【摘要】：目的:本課題通過拔罐結(jié)合針刺與常規(guī)針刺治療慢性濕疹改善瘙癢癥狀臨床療效的對比,旨在優(yōu)化臨床治療思路,為針刺配合拔罐療法治療慢性濕疹的臨床應(yīng)用推廣提供證據(jù)支持和理論依據(jù)。方法:采用簡單隨機化的方法將符合納入標(biāo)準(zhǔn)的60例慢性濕疹患者按照1:1分為治療組30例,對照組30例。治療組采用拔罐(肩耦穴、血海穴、日月期門穴、神闕穴以及環(huán)跳穴)+毫針針刺(定神針、丘墟、神門、曲池、足三里、三陰交、陰陵泉、皮損局部)+溫針灸氣海穴;對照組采用單純針刺治療(定神針、丘墟、神門、曲池、足三里、三陰交、陰陵泉、皮損局部)+溫針灸氣海穴。拔罐,可視罐印顏色的深淺,每周治療1-2次;針刺每周治療2次。4周為1療程,治療2個療程。每個療程結(jié)束后,對慢性濕疹瘙癢積分以及皮膚病生活質(zhì)量量表(DLQI)積分與治療前積分進行比較分析。結(jié)果:60例受試者中,依據(jù)剔除、脫落標(biāo)準(zhǔn)剔除3例。治療組實為29例,對照組實為28例。通過瘙癢積分計算出療效率,治療1個療程后,治療組的總有效率為79.31%,對照組的總有效率為75.00%,經(jīng)秩和檢驗,P0.05,差異有統(tǒng)計學(xué)意義,說明短期的治療效果治療組較對照組明顯;治療2個療程后,治療組的總有效率為96.55%,對照組的總有效率為89.29%,經(jīng)秩和檢驗,P0.05,差異有統(tǒng)計學(xué)意義,說明長期的治療效果治療組較對照組明顯。治療組的DLQI積分差為9.34±2.43,對照組DLQI積分差值為6.96±2.01。經(jīng)秩和檢驗,P0.05,差異有統(tǒng)計學(xué)意義。說明在改善生活方面,治療組的效果比對照組效果明顯。結(jié)論:針灸治療慢性濕疹總體療效明確,結(jié)合拔罐后治療效果更顯著。研究結(jié)果表明,拔罐結(jié)合針刺治療慢性濕疹在瘙癢癥狀減輕方面效果突出,在生活質(zhì)量改善方面的療效亦明顯。這種方法可以明顯減少服用藥物的副作用,值得臨床推廣。
[Abstract]:Objective: to compare the clinical effects of cupping combined with acupuncture and conventional acupuncture in the treatment of chronic eczema. To provide evidence and theoretical basis for clinical application of acupuncture combined with cupping therapy in the treatment of chronic eczema. Methods: 60 patients with chronic eczema were randomly divided into treatment group (30 cases) and control group (30 cases) according to 1:1. The treatment group was treated with cupping (shoulder coupling point, Xuehai point, sun and moon period gate point, Shenque point and Huanqiu point) and acupuncture at warm air sea point (Ding Shen acupuncture, Qiu Hui, Shenmen, qu Chi, Zusanli, Sanyinjiao, Yin Ling Quan, local lesions). The control group was treated with pure acupuncture (Ding Shen acupuncture, Qiu Hui, Shenmen, qu Chi, Zusanli, Sanyinjiao, Yin Lingquan, local lesions). Cupping, visible ink color, 1-2 times a week, acupuncture treatment twice a week for 4 weeks as a course of treatment, 2 courses of treatment. After each course of treatment, the scores of chronic eczema pruritus and dermatological quality of life scale (DLQI) were compared with those before treatment. Results among 60 subjects, 3 cases were excluded according to elimination and 3 cases by shedding criterion. There were 29 cases in the treatment group and 28 cases in the control group. After one course of treatment, the total effective rate of the treatment group was 79.31 and that of the control group was 75.000.The difference was statistically significant by rank sum test (P 0.05), indicating that the short-term therapeutic effect in the treatment group was more obvious than that in the control group. After two courses of treatment, the total effective rate of the treatment group was 96.555.The total effective rate of the control group was 89.29. The difference was statistically significant by rank sum test (P0.05), indicating that the long-term therapeutic effect in the treatment group was more obvious than that in the control group. The difference of DLQI score was 9.34 鹵2.43 in the treatment group and 6.96 鹵2.01in the control group. The difference was statistically significant by rank sum test (P 0.05). It showed that the effect of the treatment group was better than that of the control group. Conclusion: the total curative effect of acupuncture and moxibustion on chronic eczema is clear, especially after cupping. The results showed that cupping combined with acupuncture was effective in relieving pruritus and improving quality of life. This method can significantly reduce the side effects of taking drugs, and is worthy of clinical promotion.
