【摘要】：胃痞是臨床常見的脾胃系病癥之一,以脘腹部的脹滿不適為主要表現(xiàn)征象。胃痞的發(fā)生與氣的關(guān)系密切,而氣、血、津液是一個有機(jī)整體,因此,血和津液在胃痞的辨證治療中的重要性亦不可忽略。氣病病機(jī)在胃痞的辨治過程中運用廣泛,吾師楊康認(rèn)為單憑氣病病機(jī)辨治胃痞是不夠的,在中醫(yī)的整體觀念中,氣、血、津液是相互依存、相互制約、相互為用的,任何一個病變都有可能影響其他兩個的正常生理功能。氣病病機(jī)是導(dǎo)致胃痞主證的影響病機(jī),而血病、津液病對其兼證的影響不可忽略。氣、血、津液不僅是維護(hù)脾胃正常生理功能的基本營養(yǎng)物質(zhì),又是影響脾胃致病的病理因素。理解了氣血津液病機(jī),不僅幫助理解了病程中產(chǎn)生的不同病理產(chǎn)物,又有助于把握疾病的轉(zhuǎn)歸。吾師將臨床常見胃痞患者分為脾虛濕滯型、氣虛血瘀型、肝胃不和型和胃陰虧虛型。通過補(bǔ)氣行氣、活血化瘀、滋陰利濕達(dá)到消痞的同時,還顧養(yǎng)了脾胃之本。本文分為三部分,第一部分主要通過古代文獻(xiàn)對痞的字義、痞的病因病機(jī)、癥候分析及治療進(jìn)行研究;第二部分通過介紹現(xiàn)代醫(yī)學(xué)對胃痞的認(rèn)識;第三部分闡述氣、血、津與脾胃的生理、病理關(guān)系,論述胃痞的分型和辨證機(jī)理,并通過病例總結(jié)吾師楊康教授從事臨床30年來辨證治療胃痞經(jīng)驗。
[Abstract]:Gastric distension is one of the common diseases of spleen and stomach. The occurrence of gastric distension is closely related to qi, and qi, blood and body fluid are an organic whole. Therefore, the importance of blood and body fluid in the treatment of gastric distension can not be ignored. The pathogenesis of Qi disease is widely used in the process of differentiation and treatment of gastric distresses. Yang Kang, our teacher, thinks that it is not enough to differentiate and treat stomach distresses by relying on the pathogenesis of qi diseases. In the overall concept of traditional Chinese medicine, qi, blood and body fluid depend on each other, restrict each other, and are used for each other. Any one lesion may affect the normal physiological function of the other two. The pathogenesis of qi disease is the influence of the main syndrome of gastric distension, but the influence of blood disease and body fluid disease on its syndromes can not be ignored. Qi, blood and body fluid are not only the basic nutrients to maintain the normal physiological function of spleen and stomach, but also the pathological factors affecting the pathogenesis of spleen and stomach. Understanding the pathogenesis of qi, blood and body fluid not only helps to understand the different pathological products produced in the course of disease, but also helps to grasp the outcome of the disease. The patients were divided into spleen deficiency and dampness stagnation type, qi deficiency and blood stasis type, liver and stomach disharmony type and stomach yin deficiency type. Invigorating qi, promoting blood circulation and removing blood stasis, nourishing yin and dampness to achieve the puffiness, but also care to nourish the spleen and stomach. This article is divided into three parts, the first part mainly through the ancient literature on the meaning of ruffian, the cause and pathogenesis of the disease, symptoms analysis and treatment; the second part through the introduction of modern medicine on the understanding of gastric distension; the third part of the description of qi, blood, The physiological and pathological relationship between Jin and spleen and stomach, the classification and syndrome differentiation mechanism of gastric distension were discussed, and the experience of 30 years' clinical treatment of gastric distension was summarized by Professor Yang Kang.
