that before treatment, and the pain was obviously relieved. In the treatment group, 34 cases were cured, 8 cases were cured, the cure rate was 23.53 and the total effective rate was 97.06. In the control group, 34 cases were cured, 6 cases were cured, the cure rate was 17.65%, the total effective rate was 94.12%. The AOFAS ankle and foot function score and the Baird-Jackson ankle score of the American ankle Federation were significantly improved, which alleviated the pain and improved the symptoms of the patients. After treatment, no matter from the visual analogue score of VAS, the effective condition of cure, the score of AOFAS ankle and foot function of American ankle Federation, the score of Baird-Jackson ankle, the degree of ankle joint pain, the function of ankle joint. The working ability of exercise was more effective than that before treatment. The therapeutic effect of low temperature plasma knife group was better than that of small needle knife group. Conclusion low-temperature plasma knife surgery can effectively relieve the old ankle sprain symptoms. Good clinical safety, less trauma, less side effects, can alleviate the pain of patients and improve their symptoms. It is worth popularizing in clinic.
