【摘要】：目的:研究女性高血壓患者中醫(yī)證候分類及與臨床理化指標(biāo)的規(guī)律,為臨床辨治女性高血壓提供依據(jù)。方法:采用改良德爾菲法獲取女性高血壓中醫(yī)證候分類專家意見。調(diào)查2013年1月-2015年11月間的357例女性高血壓患者,獲取一般資料、中醫(yī)診斷、血漿同型半胱氨酸、腎素-血管緊張素-醛固酮系統(tǒng)相關(guān)腎上腺皮質(zhì)類激素水平等檢查資料。對(duì)收集到的數(shù)據(jù)進(jìn)行統(tǒng)計(jì)學(xué)分析,得出結(jié)論。結(jié)果:女性高血壓的常見證型有陰虛陽亢證、痰瘀互結(jié)證、肝火上炎證、肝氣郁結(jié)證、沖任不足證。357例患者年齡平均65.6歲,病程平均12.04年,血壓以Ⅱ、Ⅲ級(jí)居多。344例患者Hcy平均為14.287μmol/L。288例患者的血漿同型半胱氨酸≥10μmol/L,占83.72%。痰熱內(nèi)閉證血漿同型半胱氨酸最高,且與其他證候比較有統(tǒng)計(jì)學(xué)差異。133例患者腎素活性平均為2.85 ng/ml/hr,血管緊張素Ⅰ(37℃)平均為5.28ng/ml,血管緊張素Ⅰ(4℃)平均為1.26ng/ml。38例患者腎素定量平均為13.09 pg/ml,血管緊張素Ⅱ平均為150.75 pg/ml。171例患者醛固酮定量平均為205.123pg/ml。中醫(yī)證候間腎素-血管緊張素-醛固酮系統(tǒng)無統(tǒng)計(jì)學(xué)差異。血漿同型半胱氨酸與腎素-血管緊張素-醛固酮系統(tǒng)無明顯直線相關(guān)性。結(jié)論:1.女性高血壓以陰虛陽亢證、痰瘀互結(jié)證、肝火上炎證、肝氣郁結(jié)證、沖任不足證5種證型為常見證型。2.女性高血壓患者中H型高血壓所占比例居多。痰熱內(nèi)閉證血漿同型半胱氨酸水平最高。血漿同型半胱氨酸水平升高與“痰”、“瘀”、“虛”3大病因有關(guān)。3.女性高血壓腎素-血管緊張素-醛固酮系統(tǒng)大部分處于正常水平,在各中醫(yī)證候分型間無明顯差異。4.血漿同型半胱氨酸與腎素-血管緊張素-醛固酮系統(tǒng)無明顯直線相關(guān)性。
[Abstract]:Objective: to study the classification of TCM syndromes and clinical physical and chemical indexes in female patients with hypertension, and to provide evidence for clinical diagnosis and treatment of female hypertension.Methods: the modified Delphi method was used to obtain the expert opinion of TCM syndrome classification of female hypertension.357 female patients with hypertension from January 2013 to November 2015 were investigated. General data, diagnosis of traditional Chinese medicine, plasma homocysteine, renin-angiotensin-aldosterone system related adrenocortical hormone levels were obtained.Statistical analysis of the collected data, draw a conclusion.Results: the common syndromes of female hypertension were yin deficiency and yang hyperactivity syndrome, phlegm and blood stasis syndromes, liver fire inflammation syndrome, liver qi stagnation syndrome. The average age of 357 women with insufficient blood pressure was 65.6 years, the average course of disease was 12.04 years, and the blood pressure was 鈪，
