【摘要】：目的:觀察"醒神益髓開竅"針刺結(jié)合康復(fù)訓(xùn)練治療對腦卒中后單側(cè)空間忽略的影響。方法:將腦卒中后伴有單側(cè)空間忽略的40例患者隨機(jī)分為觀察組與對照組,每組20例。觀察組行頭部百會、四神聰、本神、神庭及雙側(cè)心、心包、腎經(jīng)原穴+絡(luò)穴的"醒神益髓開竅"取穴與常規(guī)循經(jīng)取穴針刺結(jié)合康復(fù)治療,對照組僅行常規(guī)循經(jīng)針刺結(jié)合康復(fù)治療,每天1次,每周5次,治療8周后觀察療效。采用直線二等分試驗(yàn)等單側(cè)空間忽略的指標(biāo)、簡易精神狀態(tài)量表(MMSE)評分、簡式Fugl-Meyer運(yùn)動功能評定評分、改良Barthel指數(shù)指標(biāo)對兩組患者的單側(cè)空間忽略的程度、認(rèn)知功能、運(yùn)動功能、日常生活活動能力進(jìn)行評定。結(jié)果:治療后兩組患者單側(cè)空間忽略指標(biāo)(直線二等分試驗(yàn)、線段劃銷試驗(yàn)、畫鐘試驗(yàn)、平面圖形臨摹試驗(yàn))及MMSE評分、簡式Fugl-Meyer運(yùn)動功能評定、改良Barthel指數(shù)均較療前明顯改善(均P0.01);且觀察組均比對照組改善明顯(P0.05,P0.01)。結(jié)論:"醒神益髓開竅"針刺結(jié)合康復(fù)訓(xùn)練治療能更有效減輕腦卒中后單側(cè)空間忽略程度,并提高患者運(yùn)動功能及日常生活自理能力。
[Abstract]:Aim: to observe the effect of acupuncture combined with rehabilitation training on unilateral spatial neglect after stroke. Methods: forty patients with unilateral spatial neglect after stroke were randomly divided into observation group and control group with 20 cases in each group. The observation group was treated with acupuncture combined with acupoints of "awaken spirit, nourishing marrow, opening orifices" and routine acupoints along meridians, such as head Baihui, Sishen Cong, Benshen, Shenting and bilateral heart, pericardium, kidney meridian and collaterals. The control group was treated with routine acupuncture combined with rehabilitation, once a day, 5 times a week, and observed the curative effect after 8 weeks of treatment. The indexes of unilateral space neglect, such as linear bisection test, simple mental state scale (MMSE), simple Fugl-Meyer motor function evaluation, and modified Barthel index were used to evaluate the degree of unilateral space neglect, cognitive function and motor function of the two groups. The ability of daily life activities is evaluated. Results: after treatment, the unilateral spatial neglect index (linear bisection test, line pin test, clock drawing test, plane pattern copying test) and MMSE score, simple Fugl-Meyer motor function evaluation were observed in both groups. The improved Barthel index was significantly improved compared with that of the control group (P 0.01) and the observation group was significantly improved than the control group. Conclusion: acupuncture combined with rehabilitation training can effectively reduce the degree of unilateral spatial neglect after stroke and improve the patients' motor function and self-care ability of daily life.
