【摘要】：目的:觀察有胚胎停育史患者再次妊娠早期情志"思""恐"與血HCG增長速度及妊娠結(jié)局之間的關(guān)系,從情志角度探討影響胚胎生長發(fā)育的可能危險因素;評價中藥聯(lián)合心理療法對妊娠結(jié)局的影響程度。方法:收集2015年9月至2016年12月于黑龍江中醫(yī)藥大學(xué)附屬第一醫(yī)院婦科門診就診的具有胚胎停育史并再次妊娠的患者,并對這些患者進行焦慮情況測評調(diào)查,收集符合入組標(biāo)準(zhǔn)的患者60例。治療組30例,采用中藥聯(lián)合心理治療;對照組30例,采用單純中藥治療。觀察干預(yù)前后患者血HCG增長速度、焦慮狀態(tài)改變情況及兩組患者妊娠結(jié)局。結(jié)果:1.具有胚胎停育史的焦慮孕婦孕早期血HCG增長速度明顯低于正常水平;2.干預(yù)后兩組患者的SAS評分較干預(yù)前有所改善,差異有統(tǒng)計學(xué)意義;3.干預(yù)后患者的SAS評分與血HCG增長速度及妊娠結(jié)局呈負(fù)向相關(guān);4.干預(yù)后治療組SAS評分下降幅度及血HCG增長速度明顯大于對照組,差異有統(tǒng)計學(xué)意義;5.干預(yù)后治療組的妊娠結(jié)局明顯優(yōu)于對照組,差異有統(tǒng)計學(xué)意義。結(jié)論:1.情志可以影響胚胎停育患者再次妊娠早期血HCG的增長;2.情志"思" "恐"有可能是有胚胎停育史患者再妊娠結(jié)局的影響因素之一;3.中藥調(diào)志安胎飲聯(lián)合心理療法可明顯改善患者的焦慮狀態(tài),提高妊娠成功率。
[Abstract]:Objective: to observe the relationship between early pregnancy mood "thought" and blood HCG growth rate and pregnancy outcome in patients with history of embryo suspension, and to explore the possible risk factors affecting embryo growth and development from the emotional point of view.To evaluate the effect of traditional Chinese medicine combined with psychotherapy on pregnancy outcome.Methods: from September 2015 to December 2016, the patients who had the history of embryo suspension and re pregnancy were collected from the gynecological outpatient clinic of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine, and the anxiety status of these patients was investigated.A total of 60 patients who met the admission criteria were collected.The treatment group (30 cases) was treated with traditional Chinese medicine combined with psychological therapy, and the control group (30 cases) with traditional Chinese medicine alone.Blood HCG growth rate, anxiety state and pregnancy outcome were observed before and after intervention.The result is 1: 1.The growth rate of serum HCG in anxious pregnant women with a history of fetal arrest was significantly lower than that of normal pregnant women (2. 2%) in the first trimester of pregnancy.After intervention, the SAS scores of the two groups were improved compared with those before the intervention, and the difference was statistically significant (P < 0.05).After intervention, the SAS score was negatively correlated with the blood HCG growth rate and pregnancy outcome.After intervention, the decrease of SAS score and the increase rate of blood HCG in the treatment group were significantly higher than those in the control group (P < 0.05).The pregnancy outcome of the treatment group was significantly better than that of the control group after intervention, and the difference was statistically significant.Conclusion 1.Emotion can affect the increase of HCG in the early pregnancy of embryo suspensions.Emotion "thinking" and "fear" may be one of the influencing factors of re-pregnancy outcome in patients with history of embryo termination.Traditional Chinese medicine Tiaozhi Antian decoction combined with psychotherapy can obviously improve the anxiety state of the patients and increase the success rate of pregnancy.
