【摘要】：難治性高血壓又稱頑固性高血壓,是指聯(lián)合使用了三種以上降壓藥(一般包括利尿劑)且劑量合適,血壓仍未達(dá)到目標(biāo)水平,或者聯(lián)合使用了四種以上(包括四種)降壓藥血壓才達(dá)標(biāo)也可以考慮是難治性高血壓[1]。難治性高血壓是威脅人類生命安全的重要因素,影響心、腦、腎等臟器的功能和結(jié)構(gòu),最終導(dǎo)致這些器官衰竭。受遺傳、飲食習(xí)慣等因素的影響,發(fā)病率和死亡率逐年增高,嚴(yán)重威脅人類的生命健康,影響患者的生活質(zhì)量�，F(xiàn)代醫(yī)學(xué)對(duì)于原發(fā)性高血壓病還沒(méi)有特效療法,需要長(zhǎng)期服用降壓藥治療高血壓病,不良反應(yīng)多,患者依從性差,而且療效不穩(wěn)定,不能很好地解決患者眩暈、頭痛、失眠及耳鳴等癥狀,容易發(fā)展成為難治性高血壓。中醫(yī)學(xué)治療高血壓病歷史悠久,有其獨(dú)特的優(yōu)勢(shì),降壓療效可靠,作用溫和持久,價(jià)格低廉,作用途徑多,積極改善癥狀,提高患者的生存質(zhì)量。吾師張艷教授從事臨床醫(yī)療、教學(xué)、科研工作30余年,學(xué)驗(yàn)俱豐,在繼承中醫(yī)經(jīng)典理論的基礎(chǔ)上,結(jié)合自己的臨床實(shí)踐,繼承之中求創(chuàng)新,治療難治性高血壓有其獨(dú)特優(yōu)勢(shì),能夠個(gè)體化治療,有效改善臨床癥狀。吾師認(rèn)為“痰瘀蘊(yùn)毒,毒損腦絡(luò)”為難治性高血壓的主要發(fā)病機(jī)制,痰瘀毒存在于難治性高血壓的病程中,本著治病必求于本的思想,以“痰瘀毒”為切入點(diǎn),以祛痰化瘀解毒通竅法為主要原則防治難治性高血壓,取得較好的臨床療效。
[Abstract]:Refractory hypertension, also known as refractory hypertension, refers to the combination of three or more antihypertensive drugs (including diuretics) and the appropriate dosage, and the blood pressure is still not up to the target level. Or combined use of more than four (including four) antihypertensive drugs to achieve blood pressure may also be considered refractory hypertension [1]. Refractory hypertension is an important factor threatening the safety of human life. It affects the function and structure of organs such as heart, brain and kidney, and finally results in these organ failure. Under the influence of heredity, dietary habits and other factors, the morbidity and mortality increased year by year, which seriously threatened the life and health of human beings and affected the quality of life of patients. Modern medicine has no special treatment for essential hypertension. It needs long-term antihypertensive drugs to treat hypertension. There are many adverse reactions, patients' compliance is poor, and the curative effect is unstable, which can not solve the dizziness and headache of patients. Insomnia and tinnitus symptoms, easy to develop into refractory hypertension. Traditional Chinese medicine (TCM) has a long history in the treatment of hypertension with its unique advantages, such as reliable antihypertensive effect, mild and lasting effect, low price and many ways of action, so it can improve symptoms and improve the quality of life of patients. Professor Zhang Yan has been engaged in clinical medicine, teaching and scientific research for more than 30 years. On the basis of inheriting the classical theory of traditional Chinese medicine, combining with his own clinical practice and seeking innovation in inheritance, Professor Zhang Yan has its unique advantages in the treatment of refractory hypertension. Individualized treatment can effectively improve clinical symptoms. My teacher believes that "phlegm and stasis accumulation of toxin, toxic damage to brain collaterals" is the main pathogenesis of refractory hypertension, phlegm and blood stasis toxin exists in the course of refractory hypertension, in line with the thought that "phlegm and blood stasis toxin" is the starting point of "phlegm and blood stasis toxin". The method of removing phlegm and removing blood stasis and detoxifying the orifices is the main principle to prevent and cure refractory hypertension and obtain better clinical curative effect.
