【摘要】：目的:探討術前30min經(jīng)皮穴位電刺激對行下肢手術的老年患者體溫的影響,觀察穴位電刺激下循環(huán)系統(tǒng)、血糖、術中寒戰(zhàn)的變化。方法:將符合納入標準的60例入選病例,隨機分為觀察組和對照組,每組30例(n=30)。觀察組患者術前30min用韓式經(jīng)皮穴位電刺激儀對百會穴、大椎穴、健側(cè)足三里穴進行疏密波電刺激,持續(xù)到手術結(jié)束。對照組不給予穴位經(jīng)皮電刺激。記錄并比較手術中不同時間點兩組患者的鼻咽溫度、心率、血壓、血糖,及術中出血量、寒戰(zhàn)情況。結(jié)果:觀察組患者鼻咽溫度下降幅度、血糖下降幅度、低體溫發(fā)生率、低血糖發(fā)生率、寒戰(zhàn)發(fā)生率、寒戰(zhàn)程度均明顯低于對照組(P0.05);觀察組患者術中血壓、心率波動、失血量小于對照組,但差異無統(tǒng)計學意義(P0.05)。結(jié)論:經(jīng)皮穴位電刺激應用在老年患者下肢手術中能較好的維護體溫穩(wěn)定,降低術中寒戰(zhàn)的發(fā)生率;降低循環(huán)波動及減輕應激反應,有利于手術過程安全、平穩(wěn)。
[Abstract]:Objective: to investigate the effect of preoperative 30min percutaneous acupoint electrical stimulation on body temperature of elderly patients undergoing lower extremity operation, and to observe the changes of circulatory system, blood glucose and shivering during operation. Methods: sixty patients who met the inclusion criteria were randomly divided into observation group and control group with 30 cases in each group. In the observation group, 30min was used to stimulate Baihui point, Dazhui point and healthy side Zusanli point with Korean transcutaneous acupoint electric stimulation instrument before operation, and the stimulation lasted until the end of operation. The control group was not given acupoint percutaneous electrical stimulation. The nasopharyngeal temperature, heart rate, blood pressure, blood sugar, intraoperative bleeding and shivering were recorded and compared. Results: the decrease of nasopharyngeal temperature, blood glucose, hypothermia, hypoglycemia, shivering, shivering degree in the observation group were significantly lower than those in the control group (P 0.05), the blood pressure and heart rate fluctuated during operation in the observation group. The amount of blood loss was lower than that of the control group, but the difference was not statistically significant (P 0.05). Conclusion: the application of percutaneous acupoint electrical stimulation in lower extremity surgery of elderly patients can better maintain the stability of body temperature, reduce the incidence of shivering during operation, reduce the fluctuation of circulation and relieve stress reaction, which is beneficial to the safety and stability of the operation process.
