【摘要】：目的與意義:腦卒中是指迅速出現(xiàn)局部或彌漫性腦功能缺失的腦血管臨床事件,患者發(fā)病后會(huì)出現(xiàn)運(yùn)動(dòng)功能障礙,某種意義上嚴(yán)重影響病人的生活質(zhì)量。采用系統(tǒng)科學(xué)、效果良好的康復(fù)治療手段和方法可以減少腦卒中偏癱患者的運(yùn)動(dòng)功能障礙。太極拳是極具代表性的中國(guó)傳統(tǒng)拳術(shù),既可用于技擊防身,又能強(qiáng)身健體、防治疾病。鑒于此,本研究針對(duì)太極拳技術(shù)動(dòng)作進(jìn)行了整合、簡(jiǎn)化處理,形成簡(jiǎn)化八式太極拳,并介入到腦卒中患者運(yùn)動(dòng)功能障礙的臨床治療中,驗(yàn)證太極拳針對(duì)運(yùn)動(dòng)功能障礙的康復(fù)效果與價(jià)值,為太極拳在腦卒中患者運(yùn)動(dòng)功能障礙治療中的應(yīng)用和推廣奠定基礎(chǔ)。研究對(duì)象:本實(shí)驗(yàn)所有的研究對(duì)象均來(lái)源于2014年到2015年期間黑龍江省中醫(yī)藥大學(xué)附屬第二醫(yī)院康復(fù)中心就診的腦卒中患者,分為太極組和對(duì)照組,太極組完成32例患者,對(duì)照組完成26例患者,每名實(shí)驗(yàn)對(duì)象均為自愿參加本實(shí)驗(yàn),并簽署實(shí)驗(yàn)所需的知情同意書。研究方法:在此研究中分別應(yīng)用了文獻(xiàn)資料法、專家訪談法、實(shí)驗(yàn)法、數(shù)理統(tǒng)計(jì)法。其中實(shí)驗(yàn)法為主要研究方法,首先形成適合腦卒中患者的太極拳動(dòng)作內(nèi)容和方法,制定易于掌握、操作性強(qiáng)的簡(jiǎn)化八式太極拳干預(yù)測(cè)試。兩組實(shí)驗(yàn)對(duì)象均接受院內(nèi)常規(guī)藥物治療,以及相關(guān)的常規(guī)康復(fù)治療,太極組在此基礎(chǔ)上增加太極拳康復(fù)訓(xùn)練。分別采用Fugl-Meyer上下肢運(yùn)動(dòng)能力測(cè)試法、PK254平衡功能評(píng)估測(cè)試法,測(cè)試干預(yù)前,干預(yù)后4周、8周兩組患者運(yùn)動(dòng)功能障礙的臨床康復(fù)狀況。測(cè)試結(jié)束后使用SPSS17.0版統(tǒng)計(jì)軟件對(duì)實(shí)驗(yàn)所得數(shù)據(jù)進(jìn)行數(shù)理統(tǒng)計(jì),并做出科學(xué)分析。結(jié)論:簡(jiǎn)化八式太極拳技法對(duì)腦卒中患者的上下肢運(yùn)動(dòng)功能康復(fù)及平衡能力恢復(fù)有明顯的治療作用。
[Abstract]:Objective and significance: stroke refers to the rapid occurrence of local or diffuse cerebral functional loss of the cerebrovascular clinical events, the patient will appear after the onset of motor dysfunction, a serious impact on the quality of life of patients. Using systematic science and good rehabilitation treatment can reduce motor dysfunction in stroke hemiplegic patients. Taijiquan is a representative of Chinese traditional boxing, which can be used not only to fight and defend oneself, but also to strengthen one's health and prevent and cure diseases. In view of this, this study integrated Taijiquan technique, simplified treatment, formed simplified eight-style Taijiquan, and intervened in the clinical treatment of stroke patients with motor dysfunction. To verify the rehabilitation effect and value of Taijiquan for motor dysfunction, and to lay a foundation for the application and popularization of Taijiquan in the treatment of motor dysfunction in stroke patients. Participants: all the subjects in this study were stroke patients from the rehabilitation center of the second affiliated Hospital of Heilongjiang University of traditional Chinese Medicine from 2014 to 2015. They were divided into Taiji group and control group. In the control group, 26 patients were enrolled in the experiment voluntarily, and the informed consent was signed. Methods: the methods of literature, expert interview, experiment and mathematical statistics were used in this study. Among them, experimental method is the main research method. Firstly, the content and method of Taijiquan for stroke patients are formed, which is easy to master and easy to master, which can simplify the intervention test of eight-style Taijiquan. Both groups received routine medical treatment in hospital and related routine rehabilitation therapy. Taiji group added Taijiquan rehabilitation training on this basis. The Fugl-Meyer test of upper and lower extremity motor ability and PK254 balance function evaluation method were used to test the clinical rehabilitation status of motor dysfunction in the two groups before and after intervention 4 weeks and 8 weeks after intervention. At the end of the test, the statistical software of SPSS17.0 was used to make mathematical statistics and scientific analysis of the experimental data. Conclusion: the simplified eight-style Taijiquan technique has obvious therapeutic effect on the rehabilitation of upper and lower limb motor function and the recovery of balance ability in stroke patients.
