【摘要】：目的：觀察調(diào)經(jīng)活血膠囊治療寒凝血瘀型原發(fā)性痛經(jīng)的臨床療效。方法：本臨床研究采用隨機、平行對照的研究方法。將60例診斷為寒凝血瘀型原發(fā)性痛經(jīng)的患者,隨機分為兩組,每組各30例,分別為治療組(調(diào)經(jīng)活血膠囊)和對照組(痛經(jīng)寶顆粒)。服藥3個月經(jīng)周期后,觀察兩組患者治療前后中醫(yī)證候評分,疼痛程度評價和治療前后血清PGF2a的變化情況,并統(tǒng)計分析,評價其臨床療效性。停藥3個月經(jīng)周期后,觀察兩組患者的中醫(yī)證候評分,疼痛程度評價,并統(tǒng)計分析,評價其遠期療效。結(jié)果：1.治療組與對照組在年齡、病程、治療前的疼痛程度評價及中醫(yī)證候評分等方面的比較,均無統(tǒng)計學(xué)差異(P0.05),具有可比性。2.疼痛程度評價：治療3個月經(jīng)周期后,治療組總有效率為90.00%,對照組總有效率為93.33%,比較兩組組間療效,無統(tǒng)計學(xué)差異(P0.05)。比較治療前后兩組患者的疼痛程度評價,均較治療前有顯著性差異(P0.01)；治療后兩組組間比較,無統(tǒng)計學(xué)差異(P0.05)。3.中醫(yī)證候綜合療效評價分析：治療3個月經(jīng)周期后,治療組總有效率為93.33%,對照組總有效率為90.00%,比較兩組組間療效,無統(tǒng)計學(xué)差異(P0.05)。比較治療前后兩組患者中醫(yī)證候綜合療效評分,均較治療前有顯著性差異(P0.01)；治療后兩組組間比較,無統(tǒng)計學(xué)差異(P0.05)。4.兩組部分患者治療前血清PGF2a水平比較,無統(tǒng)計學(xué)差異(P0.05)。治療后兩組患者血清PGF2a水平與治療前比較,均有顯著性差異(P0.01),兩組組間療效比較,無統(tǒng)計學(xué)差異(P0.05)。5.停藥3個月經(jīng)周期后疼痛程度評價及中醫(yī)證候總積分變化情況：兩組與治療前比較均有顯著性差異(P0.01)。兩組與治療3個月經(jīng)周期結(jié)束后比較,具有顯著性差異(P0.01)。6.在臨床觀察中,調(diào)經(jīng)活血膠囊治療原發(fā)性痛經(jīng)患者用藥期間未發(fā)現(xiàn)明顯不良反應(yīng)。結(jié)論：調(diào)經(jīng)活血膠囊能有效減輕寒凝血瘀型原發(fā)性痛經(jīng)患者的疼痛程度、改善中醫(yī)證候評分,緩解女性經(jīng)期疼痛及各種不適癥狀,且無明顯不良反應(yīng)。調(diào)經(jīng)活血膠囊降低了患者血清PGF2α水平,可能為其臨床治療的作用機理。
[Abstract]:Objective: to observe the clinical effect of Tiaojing Huoxue capsule on primary dysmenorrhea of cold coagulation and blood stasis. Methods: a randomized, parallel controlled study was used in this clinical study. Sixty patients with primary dysmenorrhea of cold coagulation and blood stasis type were randomly divided into two groups: treatment group (Tiaojing Huoxue capsule) and control group (Tongjingbao granule). After taking medicine for 3 menstrual cycles, the TCM syndromes score, pain degree evaluation and changes of serum PGF2a before and after treatment were observed, and statistical analysis was made to evaluate the clinical efficacy of the two groups. After three menstrual cycles, the TCM syndromes score, pain degree evaluation and statistical analysis were observed to evaluate the long-term efficacy of the two groups. The result is 1: 1. There was no statistical difference between the treatment group and the control group in age, course of disease, pain degree evaluation before treatment and TCM syndrome score. Pain evaluation: after 3 menstrual cycles, the total effective rate of the treatment group was 90.00g and the total effective rate of the control group was 93.330.There was no statistical difference between the two groups (P 0.05). There was significant difference in pain degree between the two groups before and after treatment (P 0.01), but there was no significant difference between the two groups after treatment. Evaluation of Syndromes of TCM Syndromes: after 3 menstrual cycles, the total effective rate of the treatment group was 93.33, and the total effective rate of the control group was 90.00.There was no statistical difference between the two groups (P 0.05). There was a significant difference between the two groups before and after treatment compared with before and after treatment, and there was no significant difference between the two groups after treatment (P 0.05. 4). There was no significant difference in serum PGF2a levels between the two groups before treatment (P 0.05). There was significant difference in serum PGF2a level between the two groups after treatment and before treatment (P 0.01). There was no significant difference in the curative effect between the two groups (P 0.05). Evaluation of pain degree and change of total integral of TCM syndromes after three menstrual cycles: there were significant differences between the two groups and before treatment (P 0.01). There was a significant difference between the two groups after the end of three menstrual cycles. In clinical observation, there was no obvious adverse reaction during the treatment of primary dysmenorrhea with Tiaojing Huoxue capsule. Conclusion: Tiaojing Huoxue capsule can effectively alleviate the pain degree of primary dysmenorrhea patients with cold coagulation and blood stasis, improve the TCM syndrome score, relieve the pain and all kinds of uncomfortable symptoms of female menstrual period, and have no obvious adverse reactions. Tiaojing Huoxue capsule decreased the level of serum PGF2 偽, which may be the mechanism of clinical treatment.
