【摘要】：目的:通過對比觀察自擬化痰通絡(luò)湯治療冠心病穩(wěn)定型心絞痛(痰瘀互阻證)患者治療前后西雅圖心絞痛量表積分、中醫(yī)證候積分、心絞痛發(fā)作情況、硝酸甘油用量情況的變化,探討其臨床療效。方法:本臨床觀察共納入符合入組標(biāo)準(zhǔn)的患者40例,均來自2016年3月-2016年12月在黑龍江中醫(yī)藥大學(xué)附屬第一醫(yī)院心內(nèi)科門診就診的患者。采用治療前后自身對照的方法,患者入組后予自擬化痰通絡(luò)湯150ml/次,2次/日,用藥療程4周。記錄并統(tǒng)計(jì)對比患者治療前后的西雅圖心絞痛量表、中醫(yī)證候積分、心絞痛發(fā)作情況、硝酸甘油用量情況的變化。結(jié)果:1.治療后患者西雅圖心絞痛量表中軀體活動(dòng)受限程度、心絞痛穩(wěn)定狀態(tài)、心絞痛發(fā)作情況、治療滿意程度均較治療前明顯改善,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。2.治療后患者中醫(yī)證候積分較治療前明顯下降,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。3.治療后患者心絞痛積分較治療前顯著降低,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。4.治療后患者硝酸甘油停減率為84.21%,療效顯著。結(jié)論:1.自擬化痰通絡(luò)湯能有效改善冠心病穩(wěn)定型心絞痛(痰瘀互阻證)患者的軀體活動(dòng)受限程度、心絞痛穩(wěn)定狀態(tài)、心絞痛發(fā)作情況、降低硝酸甘油的使用率,并提高患者對疾病治療的滿意程度。2.自擬化痰通絡(luò)湯能有效改善冠心病穩(wěn)定型心絞痛(痰瘀互阻證)患者的中醫(yī)證候表現(xiàn)。
[Abstract]:Objective: to observe the score of Seattle angina scale, TCM syndrome score and angina pectoris attack before and after treatment of stable angina pectoris (phlegm and blood stasis mutual obstruction syndrome) patients with coronary heart disease treated with Huatan Tongluo decoction. The change of nitroglycerin dosage and its clinical effect were discussed. Methods: a total of 40 patients, all from March 2016 to December 2016, were enrolled in the clinical observation. They were admitted to the Department of Cardiology of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine from March 2016 to December 2016. Before and after treatment, the patients were treated with 150ml/ twice a day, and the course of treatment was 4 weeks. The changes of Seattle angina scale, TCM syndrome score, angina pectoris attack and nitroglycerin dosage before and after treatment were recorded and compared statistically. The result is 1: 1. After treatment, the degree of restricted somatic activity, stable state of angina pectoris, angina pectoris attack and satisfactory degree of treatment in the patients with Seattle angina pectoris scale were significantly improved compared with those before treatment, and the difference was statistically significant (P 0.05). After treatment, the score of TCM syndromes in patients was significantly lower than that before treatment, and the difference was statistically significant (P 0.05). 3. After treatment, the score of angina pectoris was significantly lower than that before treatment, and the difference was statistically significant (P 0.05. 4). After treatment, the reduction rate of nitroglycerin was 84.21 and the curative effect was remarkable. Conclusion 1. Self-made Huatan Tongluo decoction can effectively improve the degree of restricted physical activity, stable state of angina pectoris, angina pectoris attack and decrease the utilization rate of nitroglycerin in patients with stable angina pectoris of coronary heart disease (phlegm and blood stasis mutual obstruction syndrome). And to improve the patient's satisfaction with the treatment of the disease. 2. Self-made Huatan Tongluo decoction can effectively improve the TCM syndromes of patients with stable angina pectoris of coronary heart disease (phlegm and blood stasis mutual obstruction syndrome).
