【摘要】：目的:本研究通過Meta分析客觀的評價運用清熱活血法治療冠心病不穩(wěn)定性心絞痛的臨床療效,為清熱活血類中藥在臨床推廣應(yīng)用提供循證醫(yī)學(xué)依據(jù)。方法:根據(jù)制定科學(xué)的檢索策略,嚴(yán)格限定納入標(biāo)準(zhǔn)與排除標(biāo)準(zhǔn),運用電子計算機檢索或手工檢索的方法檢索各數(shù)據(jù)庫從開始建立時至2016年10月期間已發(fā)表的運用清熱活血法治療冠心病不穩(wěn)定性心絞痛臨床療效的相關(guān)文獻(xiàn),并采用Jadad量表對所篩選的文獻(xiàn)進(jìn)行質(zhì)量的評價,通過專用軟件對收集的文獻(xiàn)里包含的數(shù)據(jù)進(jìn)行Meta分析和異質(zhì)性檢驗。結(jié)果:最終納入本研究的文獻(xiàn)共19篇,發(fā)表時間為2004-2016年,共累計1576例患者。經(jīng)Meta分析得知:臨床心絞痛癥狀的改善OR=3.65,95㳠CI[2.70 4.93],P0.00001;臨床中醫(yī)證候的改善OR=4.29,95㳠CI[2.66 6.92],P0.00001;臨床心電圖表現(xiàn)的改善OR=2.53,95㳠CI[1.81 3.53],P0.00001;臨床C反應(yīng)蛋白的改善WMD=-1.53,95㳠CI[-2.35,-0.72],P=0.0002;臨床總膽固醇的改善WMD=-0.65,95㳠CI[-1.16-0.14],P=0.01;低密度脂蛋白的改善WMD=-0.31,95㳠CI[-0.52-0.09],P=0.005。結(jié)論:應(yīng)用清熱活血法治療冠心病不穩(wěn)定性心絞痛時能有效的緩解臨床心絞痛癥狀,改善中醫(yī)證候,改善心電圖表現(xiàn),降低血清炎癥因子水平及血脂水平,其臨床療效確切,是治療冠心病不穩(wěn)定性心絞痛有希望的選擇。
[Abstract]:Objective: to evaluate the clinical efficacy of clearing heat and activating blood circulation method in treating unstable angina pectoris of coronary heart disease by Meta analysis, and to provide evidence based medicine basis for the clinical application of traditional Chinese medicine for clearing heat and activating blood circulation. Methods: according to the scientific retrieval strategy, the inclusion criteria and exclusion criteria were strictly limited. The relevant literatures on the clinical efficacy of clearing heat and activating blood circulation in the treatment of unstable angina pectoris of coronary heart disease were searched by computer search or manual retrieval from the beginning of establishment to October 2016. Jadad scale was used to evaluate the quality of the selected documents, and the data contained in the collected documents were analyzed by Meta and heterogeneity test with special software.
