【摘要】：目的:本研究通過觀察臨床心梗后心衰患者聯(lián)合保心湯與未聯(lián)合保心湯治療前后其NYHA心功能分級(jí)改善情況、LVEF、LVEDd、六分鐘步行距離、BNP以及生存質(zhì)量和中醫(yī)證候積分等數(shù)據(jù),并將兩組數(shù)據(jù)進(jìn)行前后對比,總結(jié)出黃芪保心湯對于心梗后氣陰兩虛、瘀毒犯心證患者的臨床療效,以驗(yàn)證黃芪保心湯對于心梗后心衰病人發(fā)揮的改善心功能、緩解心衰癥狀、提高生存質(zhì)量、改善預(yù)后等作用。方法:本實(shí)驗(yàn)總共入選70例患者,入選患者必須曾經(jīng)或正在我院門診治療或住院治療,并且明確診斷為心肌梗死合并心力衰竭,將他們隨機(jī)分組(對照組35例、治療組35例)。70例患者均接受西醫(yī)綜合治療,治療組患者額外接受保心湯中藥治療,連續(xù)觀察3個(gè)月時(shí)間,收集70例患者治療前后的NYHA、BNP、LVEF、LVEDd、六分鐘步行距離等,并加以統(tǒng)計(jì)學(xué)處理,觀察前后是否有統(tǒng)計(jì)學(xué)差異。結(jié)果:心功能方面:與對照組相比,治療組的心功能分級(jí)改善更加明顯(P0.05),LVEF、LVEDd均較治療前改善,BNP降低較對照組更明顯,6分鐘步行距離治療組增加更明顯,且均有統(tǒng)計(jì)學(xué)差異(P0.05);生存質(zhì)量方面:明尼蘇達(dá)心衰生活質(zhì)量評分及36條簡明健康問卷均有改善,36條簡明健康問卷兩組對比差異性明顯(P0.05);治療組在改善中醫(yī)證候積分上較對照組更優(yōu)越(P0.05)。結(jié)論:黃芪保心湯為中藥復(fù)方,可以改善心梗后心衰患者的心功能、提高生存質(zhì)量,聯(lián)合西藥效果更加顯著。
[Abstract]:Objective: to observe the improvement of NYHA cardiac function classification in patients with heart failure after myocardial infarction before and after treatment with Baoxin decoction and uncombined Baoxin decoction, and to observe the data of LVEFG LVEDd, 6-minute walking distance, quality of life and syndromes integral of traditional Chinese medicine (TCM). By comparing the data of the two groups before and after, the clinical effect of Huangqi Baoxin decoction on the patients with deficiency of Qi and Yin after myocardial infarction and blood stasis poisoning on heart syndrome was summarized in order to verify the effect of Huangqi Baoxin decoction on improving heart function and relieving symptoms of heart failure after myocardial infarction. Improve the quality of life, improve the prognosis and so on. Methods: a total of 70 patients were enrolled in this study. They must have been or were in our hospital for outpatient treatment or hospitalization, and were definitely diagnosed as myocardial infarction with heart failure. They were randomly divided into two groups (control group, 35 cases). In the treatment group, 35 patients were treated with western medicine, while the patients in the treatment group were treated with Baoxin decoction for 3 months, and 70 patients were collected before and after treatment, and the walking distance of six minutes was analyzed statistically. Whether there is statistical difference before and after observation. Results: compared with the control group, the improvement of cardiac function grading in the treatment group was more obvious than that in the control group. The improvement of BNP in the treatment group was more obvious than that in the control group, and the increase in the treatment group was more obvious than that in the control group, and the improvement of BNP in the treatment group was more obvious than that in the control group. There were significant differences in quality of life between the two groups: Minnesota Heart failure score and 36 brief health questionnaires were improved and 36 simple health questionnaires were improved. The treatment group improved TCM syndrome. Compared with the control group, it was superior to the control group (P 0.05). Conclusion: Huangqi Baoxin decoction can improve heart function and quality of life of patients with heart failure after myocardial infarction.
