【摘要】：目的 系統(tǒng)評(píng)價(jià)補(bǔ)腎健脾法治療少弱精子癥的臨床療效。方法 檢索相關(guān)期刊論文(CNKI)、維普資源系統(tǒng)(VIP)以及萬(wàn)方數(shù)據(jù)庫(kù),均從建庫(kù)至2014年12月,收集數(shù)據(jù)庫(kù)中有關(guān)補(bǔ)腎健脾法治療少弱精子癥的隨機(jī)對(duì)照試驗(yàn)(RCT)。制定嚴(yán)格的文獻(xiàn)納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn)及檢索策略,并對(duì)納入文獻(xiàn)進(jìn)行方法學(xué)質(zhì)量評(píng)價(jià),采用Revman5.3軟件進(jìn)行統(tǒng)計(jì)分析。結(jié)果 納入8個(gè)RCT,共838例,根據(jù)對(duì)照不同分為單純補(bǔ)腎組和西藥組,但經(jīng)過(guò)仔細(xì)篩選后,其中2項(xiàng)研究組間樣本量差異較大,故針對(duì)單純補(bǔ)腎組做敏感性分析。Meta分析顯示,補(bǔ)腎健脾組在提高精子濃度及a級(jí)精子百分率方面優(yōu)于單純補(bǔ)腎組及西藥組(P0.05)。結(jié)論 在提高精子濃度及a級(jí)精子百分率方面,健脾補(bǔ)腎法有優(yōu)于單純補(bǔ)腎法及西藥治療的趨勢(shì),但所納入文獻(xiàn)質(zhì)量不高,部分?jǐn)?shù)據(jù)差異性過(guò)大,且敏感性分析證實(shí)本研究結(jié)果穩(wěn)定性不高,因此有待更為設(shè)計(jì)嚴(yán)謹(jǐn)、合理的RCT來(lái)證實(shí)補(bǔ)腎健脾法的臨床療效。
[Abstract]:Objective to evaluate the clinical effect of tonifying kidney and invigorating spleen on oligozoospermia. Methods Chinese periodical full-text database (CNKI, Weipu Resource system) and Wanfang database were searched. From the establishment of the database to December 2014, a randomized controlled trial of tonifying kidney and invigorating spleen in treating oligozoospermia was collected. Strict literature inclusion criteria, exclusion criteria and retrieval strategies were formulated, and the methodological quality of the included documents was evaluated, and the statistical analysis was carried out by using Revman5.3 software. Results eight RCTs were included in 838 cases. According to the control group, 838 cases were divided into simple kidney tonifying group and western medicine group. But after careful screening, there were significant differences in sample size between two study groups. Therefore, sensitivity analysis and meta analysis were done for the simple kidney tonifying group. The group of tonifying kidney and invigorating spleen was superior to the group of tonifying kidney and western medicine in improving the sperm concentration and the percentage of grade a spermatozoa. Conclusion the method of invigorating the spleen and tonifying the kidney is superior to the simple method of tonifying the kidney and the treatment of western medicine in improving the sperm concentration and the percentage of grade a spermatozoa, but the quality of the literature is not high, and the difference of some data is too great. Sensitivity analysis confirmed that the stability of the results of this study is not high, so it needs more rigorous design, reasonable RCT to verify the clinical efficacy of tonifying the kidney and invigorating the spleen.
