【摘要】：新安地區(qū)明清時期產(chǎn)生了大量醫(yī)家,在繼承的基礎(chǔ)上又有所創(chuàng)新,這一時期的新安醫(yī)家進(jìn)行了整理和總結(jié)工作,不僅豐富了中醫(yī)的理論,而且擴(kuò)大了在臨床方面的運(yùn)用范圍。本文主要以新安醫(yī)家現(xiàn)存原著為主要研究對象,運(yùn)用文獻(xiàn)研究和個案分析等方法,整理、總結(jié)和挖掘新安醫(yī)家對內(nèi)科常見疫病的認(rèn)識�？疾煨掳册t(yī)家內(nèi)科疫病學(xué)術(shù)思想和臨床經(jīng)驗(yàn)研究,本研究以內(nèi)科疫病的病名為點(diǎn),以新安醫(yī)家生活年代先后為縱線,從疫病病因病機(jī)、治則治法和方藥角度,總結(jié)提煉新安醫(yī)家內(nèi)科各種疫病的學(xué)術(shù)思想和臨床經(jīng)驗(yàn),從而深入挖掘其對醫(yī)學(xué)的貢獻(xiàn)和對疫病認(rèn)識的創(chuàng)新之處。研究結(jié)果表明:新安醫(yī)家現(xiàn)存有關(guān)內(nèi)科疫病著述的醫(yī)家40位,44部著作,其中專著2部,共14卷,94篇,醫(yī)案550余則,1200余首方劑,其中自創(chuàng)方94首。新安醫(yī)家內(nèi)科疫病學(xué)術(shù)思想是在廣泛汲取先賢各家學(xué)說經(jīng)驗(yàn),并結(jié)合自己臨床實(shí)踐經(jīng)驗(yàn)的基礎(chǔ)上形成的。對于內(nèi)科疫病病因病機(jī)有所發(fā)揮創(chuàng)新,如新安醫(yī)家提出“疫邪雜氣致痢說”和“胎瘧”的概念,及“熱伏陰分致陰黃”的觀點(diǎn)。同時擴(kuò)展了治則治法的運(yùn)用范圍,如治霍亂轉(zhuǎn)筋宜溫化膀胱,導(dǎo)滯理氣,不可補(bǔ)養(yǎng)氣血的原則;顧護(hù)脾腎陽氣,立溫陽健脾治陰黃之法門,并創(chuàng)制了茵陳術(shù)附湯。如治泄大法宜溫脾補(bǔ)中,滲濕利水;顧護(hù)陰津,不可用阿膠、地黃滋膩養(yǎng)陰液之藥。如治瘧善用參術(shù)芪甘溫補(bǔ)中,顧護(hù)正氣;大補(bǔ)營衛(wèi)氣血,以疏通少陽氣機(jī)以去膜原之瘧邪等學(xué)術(shù)思想和臨床經(jīng)驗(yàn)。在一些內(nèi)科疫病論述方面新安醫(yī)家重在繼承總結(jié),并有一些發(fā)展和創(chuàng)新。新安醫(yī)家豐富了病因病機(jī)、治則治法和方藥;善于將先賢的治則治法靈活運(yùn)用臨床實(shí)踐,并形成了自己的特色,擅長扶助正氣;同時強(qiáng)調(diào)分因治療,辨證施治,并且創(chuàng)立了一些行之有效的方劑�？傊�,新安醫(yī)家在繼承前人經(jīng)驗(yàn)的基礎(chǔ)上又有所創(chuàng)新,從理論和臨床經(jīng)驗(yàn)方面豐富了內(nèi)科各種疫病學(xué),同時也為我們研究和應(yīng)用中醫(yī)疫病學(xué)說提供了多種思路和方法。
[Abstract]:During the Ming and Qing dynasties in Xin'an area, a large number of doctors were created and innovated on the basis of inheritance. In this period, the doctors of Xin'an carried out sorting and summing up work, which not only enriched the theory of traditional Chinese medicine, but also expanded the scope of clinical application. This article mainly takes the extant original work of Xin'an doctor as the main research object, using the methods of literature research and case analysis, summarizes and excavates the understanding of common diseases of internal medicine. To investigate the academic thought and clinical experience of medical blight in Xin'an, this study focuses on the name of disease in internal medicine, takes the life of Xinan doctor as the vertical line, from the perspective of etiology and pathogenesis of epidemic disease, treatment principle and treatment method, and prescription and medicine. This paper summarizes and abstracts the academic thought and clinical experience of various epidemic diseases in the internal medicine of Xin'an physician, and further excavates its contribution to medicine and the innovation of understanding of epidemic disease. The results show that there are 44 books written by 40 doctors of Xinan Medical School, including 2 monographs, 94 articles in 14 volumes, and more than 1200 prescriptions in more than 550 medical cases, among which 94 prescriptions are self-made. The academic thought of Xin-an physician's disease of internal medicine was formed on the basis of extensively absorbing the experience of the previous theories and combining with his own clinical practice experience. The etiology and pathogenesis of internal blight have been innovated, such as the concept of "dysentery caused by epidemic pathogen and mixed qi" and "fetal malaria", and the viewpoint of "Yin and yellow caused by heat, heat and yin". At the same time, the application scope of the treatment principle is expanded, such as the principle of treating cholera, such as changing tendons to warm the bladder, guiding stagnation and regulating qi, not replenishing qi and blood, protecting spleen and kidney yang qi, setting up the method of warming yang and invigorating spleen and treating yin and yellow, and creating the decoction of Yinchenshu. Such as the treatment of large-temperature spleen tonifying, seepage dampness and water; care to protect Yin, do not use Ejiao, Rehmannia glutinosa greasy Yin-nourishing liquid medicine. Academic thoughts and clinical experience such as treating malaria by making good use of Qi, Gan, warming and tonifying Qi, protecting qi and blood, and dredging Shaoyang Qi machine to remove membrane original malarial evil, etc. In some aspects of internal disease exposition, Xinan doctors focus on inheritance and summary, and there are some development and innovation. Xin'an doctors enriched the etiology, pathogenesis, principles, and prescriptions; were good at making use of the principles and methods of the sages in clinical practice flexibly, formed their own characteristics, and were good at helping the healthy and developing qi; at the same time, they emphasized the treatment of divided causes, differentiation of symptoms and signs, and treatment based on differentiation of symptoms and signs. And has created some effective prescription. In a word, Xinan doctors have innovated on the basis of inheriting their predecessors' experience. They have enriched all kinds of blight science in internal medicine from the aspects of theory and clinical experience. At the same time, they have provided many ideas and methods for us to study and apply the theory of epidemic disease of traditional Chinese medicine.
