【摘要】：目的觀察傷科消腫湯治療脛腓骨骨折術(shù)后肢體腫脹的臨床療效。方法將山東臨沂市人民醫(yī)院2015年7月～2016年12月收治的132例脛腓骨骨折患者納入研究,隨機(jī)分為實驗1組(傷科消腫湯聯(lián)合甘露醇治療)、實驗2組(單純傷科消腫湯治療)和對照組(單純甘露醇治療),每組44例。分別于術(shù)后第1天、第3天、第5天、第7天測量患側(cè)小腿周徑并比較3組患者的腫脹值(患側(cè)小腿周徑—健側(cè)小腿周徑),并于術(shù)后第7天參照《中藥新藥臨床研究指導(dǎo)原則》標(biāo)準(zhǔn)評價療效。結(jié)果3組患者腫脹值治療過程中不同時間點間的比較,差異有統(tǒng)計學(xué)意義,存在時間效應(yīng),治療后腫脹值均逐漸減小(F=3154.562,P=0.000);3組患者腫脹值總體上比較,組間差異有統(tǒng)計學(xué)意義,存在分組效應(yīng)(F=5.057,P=0.008),兩兩比較,實驗1組腫脹值減少優(yōu)于實驗2組,差異有統(tǒng)計學(xué)意義(P=0.0070.0125);實驗1組腫脹值減少優(yōu)于對照組,差異有統(tǒng)計學(xué)意義(P=0.0060.0125);實驗2組腫脹值和對照組相比,組間差異無統(tǒng)計學(xué)意義(P=0.9740.0125);時間因素和分組因素存在交互效應(yīng)(F=33.757,P=0.000)。術(shù)后第7天,實驗1組的臨床療效優(yōu)于實驗2組和對照組,組間差異有統(tǒng)計學(xué)意義(Z=-3.772,P=0.000;Z=-5.136,P=0.000);實驗2組和對照組相比,組間差異無統(tǒng)計學(xué)意義(Z=-1.533,P=0.125)。結(jié)論傷科消腫湯治療脛腓骨骨折術(shù)后肢體腫脹療效確切,且和甘露醇聯(lián)合應(yīng)用,效果更佳,值得臨床推廣應(yīng)用。
[Abstract]:Objective to observe the clinical effect of Shangke Xiaozhong decoction on limb swelling after tibia and fibula fracture. Methods 132 patients with tibia and fibula fractures treated in Shandong Linyi people's Hospital from July 2015 to December 2016 were included in the study. The rats were randomly divided into experimental group (Shangke Xiaozong decoction combined with mannitol), experimental group 2 (simple treatment of Shangke Xiaozong decoction) and control group (simple mannitol treatment), with 44 cases in each group. On day 1, day 3, day 5, respectively, On the 7th day, the peripheral diameter of the affected leg was measured and the swelling values of the three groups were compared. The therapeutic effect was evaluated according to the guideline of clinical research of new Chinese medicine on the 7th day after operation. Results there were significant differences in the swelling values between the three groups in the course of treatment, and there was a time effect. After treatment, the swelling values of the three groups were gradually reduced. The swelling values of the three groups were compared with each other in general, and the differences between the three groups were statistically significant. The swelling value of group 1 was significantly lower than that of group 2 (P 0.0070.0125), and the swelling value of group 1 was better than that of control group (P 0.0060.0125), and the swelling value of group 2 was higher than that of control group (P < 0.05). There was no significant difference between the two groups (P < 0. 9740. 0125), but there was interaction between time factors and group factors (P < 0. 000). On the 7th day after operation, the clinical efficacy of experimental group 1 was better than that of experimental group 2 and control group, and the difference between the two groups was statistically significant (P < 0.05), but there was no significant difference between experimental group 2 and control group (P 0.125). Conclusion Shangke Xiaozong decoction is effective in treating limb swelling after tibia and fibula fracture, and it is better in combination with mannitol. It is worth popularizing in clinic.
