【摘要】：目的:擬通過(guò)臨床觀(guān)察加味無(wú)比山藥丸對(duì)III級(jí)慢性阻塞性肺疾病穩(wěn)定期(脾腎兩虛,腎不納氣)的臨床治療效果,探討其是否可以改善患者臨床癥狀、活動(dòng)能力,提高患者生活質(zhì)量,從而進(jìn)一步驗(yàn)證COPD進(jìn)展和急性加重在于穩(wěn)定期的肺脾腎三臟關(guān)系是否協(xié)調(diào),能否維持濁氣的生成和排出的平衡。方法:本實(shí)驗(yàn)選擇符合納入標(biāo)準(zhǔn)的門(mén)診及住院的III級(jí)慢性阻塞性肺疾病穩(wěn)定期(脾腎兩虛,腎不納氣)的患者60例,隨機(jī)分為中藥治療(加味無(wú)比山藥丸)組與西藥對(duì)照組(天晴速樂(lè)吸入劑),每組各30例,兩組比例為1:1。通過(guò)對(duì)氣短、喘促等癥狀、呼吸困難指數(shù)、六分鐘步行試驗(yàn)前后變化情況的評(píng)估,比較治療組和對(duì)照組的療效差異,最終得出客觀(guān)的結(jié)論。結(jié)果:治療組的總體有效率為93.333%,對(duì)照組的總體有效率83.333%(P0.05),提示治療組療效明顯對(duì)照組。加味無(wú)比山藥丸對(duì)于患者氣短、喘促癥狀的改善方面與西藥相似,但在食少腹脹、四肢涼冷的次癥以及呼吸困難指數(shù)、六分鐘步行試驗(yàn)的改善方面優(yōu)于西藥組。結(jié)論:臨床研究表明加味無(wú)比山藥丸對(duì)于慢性阻塞性肺疾病穩(wěn)定期(脾腎兩虛,腎不納氣)療效確切,相似于西藥治療,但在改善臨床癥狀方面療效優(yōu)于對(duì)照組,而且毒副作用小,用藥經(jīng)濟(jì),進(jìn)一步驗(yàn)證COPD穩(wěn)定期的治療在于協(xié)調(diào)肺脾腎三臟關(guān)系,維持濁氣的生成和排出的平衡。完善了導(dǎo)師王檀教授治療本病的理論。
[Abstract]:Objective: to observe the clinical therapeutic effect of Jiawei Huishan pill on III grade chronic obstructive pulmonary disease in stable period (spleen and kidney deficiency, kidney deficiency), and to explore whether it can improve the clinical symptoms and activity ability of the patients. Improve the quality of life of patients so as to further verify that the progression and acute exacerbation of COPD lies in the stable phase of lung spleen and kidney three visceral relationship whether to maintain the balance of turbid qi generation and expulsion. Methods: sixty patients with III grade chronic obstructive pulmonary disease (III) in stable stage (spleen and kidney deficiency, kidney failure), who met the criteria included in the study, were selected. They were randomly divided into traditional Chinese medicine treatment group and western medicine control group with 30 cases in each group. The ratio of the two groups was 1: 1. By evaluating the symptoms of shortness of breath, dyspnea index and changes before and after the six-minute walking test, the curative effects of the treatment group and the control group were compared, and the objective conclusion was reached. Results: the total effective rate of the treatment group was 93.3333.The total effective rate of the control group was 83.3330.The results indicated that the curative effect of the treatment group was obvious in the control group. The improvement of symptoms of dyspnea and dyspnea was similar to that of western medicine, but it was better than that of western medicine group in the improvement of food and abdominal distension, cold limbs, dyspnea index and 6-minute walking test. Conclusion: the clinical study shows that Jiawei Huishan pill is effective in the stable stage of chronic obstructive pulmonary disease (spleen and kidney deficiency, kidney deficiency), similar to western medicine treatment, but the curative effect in improving clinical symptoms is better than that in control group. Furthermore, the treatment of COPD in stable period is to coordinate the relationship between lung, spleen and kidney, and to maintain the balance of turbid gas generation and excretion. Improved the tutor Professor Wang Tan's theory of treating this disease.
