【摘要】：目的:觀察益腎清利活血法對于特發(fā)性膜性腎病患者的治療效果,從中醫(yī)理論探討該治療方法的可行性,分析總結(jié)該治法的特點。方法:本研究分為兩個部分。理論研究為收集古今中外文獻(xiàn)以分別闡述西醫(yī)與中醫(yī)對于特發(fā)性膜性腎病的認(rèn)識。臨床研究收集特發(fā)性膜性腎病患者40例,在西醫(yī)基礎(chǔ)治療同時以益腎清利活血法治療6個月,通過自身對照的方法,觀察治療前后患者相關(guān)指標(biāo)的變化,做出統(tǒng)計分析,并闡述孫偉教授以該法治療特發(fā)性膜性腎病的經(jīng)驗。結(jié)果:1.癥狀療效:經(jīng)6個月治療,38名患者臨床癥狀積分較治療前下降,差值有統(tǒng)計學(xué)意義(P＜0.05),該治法癥狀療效總有效率為82.5%;患者腰脊酸痛、疲倦乏力、浮腫、少氣懶言、手足心熱、肢體困重等主要癥狀均獲得顯著改善(P0.05)。2.臨床療效:40例患者經(jīng)治療后完全緩解的有6名(15%),部分緩解的病例共28例(70%),未緩解6例(15%),總體有效率為85%。患者UTP、D-D、TC及TG均經(jīng)治療后下降,前三者有統(tǒng)計學(xué)意義(P＜0.05)。6個月后患者Alb顯著提升(P＜0.05),Scr穩(wěn)定在正常范圍,差異無統(tǒng)計學(xué)意義(P0.05)。3.安全性指標(biāo):治療過程中未出現(xiàn)明顯不良事件,安全性指標(biāo)(K+、ALT、AST、Hb)于正常范圍內(nèi)穩(wěn)定,差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論:研究顯示益腎清利活血法可顯著改善患者的臨床癥狀,并可在6個月療程內(nèi)達(dá)到降低尿蛋白、調(diào)脂抗凝、提升血漿白蛋白、穩(wěn)定腎功能等效果,且該治法具有安全可行性。
[Abstract]:Objective: to observe the therapeutic effect of Yishen Qingli Huoxue method on idiopathic membranous nephropathy and discuss the feasibility of this method from the theory of traditional Chinese medicine. Methods: this study was divided into two parts. The theoretical study is to collect the literature of ancient and modern countries to explain the understanding of idiopathic membranous nephropathy between western medicine and traditional Chinese medicine. In the clinical study, 40 patients with idiopathic membranous nephropathy were treated with Yishen Qingli Huoxue method at the same time in western medicine for 6 months. Through self-control method, the changes of related indexes before and after treatment were observed and statistical analysis was made. Professor Sun Wei's experience in treating idiopathic membranous nephropathy with this method was described. The result is 1: 1. Symptom efficacy: after 6 months of treatment, the clinical symptom score of 38 patients was lower than that before treatment, the difference was statistically significant (P < 0.05), the total effective rate of the treatment was 82.5. The main symptoms such as limb weight were significantly improved (P0.05. 2). There were 6 patients with complete remission after treatment, 28 patients with partial remission, and 6 patients without remission. The overall effective rate was 85. The levels of TC and TG in patients with UTP D-DN decreased after treatment, and the first three were significantly decreased (P < 0.05). After 6 months, the Alb of the patients increased significantly (P < 0.05) and the SCR was stable in the normal range (P < 0.05), and there was no significant difference (P < 0.05). Safety index: there were no obvious adverse events in the course of treatment, and the safety index was stable in normal range (P 0.05). Conclusion: the study shows that Yishen Qingli Huoxue method can significantly improve the clinical symptoms of patients, and can reduce urinary protein, regulate lipid and anticoagulation, improve plasma albumin, stabilize renal function, and so on. The method is safe and feasible.
