【摘要】：目的:對(duì)比研究除濕通痹方、白虎加桂枝湯合宣痹湯加減方治療急性痛風(fēng)性關(guān)節(jié)炎紅腫熱痛等癥狀的臨床療效。方法:將2014年10月-2016年10月共收集70例納入本研究,運(yùn)用隨機(jī)數(shù)字表分組法,其中觀察組35例,對(duì)照組35例。觀察過(guò)程中因依從性差、病例脫落等原因,最終觀察組32例,對(duì)照組32例。最終數(shù)據(jù)統(tǒng)計(jì)分析按64病例進(jìn)行研究,通過(guò)觀察除濕通痹方、白虎加桂枝湯合宣痹湯加減方對(duì)急性痛風(fēng)性關(guān)節(jié)炎患者發(fā)病關(guān)節(jié)疼痛、腫脹、發(fā)熱、功能等癥狀積分的影響,來(lái)評(píng)價(jià)除濕通痹方治療急性痛風(fēng)性關(guān)節(jié)炎的臨床療效。結(jié)果:1.總體有效率評(píng)價(jià):觀察組治療后,其中臨床痊愈4例,顯效8例,有效19例,無(wú)效1例,總有效率96.86%;對(duì)照組治療后,臨床痊愈2例,顯效5例,有效21例,無(wú)效4例,總有效率87.50%。觀察組療效優(yōu)于對(duì)照組。2.觀察組與對(duì)照組經(jīng)過(guò)治療后,癥狀總積分均明顯改善,P0.001。3.觀察組關(guān)節(jié)腫脹的癥狀積分改善程度明顯優(yōu)于對(duì)照組,P0.001,觀察組關(guān)節(jié)功能障礙的癥狀積分較對(duì)照組下降更多,P0.05。結(jié)論:除濕通痹方在減輕急性痛風(fēng)性關(guān)節(jié)炎患者關(guān)節(jié)疼痛、壓痛、表面皮色、發(fā)熱、受累關(guān)節(jié)數(shù),尤其是關(guān)節(jié)腫脹、功能障礙方面具有良好的作用。表明除濕通痹方在急性痛風(fēng)性關(guān)節(jié)炎的治療中具有一定的應(yīng)用價(jià)值,體現(xiàn)了中醫(yī)辨證施治以及中藥的優(yōu)勢(shì)。
[Abstract]:Objective: to compare the clinical effect of dehumidification Tongbi prescription, Baihu plus Guizhi decoction and Xuanbi decoction in treating acute gouty arthritis. Methods: a total of 70 cases were collected from October 2014 to October 2016 and randomly divided into two groups: observation group (n = 35) and control group (n = 35). There were 32 cases in the observation group and 32 cases in the control group because of poor compliance and abscission. The final data were statistically analyzed according to 64 cases. By observing the effects of Qishitongbi recipe, Baihu plus Guizhi decoction and Xuanbi decoction on the joint pain, swelling, fever and function in patients with acute gouty arthritis, To evaluate the clinical efficacy of dehumidification Tongbi prescription in the treatment of acute gouty arthritis. The result is 1: 1. The overall effective rate was evaluated as follows: in the observation group, 4 cases were clinically cured, 8 cases were markedly effective, 19 cases were effective, 1 case was ineffective, and the total effective rate was 96.86 cases, while in the control group, 2 cases were cured, 5 cases were markedly effective, 21 cases were effective, 4 cases were ineffective. The total effective rate is 87.50. The curative effect of the observation group was better than that of the control group. After treatment, the total symptom scores of the observation group and the control group were significantly improved (P 0.001.3). The symptom score of joint swelling in the observation group was significantly better than that in the control group (P 0.001), and the symptom score of joint dysfunction in the observation group was much lower than that in the control group (P 0.05). Conclusion: Dishitongbi prescription has a good effect on relieving joint pain, tenderness, skin color, fever, number of affected joints, especially joint swelling and dysfunction in patients with acute gouty arthritis. It shows that the prescription of removing dampness and Tongbi has certain application value in the treatment of acute gouty arthritis, which embodies the advantages of TCM syndrome differentiation and traditional Chinese medicine.
