【摘要】：目的:客觀評(píng)價(jià)國(guó)家級(jí)名老中醫(yī)黃建業(yè)教授經(jīng)驗(yàn)方“益氣固表湯”聯(lián)合健康管理治療肺脾氣虛型小兒反復(fù)呼吸道感染(Recurrent Respiratory Tract Infections RRTI,又叫“復(fù)感兒”)的有效性、安全性,為推廣應(yīng)用名老中醫(yī)經(jīng)驗(yàn)方及本病的健康管理理念奠定基礎(chǔ)。方法:采用隨機(jī)單盲對(duì)照方法,將符合診斷、納入標(biāo)準(zhǔn)的60例患兒按1:1:1的比例分配至玉屏風(fēng)顆粒組(對(duì)照組)、益氣固表湯組(治療1組)、益氣固表湯+健康管理組(治療2組),每組各20例。對(duì)照組口服玉屏風(fēng)顆粒,治療1組口服益氣固表湯,治療2組口服益氣固表湯的同時(shí)給予健康管理,治療7天為一療程,連續(xù)治療4個(gè)療程;同時(shí)三組均給予穴位貼敷4次。在治療前、治療后第7天、第14天、第21天、28天,各觀察一次,記錄在病例觀察表(CRF表)中。療程結(jié)束后第6個(gè)月隨訪1次。采用SPSS16.0統(tǒng)計(jì)軟件對(duì)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析,比較三組治療前人口基線、疾病基線與總積分、主次癥積分,評(píng)價(jià)三組治療后疾病綜合療效、中醫(yī)證候療效、總積分、主次癥與單一癥候積分等等,并對(duì)部分病例治療前后的免疫功能進(jìn)行比較分析。結(jié)果:1.治療后三組綜合療效、中醫(yī)證候綜合療效比較,差異有統(tǒng)計(jì)學(xué)意義(P均0.05),益氣固表湯組及益氣固表湯+健康管理組療效均優(yōu)于玉屏風(fēng)顆粒對(duì)照組。2.三組治療第14天、21天、28天后中醫(yī)證候總積分時(shí)點(diǎn)療效比較,差異有統(tǒng)計(jì)學(xué)意義(P均0.05);益氣固表湯組、益氣固表湯+健康管理組與玉屏風(fēng)顆粒組在治療后時(shí)點(diǎn)療效評(píng)價(jià)上,差異有統(tǒng)計(jì)學(xué)意義(P均0.05),即益氣固表湯組及益氣固表湯+健康管理組療效較比玉屏風(fēng)散顆粒組持久。4.年齡、性別、身高、體重等相關(guān)因素對(duì)本研究無(wú)影響(P均0.05)。5.三組無(wú)不良事件/反應(yīng)發(fā)生。結(jié)論:益氣固表湯、益氣固表湯+健康管理與玉屏風(fēng)顆粒組治療肺脾氣虛型反復(fù)呼吸道感染安全、有效,說(shuō)明治療RRTI時(shí)均可選用。但益氣固表湯、益氣固表湯+健康管理在顯效率、總有效率、療效穩(wěn)定性上明顯優(yōu)于玉屏風(fēng)顆粒。提示對(duì)RRTI患兒與家長(zhǎng)進(jìn)行健康管理,將有助于提高肺脾氣虛型RRTI的有效率和療效的持久性,益氣固表湯、益氣固表湯+健康管理值得推廣應(yīng)用。
[Abstract]:Objective: to evaluate objectively the efficacy and safety of "Yiqi Gubiao decoction" and "Yiqi Gubiao decoction" in the treatment of recurrent respiratory tract infection (Recurrent Respiratory Tract Infections RRTI) in children with deficiency of lung and spleen qi.It lays a foundation for popularizing and applying the famous Chinese medicine experience prescription and the health management idea of this disease.Methods: a randomized single-blind control method was used.The 60 children who were included in the standard were divided into two groups according to the proportion of 1:1:1: control group (control group), Yiqigubiao decoction group (treatment group 1), Yiqigubiao decoction health management group (treatment group 2, 20 cases in each group).The control group was treated with Yupingfeng granule, the treatment group was treated with Yiqi Gubiao decoction, the treatment group was given health management while the treatment group was treated with Yiqi Gubiao decoction, the treatment group was treated for 7 days as a course of treatment for 4 consecutive courses of treatment, and the three groups were all given acupoint application for 4 times at the same time.Before treatment, 7 days, 14 days, 21 days and 28 days after treatment, each observation was recorded in CRF.The patients were followed up 6 months after the course of treatment.The data were statistically analyzed by SPSS16.0 software. The population baseline, disease baseline and total score, primary and secondary symptom score were compared among the three groups before treatment, and the comprehensive curative effect, TCM syndromes and total score were evaluated after treatment in the three groups.The immune function of some cases before and after treatment were compared and analyzed.The result is 1: 1.After treatment, the three groups of comprehensive efficacy, TCM syndromes comprehensive efficacy comparison, the difference was statistically significant (P 0.05), Yiqi Gu Bao Tang and Yiqi Gu Bo Tang health management group were better than the control group of Yupingfeng granule. 2.There were significant differences between the three groups in the total time points of TCM syndromes after 14 days of treatment and 21 days after 28 days of treatment, the difference was statistically significant (P < 0.05), and that of the groups of Yiqi Gubiao decoction, Yiqigubiao decoction health management group and Yupingfeng granule group was evaluated at the time point after treatment.The difference was statistically significant (P < 0.05), that is, the curative effect of Yiqi Gubiao decoction group and Yiqigubiao decoction health management group was longer than that of Yupingfeng Powder group. 4.Age, sex, height, weight and other related factors had no effect on this study (P < 0.05).No adverse events / reactions occurred in the three groups.Conclusion: the health management of Yiqi Gubiao decoction and Yupingfeng granule group is safe and effective in the treatment of recurrent respiratory tract infection with deficiency of lung and spleen qi, indicating that it can be used in the treatment of RRTI.But the health management of Yiqi Gubiao decoction and Yiqi Gubiao decoction was superior to Yupingfeng granule in the obvious efficiency, the total effective rate and the curative effect stability.It is suggested that the health management of RRTI children and their parents will be helpful to improve the effective rate and the persistence of curative effect of RRTI. It is worth popularizing and applying the health management of Yiqi Gubiao decoction and Yiqi Gubiao decoction.
