【摘要】：目的:采用回顧性研究的方法,觀察80例益火煎配合中藥足浴干預(yù)脾腎陽虛型慢性原發(fā)性腎小球腎炎(Chronic primary glomerulonephritis CPGN)的臨床療效,為內(nèi)外合治提供臨床依據(jù)。資料與方法:搜集2015年5月-2016年11月就診于遼寧中醫(yī)藥大學(xué)附屬醫(yī)院腎內(nèi)科脾腎陽虛型CPGN 80例,隨機(jī)分為對(duì)照組(40例)和治療組(40例)。對(duì)照組均是內(nèi)科常規(guī)治療,治療組是在此基礎(chǔ)上口服溫補(bǔ)腎陽、健脾利濕之“益火煎”配合“腰痛方”中藥足浴。觀察兩組病例治療前后中醫(yī)癥狀的改善情況,以及尿蛋白(Protein Pro)、鏡檢紅細(xì)胞(Red blood cell RBC)、24小時(shí)尿蛋白定量、血肌酐(Serum creatinin e Scr)、尿素氮(Blood urea nitrogen BUN)的變化情況。結(jié)果:1.綜合療效:治療組總有效率是85%,對(duì)照組總有效率是67.5%,治療組優(yōu)于對(duì)照組(P0.05)。2.中醫(yī)癥候積分:治療組總有效率是82.5%,對(duì)照組總有效率是72.5%,治療組優(yōu)于對(duì)照組(P0.05)。單項(xiàng)中醫(yī)癥狀改善情況,食少納呆兩組無差異(P0.05),其余癥狀兩組均有差異(P0.05),其中腰膝酸軟、體倦乏力兩組差異最明顯(P0.01)。3.尿蛋白:兩組尿蛋白定性均減少,治療組優(yōu)于對(duì)照組(P0.05)。4.尿鏡檢紅細(xì)胞:兩組鏡檢紅細(xì)胞均減少,治療組優(yōu)于對(duì)照組(P0.01)。5.24小時(shí)尿蛋白定量:兩組24小時(shí)尿蛋白定量均減少,治療組優(yōu)于對(duì)照組(P0.05)。6.血肌酐、尿素氮:兩組Scr、BUN均降低,治療組優(yōu)于對(duì)照組(P0.05)。結(jié)論:益火煎配合中藥足浴的內(nèi)外合治法干預(yù)脾腎陽虛型慢性原發(fā)性腎小球腎炎,明顯改善腰膝酸軟、體倦乏力中醫(yī)癥狀,減少蛋白尿、血尿及降低血肌酐、尿素氮。
[Abstract]:Objective: to observe the clinical curative effect of 80 cases of chronic primary glomerulonephritis with spleen and kidney yang deficiency treated by Yihuo decoction combined with traditional Chinese medicine foot bath in order to provide clinical basis for internal and external treatment. Materials and methods: from May 2015 to November 2016, 80 cases of CPGN with deficiency of spleen and kidney were randomly divided into control group (n = 40) and treatment group (n = 40). The control group was treated with routine internal medicine, and the treatment group was treated with "Yihuo decoction" combined with "low back pain prescription" on the basis of oral warming and reinforcing kidney yang, invigorating spleen and dampness. To observe the improvement of TCM symptoms before and after treatment, and the changes of urine protein proton, erythrocyte red blood cell RBC 24 hour urine protein, serum creatinine, serum creatinine creatinin e creatinine, urea nitrogen and blood urea nitrogen BUN) before and after treatment. The result is 1: 1. Comprehensive curative effect: the total effective rate of the treatment group is 85%, the total effective rate of the control group is 67.5%, the treatment group is superior to the control group (P 0.05). TCM symptom score: the total effective rate of the treatment group was 82.5 and the total effective rate of the control group was 72.5. The treatment group was superior to the control group (P 0.05). There was no difference between the two groups in improving the symptoms of single TCM, but there was no difference in the other two groups, among which the waist and knee were sore and soft, and the tiredness of body was the most obvious difference between the two groups. Urine protein: the qualitative quality of urine protein in both groups was decreased, and the treatment group was superior to the control group (P 0.05). 4. Urine microscopic examination red blood cells: both groups of microscopic red blood cells decreased, the treatment group than the control group P0.01. 5.24 hours urine protein quantitative: the two groups 24 hours urine protein quantitative reduction, the treatment group is better than the control group P0.05. 6. Serum creatinine and urea nitrogen: Scrn bun decreased in both groups, and the treatment group was superior to the control group (P 0.05). Conclusion: Yihuo decoction combined with internal and external treatment of traditional Chinese medicine foot bath can obviously improve the symptoms of chronic primary glomerulonephritis with deficiency of spleen and kidney yang, reduce proteinuria, blood urine and blood creatinine, urea nitrogen.
