【摘要】：目的:觀察溫肺化痰湯對(duì)AECOPD(陽(yáng)虛痰盛夾熱證)的臨床療效及對(duì)血清IL-23的影響。方法:選擇于黑龍江中醫(yī)藥大學(xué)附屬第一醫(yī)院門(mén)診就診的68例AECOPD(陽(yáng)虛痰盛夾熱證)患者,應(yīng)用隨機(jī)單盲法將患者分為治療組(34例)和對(duì)照組(34例),兩組均口服西藥頭孢克洛和多索茶堿,同時(shí)治療組給予溫肺化痰湯,對(duì)照組給予參貝咳喘丸。療程為10天。觀察兩組患者在用藥前后臨床癥狀的改善、肺功能、白細(xì)胞計(jì)數(shù)、綜合療效評(píng)估及血清IL-23的變化情況。結(jié)果:1.溫肺化痰湯治療AECOPD(陽(yáng)虛痰盛夾熱證)臨床療效顯著,其總有效率為93.75%,與對(duì)照組比較有統(tǒng)計(jì)學(xué)意義(P0.05),療效明顯優(yōu)于對(duì)照組,無(wú)不良反應(yīng)。2.溫肺化痰湯可有效改善患者中醫(yī)證候,尤其在咳嗽、咯黃痰、畏寒肢冷及氣短喘息方面,且治療組療效更加顯著(P0.05)。3.兩組藥物可相對(duì)提升患者療前療后的FEV1及 FEV1/FVC的數(shù)值,但療前療后相比無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。4.兩組患者療前療后白細(xì)胞計(jì)數(shù)均下降,且組內(nèi)比較有統(tǒng)計(jì)學(xué)意義(P0.05);療后組間比較有統(tǒng)計(jì)學(xué)意義(P0.05),且治療組優(yōu)于對(duì)照組。5.兩組藥物均可有效改善患者綜合評(píng)估,療前療后比較有統(tǒng)計(jì)學(xué)意義(P0.05),療后組間比較無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),但治療組改善程度優(yōu)于對(duì)照組。6.兩組藥物均能降低患者血清中IL-23含量,療前療后相比有統(tǒng)計(jì)學(xué)意義(P0.05)。組間比較,治療組明顯優(yōu)于對(duì)照組(P0.05)。結(jié)論:1.溫肺化痰湯治療AECOPD(陽(yáng)虛痰盛夾熱證)臨床療效顯著,可顯著改善患者的臨床癥狀,尤其在咳嗽、咯黃痰、畏寒怕冷方面。2.溫肺化痰湯可降低患者白細(xì)胞計(jì)數(shù)以及改善患者綜合評(píng)估程度。3.溫肺化痰湯可有效降低AECOPD(陽(yáng)虛痰盛夾熱證)患者血清中IL-23的表達(dá)。
[Abstract]:Objective: to observe the clinical effect of Wenfei Huatan decoction (WFHuatang) on AECOPDD (Yang deficiency and phlegm with heat retention syndrome) and the effect of WFHuatan decoction on serum IL-23. Methods: Sixty-eight patients with AECOPD (Yang deficiency phlegm and heat syndrome) were selected from the outpatient clinic of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine. The patients were randomly divided into treatment group (n = 34) and control group (n = 34). Both groups were treated by oral administration of cefaclor and doxophylline. The treatment group was given Wenfei Huatan decoction and the control group was given Shenbei Kechuan pills. The course of treatment was 10 days. The improvement of clinical symptoms, pulmonary function, white blood cell count, comprehensive evaluation of curative effect and changes of serum IL-23 were observed before and after treatment. The result is 1: 1. The clinical curative effect of Wenfei Huatan decoction in treating AECOPD (Yang deficiency and phlegm with heat) was remarkable, the total effective rate was 93.755.Compared with the control group, the effect was significantly better than that of the control group, and the curative effect was obviously better than that of the control group, and there was no adverse reaction. 2. Wenfei Huatan decoction can effectively improve the TCM syndromes, especially in cough, yellow phlegm, cold limbs and short breath, and the curative effect of the treatment group is more obvious than that of P0.05. 3. The FEV1 and FEV1/FVC of the two groups were significantly higher than that of the patients before and after treatment, but there was no significant difference between the two groups before and after treatment (P 0.05. 4). Before and after treatment, the white blood cell count decreased in both groups, and there was significant difference between the two groups (P 0.05), and the treatment group was better than the control group (P 0.05). Two groups of drugs can effectively improve the comprehensive evaluation of patients, before and after treatment compared with P0.05, there is no significant difference between the treatment group P0.05, but the treatment group is better than the control group. 6. Both groups could reduce the IL-23 content in the serum of the patients, and there was significant difference between the two groups before and after treatment (P 0.05). Compared with the control group, the treatment group was better than the control group (P 0.05). Conclusion 1. Wenfei Huatan decoction has a significant clinical effect on AECOPDD, which can improve the clinical symptoms of the patients, especially in cough, yellow phlegm, chills and cold. Wenfei Huatan decoction can reduce the white blood cell count and improve the comprehensive evaluation of patients. Wenfei Huatan decoction can effectively reduce the expression of IL-23 in serum of AECOPD patients.
