【摘要】：目的:觀察參補(bǔ)湯治療氣虛血瘀型月經(jīng)過(guò)多的臨床療效。明確該方治療氣虛血瘀型月經(jīng)過(guò)多的臨床療效,初步評(píng)價(jià)其臨床有效性及安全性,以期為月經(jīng)過(guò)多患者提供更可靠的治療方法。方法:本研究遵循平行隨機(jī)對(duì)照的臨床研究方法,將符合納入標(biāo)準(zhǔn)的氣虛血瘀型月經(jīng)過(guò)多患者隨機(jī)分為治療組(56例)和對(duì)照組(54例),脫落2例,實(shí)際完成治療組(54例)、對(duì)照組(54例),分別于經(jīng)期第3天口服參補(bǔ)湯(治療組)和宮血停顆粒(對(duì)照組)進(jìn)行治療,連續(xù)5天,療程為3個(gè)月經(jīng)周期。觀察并記錄兩組患者治療前后月經(jīng)及相關(guān)的中醫(yī)臨床癥狀評(píng)分、血紅蛋白、子宮內(nèi)膜厚度、血清雌二醇、血清孕酮變化及用藥過(guò)程中有無(wú)不良反應(yīng),對(duì)觀察結(jié)果進(jìn)行統(tǒng)計(jì)分析。結(jié)果:兩組治療前、后自身對(duì)照,主要療效觀察指標(biāo):月經(jīng)證候積分、全身證候積分、總證候積分相比,P0.01,差異顯著,具有統(tǒng)計(jì)學(xué)意義;各檢查指標(biāo):子宮內(nèi)膜厚度、血清E2、P水平,P0.05,無(wú)統(tǒng)計(jì)學(xué)意義;血紅蛋白(Hb)值,P0.05,具有統(tǒng)計(jì)學(xué)意義。兩組治療后組間對(duì)照,總療效:治療組總有效率為92.59%,對(duì)照組組總有效率為79.63%,P0.05,有統(tǒng)計(jì)學(xué)意義;主要療效觀察指標(biāo):月經(jīng)證候中月經(jīng)量、月經(jīng)質(zhì)、總證候積分相比,P0.05,具有統(tǒng)計(jì)學(xué)意義;全身證候積分相比,P0.05,無(wú)統(tǒng)計(jì)學(xué)意義,但小腹疼痛、口干不欲飲單個(gè)證候積分比較P0.05,具有統(tǒng)計(jì)學(xué)意義;各檢查指標(biāo):子宮內(nèi)膜厚度、血清E2、P水平,P0.05,無(wú)統(tǒng)計(jì)學(xué)意義;Hb值相比,P0.05,具有統(tǒng)計(jì)學(xué)意義。治療組、對(duì)照組均未觀察到不良反應(yīng)。結(jié)論:參補(bǔ)湯對(duì)治療氣虛血瘀型月經(jīng)過(guò)多患者臨床療效確切,能恢復(fù)月經(jīng)量,較好的改善中醫(yī)相關(guān)癥狀,顯著降低中醫(yī)證狀積分,提高血紅蛋白值,且未見(jiàn)明顯副作用和不良反應(yīng),值得臨床推廣、應(yīng)用。
[Abstract]:Objective: to observe the clinical effect of Shenbu decoction on menorrhagia of qi deficiency and blood stasis type. To evaluate the clinical efficacy and safety of the prescription in treating menstruation with qi deficiency and blood stasis, and to provide a more reliable treatment for the patients with menorrhagia. Methods: according to the clinical study method of parallel randomized control, the patients with menorrhagia of Qi deficiency and blood stasis type were randomly divided into treatment group (n = 56) and control group (n = 54). The treatment group (n = 54) and control group (n = 54) were treated with Shenbu decoction (treatment group) and Gongxuting granule (control group) respectively on the 3rd day of menstrual period for 5 days for 3 menstrual cycles. The clinical symptoms of menstruation, hemoglobin, endometrial thickness, serum estradiol, serum progesterone and adverse reactions were observed and recorded before and after treatment. Results: before and after treatment, the main outcome measures: menstrual syndrome integral, systemic syndrome integral, total syndrome score, compared with P0.01, the difference was significant, with statistical significance. There was no significant difference in serum E _ 2P level (P _ (0.05) and hemoglobin (HB) value (P _ (0.05). The total effective rate of the treatment group was 92.599.The total effective rate of the control group was 79.63g / kg P 0.05, which was statistically significant; main outcome measures: menstrual syndrome, menstrual quality, menstrual quality, Compared with P0.05, the score of total syndromes was statistically significant, the score of systemic syndrome was not statistically significant compared with that of P0.05, but the scores of abdominal pain and dry mouth were statistically significant compared with P0.05. The serum E _ 2 P level and P _ (0.05) had no statistical significance compared with that of P _ (0.05), and there was no significant difference between P _ (0.05) and P _ (0.05). No adverse reactions were observed in the treatment group and the control group. Conclusion: Shenbu decoction is effective in treating excessive menstruation of qi deficiency and blood stasis type, can recover menstrual volume, improve TCM symptoms, decrease syndromes of TCM, and increase hemoglobin value. There were no obvious side effects and adverse reactions, so it was worth popularizing and applying.
