【摘要】：目的觀察研究使用分段拔伸法治療腰椎間盤突出的臨床療效,以期為提高治療該病臨床療效尋找到新的方法。方法選取符合納入標(biāo)準(zhǔn)的門診及住院患者60例,采用完全隨機(jī)對照實(shí)驗(yàn)方法設(shè)計(jì),遵循隨機(jī)分組原則,將住院患者共分為觀察組和對照組各30例。觀察組施行分段拔伸法結(jié)合傳統(tǒng)推拿法;對照組施行單純傳統(tǒng)推拿法。兩組實(shí)驗(yàn)均治療均為1日1次,7日為1療程,治療期均為2療程。并對兩組患者治療前后的腰腿部自主疼痛、JOA(臨床癥狀、體征、日常生活動作)、ODI等各方面評分進(jìn)行統(tǒng)計(jì)分析,并且對治療結(jié)束后8周的隨訪復(fù)發(fā)率進(jìn)行統(tǒng)計(jì)分析從而得出結(jié)論。結(jié)果兩組患者治療后腰腿部自主疼痛經(jīng)統(tǒng)計(jì)分析t=5.39,p=0.000.05,具有顯著差異;經(jīng)統(tǒng)計(jì)分析兩組患者治療后JOA評分對比t=2.17,p=0.030.05,具有顯著差異;兩組患者治療后ODI評分對比,t=2.08,P=0.040.05,具有顯著差距;治療后的總療效經(jīng)秩和檢驗(yàn)分析,X2=10.36,0.01P0.025,表明兩組患者有效率存在差異。結(jié)論使用分段拔伸法結(jié)合傳統(tǒng)推拿法在改善患者腰腿自主疼痛感覺、臨床癥狀、體征、日常生活能力等方面具有顯著療效;使用分段拔伸法結(jié)合傳統(tǒng)推拿法在改善患者腰腿自主疼痛感覺、臨床癥狀、體征、日常生活能力等方面療效優(yōu)于單純傳統(tǒng)推拿法;使用分段拔伸法結(jié)合傳統(tǒng)推拿法在改善患者腰腿自主疼痛感覺、臨床癥狀、體征、日常生活能力等方面具有顯著的遠(yuǎn)期療效,且優(yōu)于單純傳統(tǒng)推拿法。所依據(jù)的經(jīng)筋理論與肌筋膜線理論使治療更有科學(xué)性及針對性,值得臨床研究與推廣。
[Abstract]:Objective to observe the clinical effect of segmental pull-out method in the treatment of lumbar disc herniation in order to find a new method to improve the therapeutic effect.Methods A total of 60 outpatients and inpatients who met the inclusion criteria were divided into observation group (n = 30) and control group (n = 30).The observation group was treated with traditional massage method and the control group was treated with simple traditional massage method.Both groups were treated once a day and 7 days as a course of treatment.The scores of JOAA (clinical symptoms, physical signs, ADL and ODI) before and after treatment were statistically analyzed, and the recurrence rate of 8 weeks after treatment was statistically analyzed to reach a conclusion.Results there was a significant difference between the two groups in the treatment of Lumbar and leg spontaneous pain by statistical analysis, the JOA scores of the two groups were significantly different after the treatment, the ODI scores of the two groups were significantly different after treatment, and the ODI scores of the two groups were significantly different from those of the two groups, and there was a significant difference between the two groups in the ODI scores after treatment, and there was a significant difference between the two groups in the ODI scores of the two groups after treatment, and there was a significant difference between the two groups in the ODI scores after treatment.The total curative effect after treatment was analyzed by rank sum test, indicating that there was a difference in the effective rate between the two groups.Conclusion the method of segmental stretching combined with traditional massage is effective in improving the feeling, clinical symptoms, physical signs and daily living ability of patients with spontaneous pain of waist and leg.The use of segmental stretching combined with traditional massage is superior to traditional massage in improving the feeling, clinical symptoms, signs and daily living ability of patients with spontaneous pain of waist and leg.The use of piecewise pull-out and extension combined with traditional massage has significant long-term effects in improving the patients' sense of spontaneous pain, clinical symptoms, signs and daily living ability, and is superior to traditional massage alone.The meridian tendon theory and the myofascial line theory make the treatment more scientific and targeted, which is worthy of clinical study and popularization.
