【摘要】：目的:通過臨床流行病學(xué)調(diào)查了解冠心病不穩(wěn)定性心絞痛患者的中醫(yī)證候要素分布規(guī)律。方法:對(duì)411例住院患者經(jīng)冠脈造影確診為冠心病的冠心病不穩(wěn)定性心絞痛患者進(jìn)行病例調(diào)查,運(yùn)用描述性統(tǒng)計(jì)分析法和χ2檢驗(yàn)分析法對(duì)中醫(yī)證候要素、常見癥狀、舌脈象與冠脈造影結(jié)果的相關(guān)性進(jìn)行了分析。結(jié)果:冠心病不穩(wěn)定性心絞痛患者中醫(yī)證候要素血瘀占比例最大,占72.7%;其次為氣虛和痰濁,分別占48.4%、22.9%。分析顯示,冠狀動(dòng)脈不同病變支數(shù)間舌象、脈象,均以舌暗紅或淡暗、苔白或薄白、脈沉、細(xì)、弦為主。結(jié)論:以血瘀為核心,以氣虛和痰濁為重要致病要素為冠心病的病機(jī)所在,同時(shí)需要注意伴隨癥狀的影響,辨證施治,且冠脈病變支數(shù)與舌象、脈象之間的相關(guān)性分析差異有統(tǒng)計(jì)學(xué)意義。
[Abstract]:Objective: to investigate the distribution of TCM syndromes in patients with unstable angina pectoris. Methods: 411 inpatients with unstable angina pectoris of coronary heart disease diagnosed by coronary angiography were investigated. Descriptive statistical analysis and 蠂 2 test were used to analyze the factors of TCM syndromes and common symptoms. The correlation between tongue pulse and coronary angiography was analyzed. Results: the proportion of blood stasis of TCM syndromes in patients with unstable angina pectoris of coronary heart disease was the largest (72.7%), followed by deficiency of qi and turbidity of phlegm (48.4%) and blood stasis (22.9%), respectively. The analysis showed that the tongue image and pulse image between different branches of coronary artery were mainly dark red or light tongue, white or thin fur, heavy, thin and chord. Conclusion: taking blood stasis as the core and qi deficiency and phlegm turbidity as the important pathogenic factors are the pathogenesis of coronary heart disease. At the same time, we should pay attention to the influence of accompanying symptoms, differentiation of symptoms and treatment, and the number of coronary artery lesion branches and tongue picture. There was significant difference in correlation analysis between pulse images.
