【摘要】：研究目的：觀察天舒膠囊對柴苓湯治療慢性腎小球腎炎濕熱證是否存在增效作用。研究方法：選擇符合西醫(yī)診斷為慢性腎小球腎炎,中醫(yī)辨證為濕熱證的患者46例隨機分為A組(治療組)和B組(對照組)各23例。A組采用基礎治療+柴苓湯+天舒膠囊治療,B組采用基礎治療+柴苓湯治療。臨床觀察周期為8周,每4周為1療程,連續(xù)治療2個療程后,通過觀察疾病總療效、證候療效、中醫(yī)癥狀積分變化以及實驗室指標等進行評價。研究結(jié)果：疾病療效方面,A組(治療組)總有效率91.3%,B組(對照組)總有效率87.0%,經(jīng)統(tǒng)計學分析P0.05,無統(tǒng)計學差異。證候療效：A組總有效率86.9%,B組總有效率78.2%,經(jīng)統(tǒng)計學分析P0.05,說明兩組在證候療效方面存在明顯差異。實驗室指標方面,治療前后治療組與對照組在降低尿紅細胞計數(shù)(U-RBC)、尿蛋白定量(UTP)以及減少尿蛋白定性(PRO)方面均有效(P0.05),且治療組比對照組在降低尿紅細胞計數(shù)、尿蛋白定量方面更加有效(P0.05),而在降低尿蛋白定性方面無明顯差異(P0.05)。在治療過程中兩組對血肌酐(Scr)、血尿素氮(BUN)、谷丙轉(zhuǎn)氨酶(ALT)、谷草轉(zhuǎn)氨酶(AST)、血清白蛋白(ALB)、血白細胞計數(shù)(WBC)均無影響。結(jié)論：在中醫(yī)辨證論治基礎上,天舒膠囊對柴苓湯治療慢性腎小球腎炎濕熱證存在一定的增效作用。其增效作用表現(xiàn)為有利于改善患者臨床癥狀,減少痛苦,改善生活質(zhì)量；有利于消除或減少蛋白尿與血尿,有利于延緩腎小球疾病發(fā)展速度。
[Abstract]:Objective: to observe the effect of Tianshu capsule on treating chronic glomerulonephritis with damp-heat syndrome. Methods: chronic glomerulonephritis was diagnosed as chronic glomerulonephritis in accordance with western medicine. 46 patients with damp-heat syndrome were randomly divided into group A (treatment group) and group B (control group) 23 cases each. Group A was treated with basic treatment of Chailing decoction Tianshu capsule and group B was treated with basic treatment of Chailing decoction. The clinical observation period was 8 weeks, every 4 weeks as a course of treatment. After 2 consecutive courses of treatment, the total curative effect, syndromes effect, symptom integral change of TCM and laboratory index were observed and evaluated. Results: the total effective rate of group A (treatment group) was 91.3% and the total effective rate of group B (control group) was 87.0. There was no statistical difference between group A and group B (P 0.05). The total effective rate of group A was 86.9%. The total effective rate of group B was 78.2%. The statistical analysis showed that there was a significant difference in the curative effect of syndrome between the two groups. In terms of laboratory indexes, both the treatment group and the control group were effective in reducing urine red blood cell count (U-RBCU), urinary protein quantification (UTP) and urinary protein qualitative protein (PROP) before and after treatment, and the treatment group was more effective than the control group in reducing urine red blood cell count. The quantitative analysis of urinary protein was more effective than that of P0.05, but there was no significant difference in reducing the quality of urinary protein. In the course of treatment, the two groups had no effect on serum creatinine, bun, alanine aminotransferase (alt), serum albumin (Alb) and white blood cell count (WBC). Conclusion: on the basis of TCM syndrome differentiation, Tianshu capsule has a certain synergistic effect on treating chronic glomerulonephritis with damp-heat syndrome. The synergistic effect is that it can improve the clinical symptoms, reduce the pain, improve the quality of life, eliminate or reduce proteinuria and hematuria, and delay the development of glomerular diseases.
