【摘要】：目的:本課題通過(guò)對(duì)齊刺結(jié)合溫針灸治療風(fēng)寒濕型項(xiàng)背肌筋膜炎的臨床研究,客觀評(píng)價(jià)齊刺結(jié)合溫針灸治療風(fēng)寒濕型項(xiàng)背肌筋膜炎的臨床療效,為傳統(tǒng)內(nèi)經(jīng)刺法在臨床應(yīng)用推廣提供依據(jù)。方法:共治療70例,將符合診斷標(biāo)準(zhǔn)、納入標(biāo)準(zhǔn)的受試者按隨機(jī)、對(duì)照的方法分為治療組和對(duì)照組,每組各35例。治療組采用齊刺配合溫針灸治療,選取2-4個(gè)阿是穴,運(yùn)用齊刺法,針刺得氣后留針,將長(zhǎng)度為1.0cm的艾條套在齊刺主針的針柄上捏緊,患者感覺(jué)溫?zé)釙r(shí)再用硬紙墊在齊刺主針的針身之下,每穴各灸3壯;對(duì)照組予口服雙氯芬酸鈉雙釋放腸溶膠囊對(duì)癥治療。6天為一療程,治療2個(gè)療程,療程間隔1天。選擇項(xiàng)背部疼痛、壓痛和功能活動(dòng)作為主要觀察指標(biāo),分別采用視覺(jué)模擬評(píng)分法(VAS)、壓痛積分法、功能障礙積分法進(jìn)行療效評(píng)價(jià),運(yùn)用Sp SS19.0軟件對(duì)所收集的數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析。結(jié)果:療程結(jié)束后,兩組的VAS、壓痛積分、功能障礙積分與本組治療前比較,差異均具有統(tǒng)計(jì)學(xué)意義(p0.05);治療組總有效率為96.88%,對(duì)照組總有效率為83.87%,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(p0.05)。兩組治療期間無(wú)不良事件發(fā)生。結(jié)論:1、治療組和對(duì)照組的治療均可有效地改善項(xiàng)背肌筋膜炎患者的臨床癥狀。2、齊刺結(jié)合溫針灸對(duì)于風(fēng)寒濕型項(xiàng)背肌筋膜炎的綜合療效、鎮(zhèn)痛療效和改善功能活動(dòng)療效均明顯優(yōu)于對(duì)照組,且療效確切,具有一定優(yōu)勢(shì)。
[Abstract]:Objective: to evaluate the clinical effect of acupuncture combined with warm acupuncture on the treatment of rheumatism and dampness nape dorsal myofascitis in patients with wind-cold and dampness type of nape dorsal myofascitis, and to evaluate objectively the clinical effect of acupuncture combined with warm acupuncture on the treatment of wind-cold and dampness type. To provide the basis for the traditional needling method in clinical application. Methods: a total of 70 patients were randomly divided into treatment group (n = 35) and control group (n = 35). In the treatment group, 2 to 4 Ashi points were selected, and Qi was used to keep the needle after acupuncture, and the 1.0cm was applied to the needle handle of the main needle, and the acupuncture group was treated with acupuncture combined with warm acupuncture and moxibustion, 2 to 4 Ashi points were selected, and the acupuncture method was used to keep the needle in place after the Qi was punctured. When the patients felt warm, they were treated with hard paper pad under the acupuncture body of the main acupuncture needle, each acupoint with 3 strong moxibustion points; the control group was given oral diclofenac sodium double release enteric-coated capsule as a course of treatment for 6 days and two courses of treatment with an interval of 1 day. The pain, tenderness and functional activity were selected as the main observation indexes. Visual analogue scoring method was used to evaluate the curative effect of VASA, tenderness integral method and functional disorder integral method. The collected data were statistically analyzed by Sp SS19.0 software. Results: after the course of treatment, the VAS, tenderness score and dysfunction score of the two groups had statistical significance compared with those before treatment, the total effective rate of the treatment group was 96.88 and the total effective rate of the control group was 83.87. The difference between the two groups was statistically significant (p 0.05). No adverse events occurred during treatment in both groups. Conclusion the treatment group and control group can effectively improve the clinical symptoms of patients with nape dorsal fasciitis. The analgesic effect and the effect of improving functional activity were better than that of the control group, and the curative effect was definite and had certain advantages.
