【摘要】：目的:通過觀察心腦通絡液治療冠心病早搏(氣虛血瘀型)的中醫(yī)證候積分及動態(tài)心電圖指標,初步探討中藥復方制劑對冠心病早搏的臨床療效。方法:選擇符合入組標準的42例門診冠心病早搏患者(中醫(yī)辨證為氣虛血瘀型),給予服用心腦通絡液治療,每日3次,每次20ml,前后治療4周。在治療前和治療后分別記錄患者的中醫(yī)證候積分、24h動態(tài)心電圖早搏數(shù)量、平均心率及心率變異性指標數(shù)據(jù),數(shù)據(jù)采用SPSS 19.0軟件分析結(jié)果。結(jié)果:1、中醫(yī)證候療效:治療前后中醫(yī)證侯總積分比較,治療后中醫(yī)證侯總積分減小,差異具有統(tǒng)計學意義(P0.01);治療后中醫(yī)證侯療效的總有效率為87.5%,其中顯效者有14例(35%),有效者有21例(52.5%),無效者有5例(12.5%)。2、24h動態(tài)心電圖療效:治療前后早搏數(shù)量比較,治療后早搏數(shù)量減少,差異具有統(tǒng)計學意義(P0.01);治療前后平均心率比較,差異無統(tǒng)計學意義(P0.05);治療前后心率變異性(SDNN、SDANN、SDNNinde x、rMSSD、PNN50)比較,差異有統(tǒng)計學意義(P0.01)。治療后早搏的總有效率為80.00%,其中室上性早搏、室性早搏、室上性合并室性早搏治療的有效率分別是81.82%、75.00%、80.00%;結(jié)論:1、心腦通絡液能改善冠心病早搏(氣虛血瘀型)患者的中醫(yī)證候。2、心腦通絡液能減少冠心病早搏(氣虛血瘀型)患者的早搏數(shù)量,不影響平均心率。3、心腦通絡液能提高冠心病早搏(氣虛血瘀型)患者的心率變異性,改善其自主神經(jīng)功能。
[Abstract]:Objective: to investigate the clinical effect of traditional Chinese medicine (TCM) compound preparation on premature beats of coronary heart disease (CHD) by observing TCM syndromes integral and dynamic electrocardiogram (ECG) indexes in treating premature beats of coronary heart disease (CHD) with Xinnao Tongluo liquid (XNTL). Methods: 42 outpatients with premature beats of coronary heart disease (TCM syndrome differentiation: Qi deficiency and blood stasis type) were selected and treated with Xinnao Tongluo liquid 3 times a day, 20 ml each time for 4 weeks before and after treatment. Before and after treatment, the scores of TCM syndromes and 24 h ambulatory ECG premature beats, mean heart rate and heart rate variability were recorded respectively. The data were analyzed by SPSS 19.0 software. Results 1. Curative effect of TCM syndrome: the total score of TCM syndrome before and after treatment was compared, and after treatment, the total score of TCM syndromes decreased. The difference was statistically significant (P 0.01), the total effective rate of TCM syndromes after treatment was 87.5%, of which 14 cases were effective, 21 cases were effective and 52.5 cases were effective, 5 cases were ineffective, and 5 cases had 24 hours ambulatory electrocardiogram effect: comparison of the number of premature beats before and after treatment. After treatment, the number of premature beats decreased, the difference was statistically significant (P 0.01), the mean heart rate before and after treatment had no significant difference (P 0.05), and the heart rate variability (SDNV) was SDNN Nine de XM MSSDPNN50 (P 0.01). The total effective rate of premature beats after treatment was 80.00g, in which supraventricular premature beats, ventricular premature beats, The effective rates of supraventricular and ventricular premature beats were 81.82 and 75.00, respectively. Conclusion: 1, Xinnao Tongluo liquid can improve TCM syndromes of patients with premature coronary heart disease (Qi deficiency and blood stasis type), and Xinnao Tongluo liquid can reduce premature beats of coronary heart disease (Qi deficiency and blood stasis type). The number of premature beats in patients, Without affecting the mean heart rate, Xinnao Tongluo liquid could improve the heart rate variability and improve the autonomic nervous function of patients with premature beats of coronary heart disease (qi deficiency and blood stasis type).
