【摘要】：目的:對導師應用健脾涼血法治療放射性直腸炎進行經(jīng)驗總結,并通過文獻研究總結中醫(yī)在辨證治療放射性直腸炎的中醫(yī)臨床治療方法及用藥規(guī)律。材料與方法:整理導師臨床治療放射性直腸炎病例,分析臨床療效與用藥規(guī)律,總結臨床診治思路與用藥經(jīng)驗。檢索中國知網(wǎng)數(shù)據(jù)資源庫(CNKI)、萬方全文數(shù)據(jù)庫,歸納整理了中醫(yī)藥治療放射性直腸炎相關內容,重點分析目前有關中醫(yī)辨證治療及用藥規(guī)律。結果:1.放療為外感火熱邪氣,以熱毒蘊結,耗氣傷陰,損傷脾胃為基本病機。健脾涼血法是治療放射性直腸炎的基本大法。清熱解毒,涼血止血以地榆、槐花、白頭翁、敗醬草;脾胃虛弱以四君子湯加減,用藥慎用溫補。2.篩選出135篇涉及治法方藥的文獻,研究結果表明,臨床治療放射性直腸炎以清熱涼血止血,補虛理氣為主,常用經(jīng)典方劑以白頭翁湯、葛根芩連湯、參苓白術散為主,常用中藥茯苓、黃連、白頭翁、地榆、槐花、白術等。結論:放療屬外感火熱邪氣,病機熱毒蘊結,耗氣傷陰,損傷脾胃;結合文獻證治規(guī)律研究證實健脾涼血法是治療放射性直腸炎的基本大法。
[Abstract]:Objective: to summarize the experience of treating radioactive proctitis with the method of invigorating spleen and cooling blood, and to summarize the clinical treatment method and drug use rule of TCM in treating radioactive proctitis according to syndrome differentiation. Materials and methods: the clinical treatment of radiation proctitis was treated by tutor, the clinical curative effect and drug use rule were analyzed, and the clinical diagnosis and treatment thought and medication experience were summarized. To search the CNKI database and Wanfang full-text database, the related contents of traditional Chinese medicine in the treatment of radioactive proctitis were summarized, and the current law of TCM syndrome differentiation and drug use was analyzed. The result is 1: 1. Radiation therapy is external fiery heat evil gas, heat toxin accumulation knot, gas consumption injury Yin, damage spleen and stomach as the basic pathogenesis. The method of invigorating the spleen and cooling the blood is the basic method for the treatment of radioactive proctitis. Clearing heat and detoxification, cooling blood to stop bleeding with elm, Sophora japonica, Radix Pulsatilla, Prunus vulgaris; spleen and stomach weakness with Sijunzi decoction reduced, medication caution to warm up. 2. One hundred and thirty-five articles of literature related to the treatment of traditional Chinese medicine were selected. The results showed that the clinical treatment of radioactive proctitis was to clear heat, cool blood and stop bleeding, replenish deficiency and regulate qi, and commonly used classical prescriptions such as Baitouweng decoction, Gegen Qinlian decoction, Shenling Baizhu Powder, etc. Traditional Chinese medicine Poria cocos, Coptis, Weng, Ulmus pumila, Sophora japonica, Atractylodes macrocephala and so on. Conclusion: radiation therapy belongs to external fiery heat and evil gas, the pathogenesis of which is heat toxin accumulation, qi consumption and Yin injury, and spleen and stomach injury, combined with the study of the law of syndrome and treatment of literature, it is proved that the method of invigorating spleen and cooling blood is the basic method for the treatment of radioactive proctitis.
