【摘要】：目的:基于“中醫(yī)傳承輔助平臺(V2.5)”軟件,分析導(dǎo)師丁元慶教授治療偏頭痛的用藥經(jīng)驗,進(jìn)一步豐富和發(fā)展偏頭痛的防治理論,并為名中醫(yī)專家經(jīng)驗的總結(jié)方法提供思路和參考。方法:采用關(guān)聯(lián)規(guī)則Apriori算法、復(fù)雜系統(tǒng)熵聚類等數(shù)據(jù)挖掘方法,分析丁元慶教授治療偏頭痛的病案及處方,探討丁元慶教授治療偏頭痛的經(jīng)驗以及論治規(guī)律。結(jié)果:通過對516例病案進(jìn)行分析,顯示偏頭痛的核心癥狀群為有家族史、平素急躁易怒、跳痛或脹痛多見、中重度頭痛、部位以顳側(cè)為主、經(jīng)休息或口服止痛片緩解、伴惡心甚則嘔吐、畏光畏聲、心煩、舌質(zhì)暗紅,苔薄黃膩,脈沉細(xì)等;常見的證候有濕熱內(nèi)蘊(yùn)證、痰熱生風(fēng)證、瘀熱內(nèi)擾證、陰虛陽旺證等4個。對516首處方進(jìn)行分析,得出丁元慶教授治療偏頭痛的常用藥物為天麻、僵蠶、當(dāng)歸、菊花、蟬蛻、珍珠母、制何首烏、玄參、川芎、夏枯草等,并通過復(fù)雜熵聚類法挖掘出18個潛在藥物組合和7組候選新藥隊。結(jié)論:丁元慶教授治療偏頭痛經(jīng)驗豐富,獨具特色。治分緩急,發(fā)作期重治風(fēng)火、祛邪以治標(biāo),緩解期扶正治本為基本治則。在辨證論治的基礎(chǔ)上,善用條達(dá)肝氣,疏暢情志之品,以使氣機(jī)調(diào)暢,氣血陰陽調(diào)和;并重視祛除伏邪;且根據(jù)患者的誘因特點,強(qiáng)調(diào)規(guī)避誘因,防治結(jié)合。
[Abstract]:Objective: to analyze the experience of professor Ding Yuanqing in the treatment of migraine, and to enrich and develop the theory of prevention and treatment of migraine. And for the name of TCM experts experience summary method to provide ideas and references. Methods: by using association rule Apriori algorithm and entropy clustering of complex systems, the medical records and prescriptions of Professor Ding Yuanqing in treating migraine were analyzed, and the experience and treatment rule of professor Ding Yuanqing in treating migraine were discussed. Results: the analysis of 516 cases showed that the core symptoms of migraine were family history, irritability and irritability, jumping pain or flatulence, moderate or severe headache, mainly temporal-side headache, relieved by rest or oral analgesic tablets. Accompanied by nausea and vomiting, photophobia, fear of sound, upset, tongue dark red, thin yellow and greasy fur, fine pulse, etc.; the common syndromes include damp-heat internal accumulation, phlegm heat and wind, blood stasis and heat internal disturbance, Yin deficiency and yang flourishing syndrome. From the analysis of 516 prescriptions, it is concluded that the common drugs used by Professor Ding Yuanqing for treating migraine are Gastrodia elata, silkworm, angelica sinensis, chrysanthemum, cicada slough, mother of pearl, making Polygonum multiflorum, Radix Polygonum multiflorum, Rhizoma Chuanxiong 18 potential drug combinations and 7 new drug candidate teams were mined by complex entropy clustering. Conclusion: professor Ding Yuanqing has rich experience in treating migraine. The treatment points are urgent, the attack period re-cure wind-fire, dispelling evil to cure the symptoms, the remission period to cure the root-cure as the basic principle. On the basis of syndrome differentiation and treatment, make good use of liver-qi, sparsely unobstructed emotional goods, so as to make Qi and Qi adjust unobstructed, Qi and blood Yin and Yang to reconcile; and pay attention to dispelling pathogenic factors, and according to the characteristics of inducement of patients, emphasize evading inducement and combining prevention and treatment.
