【摘要】：目的:針對視神經(jīng)萎縮的病證特點,以中醫(yī)理論為指導(dǎo),確立養(yǎng)血柔肝的治療方法。通過對治療前后肝郁血虛型視神經(jīng)萎縮患者的視力、視野、神經(jīng)纖維層厚度的觀察,評價其臨床療效,為中醫(yī)治療視神經(jīng)萎縮提供新的方法和思路。方法:將符合入選標準的40例視神經(jīng)萎縮患者,隨機分成兩組。其中,治療組20例27只眼,口服養(yǎng)血柔肝明目湯配合針刺;對照組20例25只眼,采用針刺治療。兩組都配合口服甲鈷胺分散片、穴位貼服。將兩組患者治療前后中醫(yī)癥候積分、視力、視野(MD、PSD)以及RNFL厚度進行比較,評價其臨床療效。結(jié)果:治療組中醫(yī)癥候總有效率為96.7%,對照組中醫(yī)癥候總有效率為90.4%,兩組療效比較差異有統(tǒng)計學(xué)意義(P0.05)。治療組視力總有效率為85.2%,對照組視力總有效率為60%,兩組療效比較差異有統(tǒng)計學(xué)意義。治療組在視野、RNFL厚度等方面與對照組比較差異均有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:養(yǎng)血柔肝法治療肝郁血虛型視神經(jīng)萎縮療效明顯,能顯著提高患者視覺功能,改善癥狀,具有良好的臨床應(yīng)用價值。
[Abstract]:Objective: to establish the treatment of nourishing blood and softening liver under the guidance of traditional Chinese medicine (TCM) according to the characteristics of optic nerve atrophy. By observing the visual acuity, visual field and the thickness of nerve fiber layer in patients with optic nerve atrophy of liver stagnation and blood deficiency before and after treatment, the clinical effect was evaluated, and a new method and thought for the treatment of optic nerve atrophy by traditional Chinese medicine was provided. Methods: 40 patients with optic nerve atrophy were randomly divided into two groups. Among them, 20 cases (27 eyes) in the treatment group were treated by acupuncture combined with Yangxue Rugan Mingmu decoction and 25 eyes in the control group. Both groups were combined with oral methylcobalamin dispersible tablets, acupoint application. The scores of TCM symptoms, visual acuity, visual field and RNFL thickness before and after treatment were compared between the two groups, and the clinical efficacy was evaluated. Results: the total effective rate of TCM symptoms in the treatment group was 96.7m, while that in the control group was 90.4. The difference between the two groups was statistically significant (P 0.05). The total effective rate of visual acuity was 85.2 in the treatment group and 60 in the control group. The RNFL thickness in the treatment group was significantly different from that in the control group (P 0.05). Conclusion: the method of nourishing blood and softening liver is effective in the treatment of optic nerve atrophy with liver stagnation and blood deficiency. It can significantly improve the visual function and symptoms of the patients and has a good clinical application value.
