【摘要】：目的：本研究通過觀察冠心止痛膠囊對冠心病穩(wěn)定性心絞痛患者的中醫(yī)證候療效、心絞痛療效、肱動脈血流介導(dǎo)的舒張功能(FMD)、C-反應(yīng)蛋白(CRP)、腫瘤壞死因子-α(TNF-α)的影響,以了解冠心止痛膠囊的作用機(jī)理,評價該方對冠心病穩(wěn)定性心絞痛患者的血管內(nèi)皮功能的改善及炎癥反應(yīng)的減輕作用。方法：選擇河南省中醫(yī)藥研究院附屬醫(yī)院門診符合納入標(biāo)準(zhǔn)的冠心病穩(wěn)定性心絞痛的患者60例,中醫(yī)診斷為胸痹(痰瘀交阻證),按1：1的比例隨機(jī)分為治療組與對照組,各30例,兩組均給以基礎(chǔ)治療予阿司匹林腸溶片100mg,每晚一次,口服；酒石酸美托洛爾12.5mg,早晚各一次,口服；阿托伐他汀鈣片10mg,每晚一次,口服。治療組加用冠心止痛膠囊(河南省中醫(yī)藥研究院附屬醫(yī)院院內(nèi)制劑),4粒,每日3次,口服,3個月為一療程,觀察一個療程,觀察比較該方治療前后中醫(yī)證候、心絞痛分級和肱動脈血流介導(dǎo)的舒張功能、C-反應(yīng)蛋白、腫瘤壞死因子-α的變化情況。結(jié)果：(1)中醫(yī)證候療效：治療組中醫(yī)證候的療效總有效率為93.33%,對照組總有效率為66.67%,兩組比較,差異有統(tǒng)計學(xué)意義(P0.05)。(2)兩組心絞痛分級改善情況：治療組心絞痛的改善總有效率為83.33%,對照組總有效率為46.67%,兩組比較,差異有統(tǒng)計學(xué)意義(P0.05)。(3)兩組FMD、CRP、TNF-α的變化情況：兩組治療后與治療前相比,均能升高FMD,降低患者的血清CRP、TNF-α,治療組的改善明顯優(yōu)于對照組,有統(tǒng)計學(xué)意義(P0.05)。結(jié)論：本研究顯示：冠心止痛膠囊能有效的改善穩(wěn)定性心絞痛患者中醫(yī)臨床證候,改善心絞痛分級,其臨床療效機(jī)制可能與血管內(nèi)皮功能的改善和炎癥反應(yīng)的減輕有關(guān)。
[Abstract]:Objective: to observe the effects of Guanxin Zhitong capsule on TCM syndromes, angina pectoris, blood flow mediated diastolic function of brachial artery (FMD-CRPN) and tumor necrosis factor- 偽 (TNF- 偽) in patients with stable angina pectoris of coronary heart disease. To understand the mechanism of Guanxin Zhitong capsule and evaluate the effect of Guanxin Zhitong capsule on the improvement of vascular endothelial function and inflammation in patients with stable angina pectoris of coronary heart disease. Methods: sixty patients with stable angina pectoris of coronary heart disease were selected from outpatient clinic of affiliated Hospital of Henan Institute of traditional Chinese Medicine. The patients were diagnosed as chest obstruction (phlegm and blood stasis syndrome) and randomly divided into treatment group and control group according to the proportion of 1:1. 30 patients in each group were given basic treatment with 100 mg aspirin enteric-coated tablets once a night; metoprolol tartrate 12.5 mg in the morning and evening; Atto vastatin calcium tablet 10 mg once a night. The treatment group was treated with Guanxin Zhitong capsule (4 capsules of Guanxin Zhitong capsule, 3 times a day, 3 months as a course of treatment), observing a course of treatment, observing and comparing the TCM syndromes before and after the treatment. Changes of angina pectoris grade and brachial artery blood flow mediated diastolic function of C-reactive protein and tumor necrosis factor-偽. Results: the total effective rate of TCM syndromes in the treatment group was 93.33 and that in the control group was 66.67. The improvement of angina pectoris grade in the treatment group was 83.33 and the total effective rate in the control group was 46.67. There was significant difference between the two groups (P 0.05, P < 0.05). The changes of TNF- 偽 in the two groups were as follows: compared with before treatment, the two groups could increase FMDand decrease the serum level of CRP- TNF- 偽. The improvement of the treatment group was obviously better than that of the control group, and there was significant difference between the treatment group and the control group (P 0.05). Conclusion: Guanxin Zhitong capsule can effectively improve TCM syndromes and angina pectoris grading in patients with stable angina pectoris. The mechanism of clinical curative effect may be related to the improvement of vascular endothelial function and the reduction of inflammatory reaction.
