【摘要】：目的:觀察滋陰補腎法防治腎病綜合征患者糖皮質(zhì)激素性骨質(zhì)疏松癥(glucocorticoid-induced osteoporosis,GIOP)的臨床療效,并探討左歸丸在防治本病方面的作用機制。方法:將64例符合納入標準的GIOP患者,依照隨機數(shù)列表分為治療組和對照組,組內(nèi)病例數(shù)均為32例,均按照西醫(yī)基礎(chǔ)治療方案治療,對照組加服碳酸鈣D3(1次1片,1日2次,三餐后嚼服),治療組加用碳酸鈣D3及左歸丸加減(一次200ml,每日兩次)治療,療程為6個月。分別觀察各組在治療前后臨床癥狀體征,并檢測各項化驗室指標以及骨密度的變化。6個月研究觀察結(jié)束,統(tǒng)計分析兩組治療前后各項化驗指標及癥狀積分的變化。結(jié)果:研究初期兩組各有32例患者,研究結(jié)束有60例患者完成為期6個月觀察研究,各出現(xiàn)兩例脫落病例。兩組在患者個人基本信息方面的差別不存在明顯差異性;研究結(jié)束時治療組腎病綜合征GIOP的總有效率(93.3%)明顯超過對照組(83.3%),具有統(tǒng)計學(xué)意義(P0.05);兩組患者在研究結(jié)束后于各項相關(guān)生化指標及骨密度等方面比較,均較前改善(P0.05),兩組患者治療后相比,治療組對于血鈣、甲狀旁腺激素、骨密度等的改善作用表現(xiàn)超過對照組(P0.05)。兩組患者在中醫(yī)臨床癥狀體征方面,治療組對GIOP患者改善情況明顯好于對照組(P0.05)。結(jié)論:左歸丸配合基礎(chǔ)治療方案對于腎病綜合征GIOP患者臨床癥狀、體征有明顯改善作用,同時對于改善患者各項相關(guān)生化指標及骨密度都有著明顯影響,從而對腎病綜合征GIOP患者預(yù)后產(chǎn)生積極影響。
[Abstract]:Objective: to observe the clinical effect of nourishing yin and tonifying kidney in prevention and treatment of glucocorticoid-induced osteoporosisGIOP (GIOP) in patients with nephrotic syndrome and to explore the mechanism of Zuogui Pill in preventing and treating this disease. Methods: 64 patients with GIOP were divided into treatment group and control group according to the random number list. 32 cases in the group were treated in accordance with the basic treatment plan of western medicine, while the control group was treated with calcium carbonate D _ 3O _ 3 once a time, twice a day. The treatment group was treated with calcium carbonate D3 and Zuo Gui Wan (200ml, twice a day) for 6 months. The clinical symptoms and signs were observed before and after treatment, and the changes of laboratory indexes and bone mineral density were detected. After 6 months' observation, the changes of the laboratory indexes and symptom scores before and after treatment were statistically analyzed. Results: at the beginning of the study, there were 32 patients in each group, and 60 patients completed the study for 6 months. There was no significant difference between the two groups in the basic information of patients. At the end of the study, the total effective rate of GIOP in the treatment group was significantly higher than that in the control group (83.3%, P 0.05). After treatment, the improvement of serum calcium, parathyroid hormone and bone mineral density in the treatment group was higher than that in the control group (P 0.05). In terms of clinical symptoms and signs of TCM, the improvement of GIOP in the treatment group was better than that in the control group (P 0.05). Conclusion: Zuo Gui Pill combined with basic treatment can significantly improve the clinical symptoms and signs of patients with nephrotic syndrome (GIOP), at the same time, it has a significant effect on the improvement of the related biochemical indexes and bone mineral density in patients with nephrotic syndrome. Therefore, the prognosis of patients with nephrotic syndrome (GIOP) was positively affected.
