【摘要】：目的:研究就診于遼寧省級(jí)中醫(yī)院的腸癌患者中西醫(yī)綜合治療情況,從而總結(jié)中醫(yī)治療模式在各階段腸癌患者中的應(yīng)用比例以及處方用藥規(guī)律。材料與方法:收集2015年5月到2015年6月就診于遼寧中醫(yī)藥大學(xué)附屬醫(yī)院腫瘤科以腸癌為第一診斷的患者,以2014年人民衛(wèi)生出版社出版的由林洪生編寫的《惡性腫瘤中醫(yī)診療指南》中的五種中醫(yī)治療模式為指導(dǎo),并根據(jù)腸癌患者的綜合情況(例如前治療情況,就診目的,臨床分期)等進(jìn)行選擇,分析門診與病房的腸癌患者所接受的中醫(yī)治療模式的差異,以及各中醫(yī)治療模式下處方用藥的規(guī)律。結(jié)果:1.門診患者以中醫(yī)鞏固治療居多,病房患者以單純中醫(yī)治療居多。2.中醫(yī)治療手段門診為辨證湯藥+口服中成藥,病房為靜點(diǎn)中成藥+辨證湯藥+口服中成藥+外敷中藥。3.辨證湯藥的組方原則為辨證擬定主方,隨癥加減。根據(jù)中醫(yī)治療模式指導(dǎo)選擇扶正及祛邪中草藥的種類及味數(shù)。4.中醫(yī)防護(hù)治療以扶正為主,祛邪中草藥味數(shù)應(yīng)用少且輕。5.中醫(yī)加載治療以祛邪為主,祛邪中草藥味數(shù)應(yīng)用多且量大。6.中醫(yī)鞏固治療以扶正祛邪為主,術(shù)后兩年以內(nèi)患者以扶正祛邪并重,祛邪中草藥味數(shù)多且量大,術(shù)后兩年以上患者以扶正為主,祛邪中草藥味數(shù)少且輕。7.中醫(yī)維持治療以扶正祛邪為主,此類患者病情復(fù)雜,祛邪中草藥應(yīng)用依患者情況而定。8.單純中醫(yī)治療以扶正祛邪為主。若患者狀態(tài)好,加重祛邪,祛邪中草藥味數(shù)多且量大。若患者狀態(tài)差,加重扶正,祛邪中草藥味數(shù)少且輕�；颊郀顟B(tài)一般,扶正祛邪并重,祛邪中草藥味數(shù)及用量以具體情況而定。結(jié)論:腸癌的綜合治療已成為趨勢,我們在確定中醫(yī)治療原則的時(shí)候是將西醫(yī)治療的情況,患者的綜合情況有機(jī)的結(jié)合起來,在總的中醫(yī)治療模式的指導(dǎo)下,選擇最佳中醫(yī)治療方法,擬定中藥處方。
[Abstract]:Objective: to study the comprehensive treatment of intestinal cancer patients in Liaoning Provincial Hospital of traditional Chinese Medicine (TCM), and to summarize the proportion of TCM treatment mode in patients with colorectal cancer in different stages and the rule of prescribing drugs. Materials and methods: patients with colorectal cancer were collected from May 2015 to June 2015 in Department of Oncology, affiliated Hospital of Liaoning University of traditional Chinese Medicine. Guided by the five TCM treatment models published by the people's Health Publishing House in 2014 and compiled by Lin Hongsheng for the diagnosis and treatment of malignant tumors, and according to the comprehensive situation of patients with colorectal cancer (such as pre-treatment, the purpose of seeing a doctor), To analyze the difference of TCM treatment mode between outpatients and wards, and the rule of prescribing drugs under each TCM treatment mode. The result is 1: 1. Outpatients with Chinese medicine consolidation treatment is the majority, ward patients with Chinese medicine treatment alone. 2. 2. Traditional Chinese medicine treatment means outpatient for syndrome differentiation decoction oral Chinese patent medicine, wards for static point Chinese patent medicine differentiation decoction oral Chinese patent medicine external application of traditional Chinese medicine .3. The prescription principle of syndrome differentiation decoction is to make the main prescription according to syndrome differentiation, plus or minus with syndrome. According to the traditional Chinese medicine treatment model guide selection of the right and dispelling evil Chinese herbal medicine species and flavor number. 4. Traditional Chinese medicine protection treatment to strengthen the main, expelling evil herbs taste number of less and light. 5. Traditional Chinese medicine loading treatment to expelling evil mainly, expelling evil Chinese herbal medicine taste number and the amount of large. 6. Traditional Chinese medicine consolidation treatment is mainly to nourish qi and dispel evil, two years after the operation, the patients pay equal attention to strengthening the spirit and dispelling evil, the number of Chinese herbal medicines for dispelling evil is more than two years and the quantity of Chinese herbal medicine is large, and more than two years after operation, the Chinese herbal medicine of removing evil is small and light. Traditional Chinese medicine maintenance treatment is mainly to strengthen the qi and dispel evil, this kind of patient's condition is complex, expelling evil Chinese herbal medicine is used according to the patient's condition. Traditional Chinese medicine treatment is based on strengthening the spirit and dispelling evil. If the patient is in good condition, aggravation dispelling evil, expelling evil Chinese herbal medicine taste number and quantity. If the patient is in a bad state, accentuates the rectification, dispels the evil Chinese herbal medicine flavor to be few and light. The patient's condition is general, strengthening the spirit and dispelling evil, the number and dosage of Chinese herbal medicine are determined by specific conditions. Conclusion: comprehensive treatment of colorectal cancer has become a trend. When we determine the principle of TCM treatment, we combine the situation of western medicine treatment and the comprehensive situation of patients organically, under the guidance of the general TCM treatment mode. Select the best Chinese medicine treatment method, draw up the prescription of traditional Chinese medicine.
