【摘要】：目的:對(duì)胃十二指腸潰瘍大出血患者鼻胃管推注祛瘀止血散的止血改善程度進(jìn)行評(píng)估。方法:選取2014年6月至2016年2月胃十二指腸潰瘍大出血患者86例,按隨機(jī)數(shù)字法分為觀察組和對(duì)照組各43例,對(duì)照組給予奧美拉唑40 mg、對(duì)氨甲基苯甲酸注射液0.2 g、西米替丁0.2 g靜脈滴注,qd。觀察組在對(duì)照組治療基礎(chǔ)上加用祛瘀止血散鼻胃管推注,2組均治療14 d為1個(gè)療程,對(duì)2組患者治療前后癥狀評(píng)分和實(shí)驗(yàn)室指標(biāo)進(jìn)行觀察評(píng)估。結(jié)果:2組治療后嘔血、黑便等癥狀評(píng)分與治療前比較均有明顯降低,且觀察組低于對(duì)照組,觀察組治療后血小板計(jì)數(shù)(blood platelet,PLT)顯著高于對(duì)照組,便隱血(fecal occult blood)陽(yáng)性率顯著低于對(duì)照組;觀察組治療后經(jīng)Rockall評(píng)分高危占13.95%,低危占65.12%,低危人數(shù)顯著高于對(duì)照組,高危人數(shù)顯著低于對(duì)照組(Uc=-3.8606),2組比較差異有統(tǒng)計(jì)學(xué)意義。結(jié)論:鼻胃管推注祛瘀止血散可以增強(qiáng)胃十二指腸潰瘍大出血患者血小板聚集功能,促進(jìn)創(chuàng)口止血愈合,明顯改善患者臨床癥狀。
[Abstract]:Objective: to evaluate the degree of hemostasis in patients with massive bleeding of gastroduodenal ulcer. Methods: 86 patients with massive bleeding of gastroduodenal ulcer from June 2014 to February 2016 were randomly divided into observation group (n = 43) and control group (n = 43). The control group was given omeprazole (40 mg), p-aminomethylbenzoic acid injection (0.2 g) and simitidine (0.2 g iv). On the basis of the treatment of the control group, the observation group was treated with removing stasis and hemostatic powder for 14 days. The symptom scores and laboratory indexes were observed and evaluated before and after treatment in both groups. Results after treatment, the scores of hematemesis, melanology and other symptoms in the observation group were significantly lower than those in the control group, and the platelet count in the observation group was significantly higher than that in the control group. The positive rate of fecal occult blood in the observation group was significantly lower than that in the control group, and the high risk rate was 13.95, the low risk was 65.12 in the observation group, and the number of the low risk was significantly higher than that in the control group, and the number of high risk was significantly lower than that in the control group, which was significantly lower than that in the control group. Conclusion: nasogastric tube injection of Quyu Zhixue Powder can enhance platelet aggregation in patients with massive bleeding of gastroduodenal ulcer, promote wound hemostasis healing, and improve the clinical symptoms of the patients.
