【摘要】：黃元御(1705-1758),清代乾隆年間著名醫(yī)家,非常推崇《內(nèi)經(jīng)》、《難經(jīng)》、《傷寒論》、《金匱要略》等經(jīng)典著作,并繼承陰陽學(xué)說、五行學(xué)說、六氣學(xué)說以及氣化學(xué)說,將其精髓融會貫通于一體,結(jié)合多年臨床確踐廣聞、心悟體會,著有《四圣心源》,諸多章目共譜“土樞四象、一氣周流”之理,執(zhí)簡馭繁,舉綱目張,在臨床應(yīng)用中可有效地指導(dǎo)疾病的辨證論治。多囊卵巢綜合征臨床證候復(fù)雜多樣,病機變化多端,各家對此各執(zhí)一詞,眾說紛紜,臨床不易辨治。筆者試從“一氣周流”角度入手,從最本質(zhì)的氣的層次探析多囊卵巢綜合征的病機,初步將本病病機歸為水寒、土濕、木郁,尤以水寒土濕為根本。臟腑功能失調(diào)必會導(dǎo)致氣機升降運轉(zhuǎn)失序,從而產(chǎn)生各式病理狀態(tài)。脾腎陽虛日久,導(dǎo)致陽不制陰,陰陽失衡與氣機逆亂惡性循環(huán),妄生痰濕、血瘀。病理產(chǎn)物與氣機郁滯互成鬼使,人體自難復(fù)康。故臨證診治多囊卵巢綜合征時,可依據(jù)“一氣周流”理論,溫水寒、燥脾土、疏木氣的同時,結(jié)合具體情況靈活辨證論治,修復(fù)源頭的同時處理顯現(xiàn)情況,幫助一氣運轉(zhuǎn)起來,實現(xiàn)治本與對癥共舉,使人體陰陽氣血隨著“周流之氣”按部就班地各行其道、各使其責(zé),以致土樞四象、一氣周流、陰陽相濟、氣血歸和。筆者試從“一氣周流”入手,從根本論述,以氣的角度剖析多囊卵巢綜合征的病機,研學(xué)導(dǎo)師多年臨床診療經(jīng)驗,試為臨床中多囊卵巢綜合征的辨治提供更廣闊的論治方向及思路。
[Abstract]:Huang Yuan-yu (1705-1758), a famous physician in the Qianlong period of the Qing Dynasty, highly respected classical works such as the Internal Classic, the difficult Classic, the Theory of febrile Diseases, the synopsis of the Golden Chamber, and inherited the theory of Yin and Yang, the five elements, the six Qi Theory and the Theory of Gasification, so as to integrate its essence into one. Combined with many years of clinical experience, the author of "four saints heart source", many chapters of the common spectrum of "earth pivot four images, one gas around the flow" of the principle, hold simple and complex, the program can effectively guide the clinical application of the disease dialectical treatment. The clinical syndrome of polycystic ovary syndrome is complicated and varied. The author tries to analyze the pathogenesis of polycystic ovary syndrome (PCOS) from the point of view of "one-gas circulation" and from the most essential level of qi, and initially classify the pathogenesis of the disease as water cold, soil wet, wood depression, especially water cold and soil dampness as the fundamental. The disorder of the viscera function will lead to the disorder of the ascending and descending movement of the gas engine, which will result in various pathological states. Spleen and kidney yang deficiency for a long time, leading to yang do not control yin, yin-yang imbalance and qi-qi reverse chaos vicious circle, sputum and dampness, blood stasis. Pathological products and qi stagnation mutual into ghosts, the human body is difficult to recover. Therefore, in the diagnosis and treatment of polycystic ovary syndrome, according to the theory of "one-gas circumfluence", warm water cold, dry spleen soil, thinning wood and qi, combined with specific conditions, can be treated with flexible differentiation of symptoms and signs, and at the same time, when repairing the source, it can deal with the appearing situation and help the one-gas to run. To achieve the treatment of the root causes and symptomatic treatment, so that the human body with the "circumfluence of Qi" according to their own ways, each of their responsibilities, so that the earth pivoting four elephants, one gas flow, Yin and Yang together, Qi and blood return to harmony. The author tries to analyze the pathogenesis of polycystic ovary syndrome (PCOS) from the point of view of qi from the point of view of "one gas circulation", and the experience of clinical diagnosis and treatment of polycystic ovary syndrome is many years. The aim is to provide a broader approach to the treatment of polycystic ovary syndrome (PCOS).
